Increased serum levels of interleukin-6 in erythema nodosum leprosum suggest its use as a biomarker.

BACKGROUND: Erythema nodosum leprosum (ENL) is a frequent complication of multibacillary leprosy that can result in significant morbidity, including peripheral nerve damage and physical disability. The identification of possible serum markers could be a valuable tool for the early detection of ENL. AIMS: The purpose of this study was to evaluate selected serum mediators involved in the innate and adaptive immune responses to identify possible immunomarkers for ENL. METHODS: The levels of interleukin-2, interleukin-4, interleukin-6, interleukin-10, interleukin-17, interferon-γ, tumor necrosis

Utility and limitations of serodiagnostic tests in monitoring the response to treatment of leprosy patients.

Simple measures that can facilitate early recognition of leprosy complications are still lacking. We therefore evaluated a lateral flow-based rapid diagnostic test and fast enzyme-linked immunosorbent assay measuring anti-LID-NDO antibody responses among leprosy cases in Cebu, Philippines. Responses were measured at diagnosis, then during and after the provision of standard multidrug therapy. Our data indicate that both platforms are highly sensitive tools for the primary diagnosis of, in particular, multibacillary leprosy. A gradual, quantifiable decline in both magnitude of response and perc

Social participation of people affected with leprosy in India: what does the literature tell us? A systematic review.

Leprosy is a public health concern in India, with significant social stigma and exclusion faced by affected individuals. Social participation is a critical component of rehabilitation and overall well-being, yet limited evidence has been synthesized regarding the associated factors. Therefore, this systematic review aimed to provide an overview of the social participation of people affected by leprosy from the existing literature in India. Scopus, PubMed and InfoLep databases were searched to identify the articles for the systematic review, which only includes studies conducted in India. The c

Accuracy of the newly manufactured multiplex XGEN MASTER LEPRAE molecular kit and the point-of-care Bioclin FAST ML FLOW test for case definition and early diagnosis of leprosy among household contacts.

BACKGROUND: Close household contact with multibacillary leprosy is the primary risk factor for disease transmission. Therefore, early detection of leprosy is crucial for disrupting the transmission chain. The primary objective of this study was to evaluate the diagnostic accuracy of the multiplex XGEN MASTER LEPRAE kit (XGEN) and the point-of-care Bioclin FAST ML FLOW Hanseníase test (PoC-ML-FLOW) among household contacts (HHCs) of leprosy index cases in midwestern Brazil. METHODS: We included HHCs of leprosy index cases. After the application of a composite reference standard comprising clin

A 35-kilodalton protein is a major target of the human immune response to Mycobacterium leprae.

The control of leprosy will be facilitated by the identification of major Mycobacterium leprae-specific antigens which mirror the immune response to the organism across the leprosy spectrum. We have investigated the host response to a 35-kDa protein of M. leprae. Recombinant 35-kDa protein purified from Mycobacterium smegmatis resembled the native antigen in the formation of multimeric complexes and binding by monoclonal antibodies and sera from leprosy patients. These properties were not shared by two forms of 35-kDa protein purified from Escherichia coli. The M. smegmatis-derived 35-kDa prot

Ulcers in leprosy patients, an unrecognized clinical manifestation: a report of 8 cases.

BACKGROUND: Leprosy is a chronic granulomatous infection caused by Mycobacterium leprae. It is a polymorphic disease with a wide range of cutaneous and neural manifestations. Ulcer is not a common feature in leprosy patients, except during reactional states, Lucio's phenomenon (LP), or secondary to neuropathies. CASES PRESENTATION: We report eight patients with multibacillary leprosy who presented specific skin ulcers as part of their main leprosy manifestation. Ulcers were mostly present on lower limbs (eight patients), followed by the upper limbs (three patients), and the abdomen (one patie

Erythema multiforme in leprosy.

The clinical course of leprosy is often interrupted by reactions, which are acute inflammatory episodes that can be classified as type I or type II. Type II reactions can present as cutaneous lesions that resemble erythema multiforme (EM). EM is classically associated with drug allergies or pre-existing viral infections. However, the differential diagnostic criteria of the diverse causative agents remain controversial. The aim of this study was to determine both the clinical relevance and the morphological and immunohistochemical characteristics of the EM-like lesions during the course of type

Myriad erythematous skin lesions, paresthesia, and anterior uveitis in a 34-year-old man.

A 34-year-old man recently immigrated from Guam to Colorado presented with a 3-year history of skin lesions and months of worsening eye irritation and vision loss. Examination revealed leonine facies, tender erythematous nodules, corneal neovascularization, and granulomatous anterior uveitis. Diagnosed with multibacillary leprosy complicated by erythema nodosum leprosum, he responded well to multidrug therapy and immunosuppressive treatment, with significant improvement in systemic symptoms, ocular inflammation, and skin lesions at follow-up.

[Current management of hand leprosy].

Leprosy, a chronic infectious disease caused by mycobacterium leprae affects an estimated 700,000 persons each year. Clinically, leprosy can be categorized as paucibacillary or multibacillary disease. Leprosy is important largely because of the deformities, disabilities and handicap it causes in a proportion of those affected by the disease. There are surgical procedures and techniques to correct or limit the deterioration of these conditions. In the past these surgical procedures were only performed in special institutions for treating leprosy and their complications. However, with the widesp

Prevalence and risk factors for hepatitis B and C viruses in patients with leprosy.

It has been reported a higher seroprevalence of HBV and HCV in leprosy patients than in the general population, but the reasons for these findings are not yet clear. On the other hand, there is evidence that these viruses may influence the onset of leprosy reactional episodes, an important cause of neurological sequelae. This study aimed to determine seroprevalence and risk factors for HBV and HCV in leprosy patients and to investigate its association with leprosy reactions. Patients attended from 2015 to 2016 at a Reference Center in Leprosy in Northeastern region of Brazil, were interviewed,

Effect of BCG on the risk of leprosy in an endemic area: a case control study.

The effect of BCG on the risk of leprosy was measured using a case-control design in an area endemic for the disease. In this study, 397 newly diagnosed cases and 669 controls matched for age, sex and locality were selected from a defined population. Information on exposure to BCG, contact with another case of leprosy, and relevant socioeconomic variables were obtained from the subjects. Having infectious (multibacillary) and noninfectious (paucibacillary) contacts in the household increased the risk of disease 11.7 times (p less than 0.001) and 2.7 times (p less than 0.001), respectively. Agranulocytosis induced by multidrug therapy in leprosy treatment: a case report.

Multidrug therapy (WHO/MDT) in multibacillary leprosy consists of treatment with rifampicin, dapsone and clofazimine. However, adverse effects can cause the patient to abandon treatment. We report on a patient who presented agranulocytosis and hemolytic anemia associated with this treatment regime. We also examined the importance of laboratory exams for diagnosis and follow-up of the patient, and for early detection of adverse effects, with a view to improving adhesion to treatment and contributing to the eradication of Hansen's disease as a public health issue.

Reproductive health issues in Pakistan; do myths take precedence over medical evidence?

OBJECTIVES: In the Philippines, there has been a lack of information on the concordance between classifications of Hansen's disease or leprosy clinically, histopathologically, and with AFS results. The study ultimately aimed to determine the concordance between the clinical diagnosis, histopathological results, and AFS results of patients with leprosy seen at the Dr. Jose N. Rodriguez Memorial Hospital and Sanitarium (DJNRMHS). METHODS: This is a descriptive, retrospective, single-center study conducted at the DJNRMHS, a tertiary government hospital and one of the last remaining sanitaria in

The curious constraints on clofazimine.

Clofazimine is a WHO-recommended therapy for multibacillary leprosy and an emerging option for drug-resistant tuberculosis and non-tuberculous mycobacterial infections. Yet in high-income countries like the USA, access remains restricted through expanded access pathways and other limitations, despite decades of safe use. This review examines clofazimine's regulatory trajectory in the USA, contrasts global access models, and highlights the consequences of relying on a single manufacturer and informal donation programmes. We argue that existing systems can be ill-suited to ensure equitable, sust

[Leprosy: control of household contacts in the municipality of Londrina-PR for a ten-year period].

The purpose of this descriptive study is to analyze variables related to leprosy patients' household contacts who received treatment in Londrina-PR-Brazil for a ten-year period. The data analysis was based on the health service's records and from a system of infectious disease. Out of 1055 leprosy's patients, it was recorded 3394 contacts with an average of 3,2. The most exposed individuals were those aged up to 40 (71,5%); son/daughter (40.6%) and husband/wife (17.8%). Of the1731 contacts (51.0%) examined, 183 showed some signs of the disease: there were 16 confirmed cases, 47 were excluded a

Grade 2 Disability in Leprosy in the Post-Elimination Era: A Case Report Highlighting Stigma and Programmatic Blind Spots.

Leprosy has achieved elimination targets in many countries; however, patients continue to present with established disability. The presence of grade 2 disability (G2D) at diagnosis reflects delayed detection and persistent gaps in surveillance. I report a family cluster of three leprosy cases identified during routine program monitoring in Telangana, India, indicating a localized transmission pocket despite elimination status. The index case, a 35-year-old male with multibacillary leprosy, presented with advanced disability, including clawing of the right hand, plantar ulceration, and bilatera

A Six Year Old Boy with Lepromatous Leprosy: A Case Report Revealing an Unsolved Mystery.

Leprosy is a chronic granulomatous disease resulting from infection with the unculturable pathogen Mycobacterium leprae. Lepromatous leprosy is the most severe form, typically seen in adults with poor immune response to the bacillus. It is exceptionally rare in young children, specially under 10 years of age due to disease's long incubation period and lower exposure. Early presentation in a child raises question about atypical transmission and host susceptibility. We report a case of a six-year-old boy from Purbadhola, Netrokona, Bangladesh with a four-year history of progressive, asymptomatic
