[Traumatic knee dislocation with popliteal vascular disruption: retrospective study of 14 cases].

PURPOSE OF THE STUDY: Complex femorotibial dislocation of the knee joint generally results from high-energy trauma caused by a traffic or a contact sport accident. Besides disruption of the cruciate ligaments, in 10-25% of patients present concomitant palsy of the common peroneal nerve and more rarely disruption of the popliteal artery. The purpose of this work was to assess outcome in a monocentric consecutive series of knee dislocations with ischemia due to disruption of the popliteal artery and to focus on specific aspects of management. MATERIAL AND METHODS: This retrospective series incl

Dislocation of the rotating platform after low contact stress total knee arthroplasty.

From a one-surgeon series of 2485 patients, we report on 10 patients with rotating platform dislocation after primary Low Contact Stress total knee arthroplasty. All dislocations occurred within 2 years of the index procedure. Of the 10 patients, nine required open reduction. Five of these patients also had exchange of the original insert. One patient was treated by closed reduction. All knees were immobilized in a cast for 8 weeks. Eight of the 10 patients had no additional dislocation and at followup (average, 35 months; range, 12 months-5 years), had a stable functional joint. Two patients

Total knee arthroplasty for neglected permanent post-traumatic patellar dislocation--case report.

Permanent post-traumatic patellar dislocation is a rare entity resulting from recurrent trauma. This uncommon condition is often confused clinically with congenital dislocation of the patella. Treatment options, although not well defined, include observation, various soft tissue and bony procedures for realigning the patella and patellectomy. To our knowledge, this is the first case reporting the use of conventional non-constrained total knee arthroplasty with patellar resurfacing and soft tissue realignment procedure for long-standing patellar dislocation in a single stage. An interesting fea

Knee dislocation following anterior cruciate ligament disruption without any other ligament tears.

We report a rare case of complete knee dislocation following anterior cruciate ligament (ACL) disruption without any other ligament tears. The pathology of the knee joint was torn ACL, intact other ligaments, osteochondral fractures and bone bruise of the lateral femoral condyle, and torn lateral meniscus. In this case, osteochondral fracture resulting from the anteriorly sublaxiation of the tibia following ACL disruption was considered to prevent from spontaneous reduction. This case suggests that anterolateral knee dislocation and spontaneous reduction may occur in ACL injuries.

Rotational knee strain resulting in patellar dislocation. An experimental study in rabbits.

The right lower extremities of 64 young rabbits were immobilized by a plaster spica. The animals developed a gait pattern, which included internal tibial rotation and adduction of the left (unimmobilized) tibia. Twenty-one of the animals developed medial patellar dislocation in the unimmobilized lower extremity. The mechanism of the patellar dislocation in this experimental model was possibly overstretching of the lateral colateral ligament and the lateral side of the joint capsule, associated with medial rotation of the tibia and the tibial tubercle. The direction of patellar pull when glidin

[Permanent, habitual dislocation and recurrent dislocation of the patella in children: surgical management by patellar ligamentous transfer in 50 knees].

PURPOSE OF THE STUDY: Appropriate treatment of patellar dislocation in children and adolescents remains a subject of debate. Ligamentous transfer or "soft rod" is a technique for realigning the extensor mechanism that can be adapted to children. We analyzed results obtained in our patients. MATERIAL AND METHODS: Between 1979 and 2000, 35 children (50 knees) underwent realignment which was combined in all cases with lateral retinacular release and medial reefing using the Insall procedure. Complementary procedures were needed in certain cases (quadriceps lengthening, femoral sulcus plasty, qua

Novel Technique for Closed Reduction of an Irreducible Patella Dislocation in a Degenerative Knee.

Patellar dislocations commonly occur in young adults and often reduce spontaneously or with closed reduction in the emergency department. However, traumatic patellar dislocations in older adults pose unique challenges, often complicated by degenerative osteophytes that hinder standard closed reduction methods. When closed reduction proves ineffective, patients may necessitate arthroscopic or open surgical intervention. This case report details the experience of a 60-year-old man with a traumatic patellar dislocation resistant to standard closed reduction methods, attributed to impaction of the

Bilateral Knee Dislocations in an Active Duty Service Member: A Case Report.

This case highlights a staged operative approach for a patient who sustained bilateral knee dislocations and subsequent staged operative treatment and rehabilitation. This patient underwent bilateral multiligamentous knee reconstructions and left-sided peroneal nerve allografting because of complete peroneal nerve palsy. The initial treatment was bilateral medial collateral ligament reconstructions, posterolateral corner reconstructions, and capsular repairs with left leg peroneal nerve allografting and repair of avulsed biceps femoris tendon. The patient underwent physical therapy following t

[Open dislocation of the mobile bearing in a unicondylar knee arthroplasty. A rare complication].

Unicompartmental knee arthroplasty can be viewed as a procedure with reliable medium- to long-term success. However, complications occur. To the authors' knowledge, this is the first report on an open dislocation of the tibial component (polyethylene plate) of a mobile bearing unicondylar knee prosthesis. Twelve days after a unicompartmental knee arthroplasty a 69-year-old patient suffered an open knee trauma due to a simple fall on the ground. The clinical examination showed wound dehiscence of the former mid-line approach, ventral dislocation of the polyethylene plate and a rupture of the an

Multiple sclerosis and bilateral dislocations of total knee replacements: a case report.

We report on a 67-year-old lady with multiple sclerosis (MS) who underwent bilateral total knee replacements using a fixed bearing posterior cruciate retaining prosthesis. Having initially achieved a satisfactory result, she developed recurrent dislocations of both knees necessitating bilateral revision surgery. Such complications are not previously described as a sequela of a neurological disease.

Bilateral luxatio erecta humeri and bilateral knee dislocations in the same patient.

Bilateral knee dislocations seldom occur, and fewer than a dozen cases of bilateral luxatio erecta humeri (erect dislocation of the shoulder) have been reported. Here we present the case of a patient who sustained bilateral knee dislocations and bilateral luxatio erecta humeri in a motorcycle accident. Transient neurologic compromise resolved with closed reduction of the bilateral shoulder and left knee dislocations. To our knowledge, this is the first case report of a patient with this combination of injuries.

Dislocation of an enlarged fabella as uncommon cause of knee pain: a case report.

The fabella is a sesamoid bone that can very occasionally act as a source of atypical knee pain, often mistaken for a more common cause of clinical symptoms, such as an intra-articular loose body or an osteophyte. We report a case of symptomatic dislocation of an osteoarthritic, enlarged fabella, 4 years after total excision of the patella for a comminuted fracture, that was primarily overlooked and led to repeated diagnostic arthroscopy of the knee. A thorough history and physical examination of the joint as well as appropriate roentgenographic studies could have spared this patient an unnece

[Traumatic dislocation of the hip in a 3-year-old boy with a painful knee].

A 3-year-old boy had pain on the medial side of his left knee after his father fell on him. When the pain still persisted 4 days later, further diagnostics revealed dislocation of the hip. The pain disappeared and function was restored after closed reposition under fluoroscopic control followed by immobilisation with a plaster cast. Traumatic dislocation of the hip in childhood is a relatively rare injury, but may have far-reaching consequences. As the sensitivity to pain in children differs from that in adults, an examiner must be on the alert for a traumatic dislocation of the hip whenever a

Results after treatment of traumatic knee dislocations: a report of 26 cases.

BACKGROUND: A retrospective study was carried out with all the cases of traumatic dislocation of the knee joint treated in our institution between 1988 and 1998. METHODS: In most cases (81%), the dislocation was reduced under general anesthesia and early surgical repair of damaged structures was performed as each case required. In five cases, the treatment was conservative because of associated skeletal and visceral injuries that made immediate surgery inadvisable. RESULTS: Peroneal nerve palsy (23%) and popliteal artery disruption (7.5%) were the most frequent early complications. The most

[Isokinetic torque of the knee extensor and flexor in patients with recurrent patellar dislocation].

UNLABELLED: In order to establish the role of muscle strength in the recurrent dislocation of the patella, an isokinetic study was performed using BIODEX, by which the concentric (CC) and passive eccentric (PEC) torques of the quadriceps and hamstrings were measured. The angle velocity was set at 30 degrees and 90 degrees/sec. The subjects were divided into three groups: (I) non-surgical, (II) surgical and (III) normal control. In each group 20 knees were tested, and the following results were obtained. 1. Peak torque: At either 30 degrees/sec or 90 degrees/sec of velocity, it was significantl

Dislocation of the posterior stabilized total knee arthroplasty. A report of two cases.

This brief report details the previously unreported complication of dislocation of the posterior stabilized total knee arthroplasty. Both patients had valgus deformities requiring extensive release of the contracted lateral soft tissues. The mechanism of dislocation was one of slight flexion and external rotation. The prosthetic tibial spine became locked posterior to the femoral cam. Reduction was accomplished by applying traction and anterior translation on the tibia with the knee slightly flexed. No redislocations have occurred. A literature review is presented and the authors discuss techn
