Breastfeeding and mothering one-handed.

This case study describes how a mother with a congenital below-elbow limb absence manages breastfeeding and child care. Information for the case study was gained from direct observation, detailed interviews, and the subject's written reflections. Literature review revealed that, worldwide, many women have upper limb deficiencies. Such deficits may occur congenitally from teratogens or other causes, or they may occur as a result of disease or trauma. In spite of recent technological advances, prostheses are restricted in their efficiency and are not frequently used. Support from others is a cri

[Resuscitation - Adult advanced life support].

Enhanced measures for resuscitation of adults are based on basic measures of resuscitation. The central elements are highly effective chest compressions and avoidance of disruptions that are associated with poor patient outcomes that occur within seconds. The universal algorithm distinguishes the therapy for ventricular fibrillation from the therapy in asystole or pulseless electrical activity (PEA) by the need of defibrillation, and amiodarone administration in the former. Defibrillation is biphasic. In all other aspects, there are no differences in therapy. In each episode of cardiac arrest,

Resuscitation of the pediatric patient.

Fortunately, the incidence of pediatric cardiopulmonary arrest is extremely low in the outpatient dental office setting. Because most cardiopulmonary arrests in children result from a progressive deterioration in respiratory function, outcome critically depends on rapid diagnosis and evaluation of the adequacy of ventilation and the pediatric airway. This holds true for any pediatric medical emergency. Our goal must be instituting simple resuscitative measures before full cardiopulmonary arrest develops. Whatever the nature of the medical emergency, caring for a child under these circumstances

The Medicare Hospice Benefit's Origins: Lessons on End-of-Life Caregiving.

Legislation in 1982 formalized hospice care as a Medicare-reimbursed service. In the years that have followed, while many Americans have benefited from hospice's holistic, comfort-focused care, Medicare reimbursement frequently prevents hospices from providing the degree of caregiving support that patients and their informal caregivers need. Prior to the Medicare Hospice Benefit, leaders of the hospice movement emphasized that informal caregivers need significant support at the end of life, including custodial support through options for facility-based care for dying patients. The Medicare Hos

Identifying individuals satisfying the termination of resuscitation rule but having potential to achieve favourable neurological outcome following out-of-hospital cardiac arrest.

AIM: To develop a simple scoring model that identifies individuals satisfying the termination of resuscitation (TOR) rule but having potential to achieve favourable neurological outcome following out-of-hospital cardiac arrest (OHCA). METHODS: This study analysed the All-Japan Utstein Registry from 1 January 2010 to 31 December 2019. We identified patients satisfying basic life support (BLS) and advanced life support (ALS) TOR rules and determined factors associated with favourable neurological outcome (cerebral performance category scale of 1 or 2) for each cohort using multivariable logisti

The "PSOST": Providers' Signout for Scope of Treatment.

Palliative care provides open and honest communication, medically appropriate goal setting, and meticulous attention to symptom assessment and control. The Physicians Orders for Life Sustaining Treatment (POLST) is a growing movement to allow health care providers to indicate, with their patients, what they want done in specific situations, such as feeding tubes, mechanical ventilation, or transfer to an intensive care unit. We have developed an internal signout tool used by palliative medicine fellows in our institution to specify similar interventions-or not-with seriously ill palliative car

Destitute and dying: interventions and models of palliative and end of life care for homeless adults - a systematic review.

BACKGROUND: Homeless adults experience a significant symptom burden when living with a life-limiting illness and nearing the end of life. This increases the inequalities that homeless adults face while coping with a loss of rootedness in the world. There is a lack of palliative and end of life care provision specifically adapted to meet their needs, exacerbating their illness and worsening the quality of their remaining life. AIM: To identify interventions and models of care used to address the palliative and end of life care needs of homeless adults, and to determine their effectiveness. ME

Shifting Public Discourse on Life-Sustaining Treatment in South Korea From 2014 to 2025: Longitudinal Social Media Analysis.

BACKGROUND: Public discourse plays an important role in shaping how end-of-life care and medical decision-making are understood within society. In South Korea, the implementation of the Life-Sustaining Treatment Decision Act in 2018 established a legal framework for withholding or withdrawing life-sustaining treatment for patients at the end of life. However, relatively little is known about how public discourse surrounding life-sustaining treatment has evolved over time, particularly before and after the act's implementation. OBJECTIVE: This study aimed to examine how online discourse relate

[Nutrition and fluid management in palliative medicine: do food and drink keep body and soul together?].

Induction, implementation and continuation of an invasive nutrition or fluid administration in patients with advanced, life-limiting illnesses is an often controversial but also very emotionally discussed topic. This article summarizes the current state of knowledge based mainly on the European Society for Clinical Nutrition and Metabolism (ESPEN) guidelines and is intended as a simple guide for clinical practice. In the early phase of disease the induction of an invasive food and fluid administration may be indicated in order to prevent undernutrition and cachexia, to enhance compliance with

The management of patients with advanced motor neuron disease.

BACKGROUND: There is no specific treatment for motor neuron disease (MND) except hospice or palliative care to improve patients' quality of life and decrease complications. This topic is seldom discussed in Taiwan. METHODS: A retrospective study was conducted of patients with terminal MND who were treated and died at the Veterans General Hospital-Taipei from March 1986 through April 1996. Patients' characteristics, management, length of survival and cause of death were analyzed. RESULTS: Twenty-three patients (M/F, 17/6) were included. The median age of onset was 59 years (range, 24-69). The

Network Types for End-Of-Life Care and Home Deaths Among Older Adults in Europe During COVID-19 Pandemic.

The study identified care network types comprising informal and formal care providers during the end-of-life period, and examined their relationship with home deaths. End-of-life interviews were conducted with proxies during the two waves of the COVID-19 pandemic in the Survey of Health, Ageing and Retirement in Europe (SHARE). The sample included 486 participants who passed away during the pandemic and received care during their final year. Latent Class Analysis identified three care network types: "mixed-care", mostly consisting of adult children in an informal network; "spouse", dominated b

Policy, system and service design influence on healthcare inequities for people with end-of-life chronic obstructive airways disease, their support people and health professionals.

BACKGROUND: People with end-of-life chronic obstructive pulmonary disease (COPD) experience debilitating physical limitations, with a high mortality rate. Our research has shown health system design and delivery leads to inequitable outcomes. Enabling people with end-of-life COPD, their support people, and health professionals to partner in setting the agenda for resource allocation may inform health service improvement. DESIGN: Qualitative methodology utilising focus groups including patients, family, friends, informal support people, health care workers and professionals. METHODS: The anal

Cardiopulmonary support in the intensive care unit.

The cardiopulmonary support system is an extracorporeal device that allows for rapid cardiopulmonary support of the critically ill patient in the intensive care unit. It provides immediate and complete support of cardiac and pulmonary functions to maintain perfusion to vital organs in patients who are severely physiologically compromised (eg, in cardiogenic shock, adult respiratory distress syndrome or pulmonary edema). Successful cardiopulmonary support requires systemic anticoagulation, percutaneous venous and arterial cannulation and careful monitoring by the critical care team to maintain

The Unmet Needs of Parents in Pediatric Palliative Care: A Qualitative Systematic Review.

BACKGROUND: The goal of pediatric palliative care is to improve the quality of life of children with life-limiting or life-threatening disease and their families through a holistic care approach. Although the current evidence suggests that the existing palliative care services are not meeting the needs of these families, especially the parents, these studies are heterogeneous with inconsistent results. The aim of this study was to systematically synthesize and critically evaluate the available evidence on unmet needs of parents in pediatric palliative care. METHODS: A qualitative systematic r
