Anthrax vaccines.

The only impetus for the development of new anthrax vaccines is to protect humans against the intentional use of Bacillus anthracis as a bioterrorist or warfare agent. Live attenuated vaccines against anthrax in domesticated animals were among the very first vaccines developed. This was followed by the development of nonliving component vaccines leading to the eventual licensure of protein-based vaccines for human use in the 1970s. This chapter will review the recent advances in developing protein, live attenuated, and genetic vaccines against anthrax.

Vaccines for preventing Japanese encephalitis.

BACKGROUND: Vaccination is recognized as the only practical measure for preventing Japanese encephalitis. Production shortage, costs, and issues of licensure impair vaccination programmes in many affected countries. Concerns over vaccine effectiveness and safety also have a negative impact on acceptance and uptake. OBJECTIVES: To evaluate vaccines for preventing Japanese encephalitis in terms of effectiveness, adverse events, and immunogenicity. SEARCH STRATEGY: In March 2007, we searched the Cochrane Infectious Diseases Group Specialized Register, CENTRAL (The Cochrane Library 2007, Issue 1

Disseminated vaccine strain varicella as the acquired immunodeficiency syndrome-defining illness in a previously undiagnosed child.

The Food and Drug Administration licensed a live-virus varicella vaccine (Varivax; Merck & Co Inc, West Point, PA) in March 1995. Prelicensure adverse events were minimal; however, since licensure and increased vaccine use, rare previously undetected risks have arisen. Presented here is the clinical course of a previously undiagnosed, human immunodeficiency virus-infected boy who developed dissemination of the vaccine strain of varicella zoster after immunization. chickenpox, human immunodeficiency virus, pneumonia, encephalopathy, varicella vaccine, adverse events, dissemination.

Genetic counselor training: a review and considerations for the future.

The first training program for genetic counselors began in 1969. Since then a number of other programs have been developed and more than 650 individuals have graduated from these programs. This article reviews the development and current status of training opportunities for genetic counselors. Twelve programs that currently grant a master's-level degree in genetic counseling are reviewed. Other areas, such as certification and licensure, that reflect genetic counseling training or such issues of professional growth as continuing education and career advances are addressed.

The pentavalent rotavirus vaccine, RotaTeq.

Vaccination against rotavirus disease has been a global preventive health priority since rotaviruses were first discovered in the 1970s. The first licensed rotavirus vaccine, RRV-TV, was removed from the market shortly after licensure because of an unexpected serious adverse event, intussusception. A pentavalent rotavirus vaccine, PRV, is the second FDA-licensed rotavirus vaccine and does not signal the risk observed with RRV-TV. The properties and large safety study results of PRV are reviewed here.

How Does Physician Clinical Experience Affect Quality of Care, as Measured by Length of Stay?

INTRODUCTION: Length of stay (LOS) is a widely utilized metric of hospital quality and operational efficiency. Prolonged LOS is associated with increased healthcare costs, greater risk of hospital-acquired complications and infections, and reduced bed turnover, negatively affecting patient outcomes and institutional performance. Hospitalists and internal medicine physicians play an integral role in determining a patient's readiness for discharge and facilitating timely transitions of care. Examining factors that influence LOS, including physician characteristics such as clinical experience or

Better late than never: defining the ideal vaccine that targets pre-erythrocytic malaria infection.

Progress towards a much-needed malaria vaccine has accelerated in recent years. The most significant recent event was the independent licensure of the first two subunit vaccines against malaria, RTS,S/AS01 (MosquirixTM) from GSK and R21/Matrix-M from the Serum Institute of India, which received full World Health Organisation (WHO) prequalification in 2022 and 2023, respectively. Both vaccines require a primary series of four doses to confer maximum protection to children against the most virulent species of malaria caused by Plasmodium falciparum. This immunisation series, given over a 12 to 1

Overcoming Barriers to Healthcare Reform: A Call to Action.

The success of healthcare reform hinges on policymaker, regulator, and administrator actions that shape policies at various levels. These policies can either facilitate or hinder the practice of healthcare professionals and collaborative work environments. It is imperative for all healthcare professionals to fully utilize their education and certification, as fostering an equitable workplace culture is vital for retaining staff and improving access to care. Using nurse practitioners (NPs) as an exemplar, this article aims to specify systemic barriers to healthcare reform and call for policymak

Surveillance of adverse events following immunization with meningococcal group C conjugate vaccine: Tuscany, 2005-2012.

INTRODUCTION: Post-licensure vaccine safety studies are essential to identify uncommon events that may be difficult to assess during pre-licensure studies. The aim of our study was to evaluate the safety of serogroup C meningococcal conjugate (MCC) vaccine in Tuscany from 2005 to 2012. METHODS: All adverse events (AEs) to MCC vaccine notified from 2005 to 2012 were obtained from the regional health authority. RESULTS: Following 451,570 doses administered, 110 suspected AEs were notified (mean annual reporting rate: 2.8/10,000 doses). The most frequently AE reported was fever (60%), followed

Postexposure prevention of progressive vaccinia in SCID mice treated with vaccinia immune globulin.

A recently reported case of progressive vaccinia (PV) in an immunocompromised patient has refocused attention on this condition. Uniformly fatal prior to the licensure of vaccinia immune globulin (VIG) in 1978, PV was still fatal in about half of VIG-treated patients overall, with a greater mortality rate in infants and children. Additional therapies would be needed in the setting of a smallpox bioterror event, since mass vaccination following any variola virus release would inevitably result in exposure of immunocompromised people through vaccination or contact with vaccinees. Well-characteri

Translational research to assist policy decisions about introducing new vaccines in developing countries.

Few new-generation vaccines have found their way into public-health programmes for the poor in developing countries, and for those that have, delays of years or even decades after their licensure and introduction in industrialized countries have been the rule. Financial constraints and political obstacles have played major roles in delaying the introduction of the vaccines. Also contributing to this situation has been a dearth of needed research. While past analyses have identified inadequate support for conducting Phase 1 studies as an obstacle, other types of translational research are also

Barriers to dengue vaccine coverage in low- and middle-income countries (LMICs).

Dengue is a climate-sensitive infectious disease and the world's fastest-growing vector-borne disease. It disproportionately affects low- and middle-income countries (LMICs), with children most severely affected. Dengvaxia (CYD-TDV) and Qdenga (TAK-003) are the only two licensed dengue vaccines currently available, while Butantan-DV (TV003) is an advanced candidate with promising Phase 3 clinical trial results. Despite the commercial availability of dengue vaccines, coverage remains low in areas with the greatest need. This is driven by technical, social, and geopolitical challenges, as well a

Landscape analysis of invasive non-typhoidal salmonella (iNTS) disease and iNTS vaccine use case and demand: Report of a WHO expert consultation.

Invasive disease caused by non-typhoidal Salmonella serovars (iNTS) occurs with increased risk in the presence of other comorbidities such as malaria, HIV, malnutrition, anaemia and sickle cell disease. While infection with non-typhoidal (NTS) serovars often results in self-limited enterocolitis in high-income settings, in sub-Saharan Africa (SSA) where these risk-comorbidities are common, an invasive (iNTS) disease phenotype is seen, associated with up to 20 % case-fatality ratio, and antimicrobial resistance is both significant and growing. The need to evaluate the potential public health va

Vaccine Safety and Surveillance for Adverse Events Following Immunization (AEFI) in India.

Assured quality vaccines and safe immunization practices are pre-requisite to successful immunization programs. All vaccines go through stringent safety checks during pre-licensure stage. Adverse Events Following Immunization (AEFI) Surveillance program is an integral part of routine immunization program in India to monitor the vaccine safety in the post licensure phase. Indian AEFI Program: National AEFI surveillance relies on passive surveillance and reporting by the health functionaries and practitioners. Vigorous strengthening of AEFI surveillance has resulted in manifold rise in absolute

Bioptic telescopes meet the needs of drivers with moderate visual acuity loss.

PURPOSE: Visually impaired people are permitted to use bioptic telescopes for driving in many states in the United States. However, it has been suggested that the telescope is used only to meet the visual acuity criteria for licensure. In this study, a survey was used to establish the extent to which bioptic telescopes are used by and meet the driving needs of people with moderately reduced visual acuity. METHODS: A cross-sectional survey of a convenience sample of 58 bioptic drivers was administered by telephone interview. Bioptic telescope usage patterns were quantified with questions desig

Considerations for a phase-III trial to evaluate a group B Streptococcus polysaccharide-protein conjugate vaccine in pregnant women for the prevention of early- and late-onset invasive disease in young-infants.

In 2010, an estimated 393,000 infection-related neonatal deaths occurred worldwide with Group B streptococcus (GBS) being a leading cause. Prevention of early-onset disease (0-6 days; EOD) is currently focused on intra-partum antibiotic prophylaxis to mothers identified as being at risk; such strategies reduce EOD by 75-80% but are resource-intensive and logistically-difficult to implement in developing countries. Vaccination of pregnant women is an alternate strategy for preventing both EOD and late-onset disease (7-89 days; LOD). A trivalent GBS polysaccharide-protein conjugate vaccine (GBS-
