Generalized Joint Laxity Increases the Risk of Recurrence of Distal Radioulnar Joint Instability after Arthroscopic Foveal Repair of the Triangular Fibrocartilage Complex.

PURPOSE: To compare the clinical results of the arthroscopic foveal repair of the triangular fibrocartilage complex (TFCC) for distal radioulnar joint (DRUJ) instability in patients with or without generalized joint laxity. METHODS: Patients who underwent arthroscopic transosseous foveal TFCC repair of Palmer 1B foveal TFCC tears (Atzei classification class II or III) from January 2018 to October 2021 were identified. Patients treated for symptomatic DRUJ instability for more than 3 months, and with at least 2 years of follow-up, were included. Patients were categorized into two groups based

Plantaris Tendon Reconstruction for Chronic Varus Instability of the Hallux Interphalangeal Joint: A Case Report.

CASE: We report a 13-year-old female karate athlete with chronic varus instability of the left hallux interphalangeal (IP) joint following injury 16 months prior. Surgical reconstruction was performed using plantaris tendon autograft with biotenodesis screws through a single lateral approach. At 1-year follow-up, the patient had complete resolution of symptoms and returned to competitive karate. CONCLUSION: Lateral collateral ligament reconstruction using plantaris tendon autograft is an effective treatment option for chronic varus instability of the hallux IP joint, offering minimal donor si

Assessment Strategies for Distal Radioulnar Joint Instability: Current State and a Need for Improved Tools.

The distal radioulnar joint (DRUJ) plays a crucial role in wrist and forearm motion by facilitating pronation and supination while transmitting forces between the ulna and carpus. The stability of the DRUJ is dependent on both bony and soft tissue structures; however, because of the variability in joint congruence during wrist motion, soft tissue stabilizers are more critical for maintaining stability. The primary static soft tissue stabilizer of the DRUJ is the triangular fibrocartilage complex. Various clinical tests, such as the ballottement test, ulnar fovea sign, and press test, are commo

Effects of anticipation on joint kinematics during inversion perturbation in individuals with chronic ankle instability.

BACKGROUND: Although chronic ankle instability (CAI) patients display altered reactive joint kinematics after inversion perturbation, little is known about the effects of anticipation on reactive joint kinematics among CAI, coper, and control groups. OBJECTIVE: To assess changes in reactive joint kinematics after different inverted landing situations including planned- and unplanned-condition among groups of CAI, coper, and control. METHODS: Sixty-six volunteers participated (22 per group). Participants completed three trials of both planned and unplanned single-leg landing onto an inverted

The use of the internal joint stabiliser for elbow instability Report of two cases and systematic review.

The management of elbow fracture-dislocations is challenging. The internal joint stabiliser (IJS) (Skeletal Dynamics, Miami, FL) has been advocated as an alternative to traditional techniques. This article shares our initial clinical experience and provide a systematic review analysing the ability of the IJS to maintain radiographic joint reduction and the associated complication profile. Two cases of elbow fracture-dislocations treated at our centre using the IJS are presented. A systematic review of the literature was conducted using the online databases Medline, Scopus and EMBASE. Clinical

Post traumatic medial instability of the MTP1 joint and ensuing surgical reconstruction: A case report.

Sport injuries of the first metatarsophalangeal joint are well described, especially with traumatic hallux valgus and turf toe reports. However, there is no description of infraclinical medial laxity and following articular disorders. We report the case of a thirty-year-old runner who suffered a sprain initially treated with retentive dressing and local injection. He developed microinstability of the first ray and quickly a bone cyst, pushing us to suggest surgical treatment. After one year of follow up after surgery, he returned to previous high-performance sport. This case highlights the pro

Kinematics and biomechanical validity of shoulder joint laxity tests as diagnostic criteria in multidirectional instability.

BACKGROUND: Clinical laxity tests are commonly used together to identify individuals with multidirectional instability (MDI). However, their biomechanical validity in distinguishing distinct biomechanical characteristics consistent with MDI has not been demonstrated. OBJECTIVE: To determine if differences in glenohumeral (GH) joint laxity exist between individuals diagnosed with multidirectional instability (MDI) and asymptomatic matched controls without MDI. METHODS: Eighteen participants (9 swimmers with MDI, 9 non-swimming asymptomatic matched controls without MDI) participated in this ob

The Examination of Repeated Self-Mobilizations With Movement and Joint Mobilizations on Individuals With Chronic Ankle Instability.

CONTEXT: Chronic ankle instability (CAI) is one of the most common chronic conditions in the world, resulting in millions of dollars contributed to the health care system. Joint mobilizations have been shown to effectively improve patient and disease-specific impairments secondary to CAI. The ability for patients to complete an effective manual therapy intervention without the need for continuous visits to a health care provider can alleviate burdens on the health care system and improve patient satisfaction. OBJECTIVE: To examine the effect of clinician-applied Maitland talocrural joint mobi

Calcaneal Apophyseal Fracture Followed by Subtalar Joint Instability in a Young Female: A Case Report.

Calcaneal apophyseal fractures are usually considered sport-related injuries. Previous repetitive microtrauma has been proposed as a predisposing factor for such injuries. However, unlike previously reported cases, in our patient, the fracture resulted from acute trauma after stepping on uneven ground. Although the first treatment option for such cases is closed reduction, most cases will require surgical treatment. Furthermore, lesions accompanying this injury have not been sufficiently considered. We report the case of a 9-year-old female with a calcaneal apophyseal fracture treated by fixat

Risk of chronic ankle instability: A reliability study on radiographic assessment of the ankle joint geometry.

BACKGROUND: Chronic ankle instability (CAI) is a multifactorial disabling condition. Ideally all factors contributing to CAI are identified and implemented in a risk assessment model. However, they need to meet strict reliability requirements. To assess usability of radiographic factors for this risk assessment model and future clinical practice, the objective of the current study was to assess the intra and inter observer reliability of three radiographic measurements. METHODS AND METHODS: The radiographs of 39 consecutive patients, at least 16years, who visited the Emergency Department afte

Current concepts in the management of recurrent anterior gleno-humeral joint instability with bone loss.

The management of recurrent anterior gleno-humeral joint instability is challenging in the presence of bone loss. It is often seen in young athletic patients and dislocations related to epileptic seizures and may involve glenoid bone deficiency, humeral bone deficiency or combined bipolar lesions. It is critical to accurately identify and assess the amount and position of bone loss in order to select the most appropriate treatment and reduce the risk of recurrent instability after surgery. The current literature suggests that coracoid and iliac crest bone block transfers are reliable for treat

[Chronic ankle joint instability: in unrecognized distal rupture of the syndosmosis and malunion of the distal fibula].

BACKGROUND: It is often difficult in the acute phase to diagnose a lesion of the distal tibiofibular syndesmosis. If this lesion is overlooked, the patient will develop an incongruity of the upper ankle joint with a pathological external rotation of the talus. The risk of a possible premature arthritis is clearly increased. OBJECTIVES: In this case study a distal rupture of the syndesmosis in a young patient was overlooked in the initial diagnostic work-up. METHODS: A search of the relevant literature and a case report. RESULTS: In the case described the shortened fibula and chronic instabi

Second MTP joint instability: grading of the deformity and description of surgical repair of capsular insufficiency.

A crossover second toe is a commonly seen forefoot problem, usually occurring in women aged > 50 years, and often in association with a bunion deformity. The plantar plate is the principal static stabilizer of the second metatarsophalangeal (MTP) joint. Different authors have proposed classifications to define instability of the second MTP joint, but only describe clinical progression of the deformity. Once a plantar plate tear has developed, conservative treatment can eliminate the symptoms and prevent progression of the deformity but cannot achieve correction or realignment of the deformity.

Post-traumatic instability of the trapeziometacarpal joint of the thumb. Diagnosis and the results of reconstruction of the beak ligament.

In 28 patients with a solitary diagnosis of instability of the trapeziometacarpal joint because of a rupture of the anterior oblique ligament, reconstruction was carried out using a slip of the tendon of flexor carpi radialis. We were able to review 26 patients. The results after a follow-up of four years seven months showed that most (87%) had significant relief from pain and symptoms. Seventeen were graded as good to excellent. The mean grip strength recovered to 86% of the contralateral side. Most patients (81%) felt that they had subjective improvement and would have undergone the operatio

Quantitative study of the quadriceps muscles and trochlear groove geometry related to instability of the patellofemoral joint.

This was a quantitative study of the major anatomical structures associated with instability of the patellofemoral joint: the quadriceps muscles and the femoral trochlear groove. The attachments of the muscles to the patella, their lines of action, and their relative sizes (physiological cross-sectional areas) were found. On the basis of the physiological cross-sectional areas, it was estimated that the central muscles-the rectus femoris and vastus intermedius-contributed 35% of the quadriceps strength, with 40% from the vastus lateralis and 25% from the vastus medialis. The vastus lateralis h
