Decoding Ultramarathon: Muscle Damage as the Main Impediment to Performance.

The biological determinants of performance have been well described for running races up to and including the marathon (42.2 km). Ultramarathon is more complex. Events range from 50 to 5000 km in single or multiple stages, are contested in various environments and terrains, and force athletes to contend with diverse performance-limiting issues such as fueling, hydrating, gastrointestinal distress, muscle damage, and sleep deprivation. Ultramarathons are not simply "long marathons." Nevertheless, scientific developments over the past decade have inched us toward a more complete picture of the p

Lower Extremity Kinematic and Kinetic Adaptations During a Half Marathon.

This study aimed to clarify the biomechanical adaptations recreational runners experience during a half marathon. Thirty-seven healthy runners completed a half marathon as fast as possible on a laboratory-instrumented treadmill. Kinematic and kinetic data were collected every 5 km from 0 to 20 km. As mileage accumulated, peak vertical ground reaction force (GRF) decreased from 5 km onward (p < 0.005) and impulse declined from 10 km (p < 0.05). Ground contact time increased from 10 km (p < 0.05). Joint-level adaptations included an increased hip extension moment (p < 0.005) and hip adduction an

Review of running injuries of the foot and ankle: clinical presentation and SPECT-CT imaging patterns.

Distance running is among the fastest growing sports, with record registration to marathons worldwide. It is estimated that more than half of recreational runners will experience injuries related to the practice of their sport. Three-phase bone scintigraphy is a very sensitive tool to identify sports injury, allowing imaging of hyperemia, stress reaction, enthesopathy and fractures, often before abnormalities can be detected on conventional anatomical modalities. In this article, we review the most common running related injuries and their imaging findings on bone scintigraphy with SPECT-CT.

Cardiac autonomic regulation following a 246-km mountain ultra-marathon: An observational study.

Physical exercise requires integrated autonomic and cardiovascular adjustments to maintain homeostasis. We aimed to observe acute posture-related changes in blood pressure, and apply a portable noninvasive monitor to measure the heart index for detecting arrhythmia among elite participants of a 246-km mountain ultra-marathon. Nine experienced ultra-marathoners (8 males and 1 female) participating in the Run Across Taiwan Ultra-marathon in 2018 were enrolled. The runners' Heart Spectrum Blood Pressure Monitor measurements were obtained in the standing and supine positions before and immediately

Bioactive Lipids of Cardiorespiratory Fitness Insights From Exercise Models in Humans, Mice, and Burmese Pythons.

BACKGROUND: Cardiorespiratory fitness, commonly measured as peak oxygen consumption (peak VO2, is a strong predictor of cardiovascular health. Circulating lipids have been linked to exercise responses and metabolic adaptation, but the bioactive lipid species associated with objectively measured aerobic capacity and their dynamic regulation during exercise remain incompletely defined. METHODS: We performed directed, nontargeted liquid chromatography-mass spectrometry profiling of circulating bioactive lipids in adults undergoing clinically indicated cardiopulmonary exercise testing, relating r

Running kinematics, spatiotemporal variation and lower limb strength at different stages of a 156-km ultra-trail race.

BACKGROUND: Ultra-trail running is increasingly popular, yet most research on biomechanics focuses on road running or treadmills. This study assesses changes in foot strike pattern (FSP), step frequency, and other biomechanical parameters over a 156-km ultra-trail race. METHODS: Fifty-five experienced runners participated in the Trail Scientifique de Clécy, a 156-km race with 6000 meters of elevation gain. Biomechanical data were collected at the end of each 26-km lap using video for FSP and Optojump for other parameters. Muscle strength was measured via maximal voluntary isometric contractio

Exploratory case comparison of gut microbiome functional potential in ultramarathoners differing in adiposity and finish time.

Understanding variations in gut microbial functional potential among endurance athletes may inform future personalized nutritional strategies. This exploratory case-comparison study aimed to investigate predicted microbial functional potential using shotgun metagenomic sequencing and fecal metabolites, in two post hoc-selected runners who represented extreme and contrasting outcomes from the same single-stage 217 km mountain ultramarathon: a normal-BMI fast-finisher and an obese slow-finisher. The main descriptive findings suggest that the normal-BMI fast finisher exhibited smaller observed di

Ultramarathon racing elicits changes in behavioral and electroencephalographic indices of executive function.

The purpose of this study was to determine whether racing a 50-km ultramarathon alters behavioral and electrophysiological (EEG) indices of executive function. Seventy-six recreational runners (37 ± 10 yr; 39 F, 37 M) completed EEG assessments before and immediately after the race at one of six ultramarathons. Executive function was evaluated with a visual oddball task, with accuracy and reaction time as behavioral markers, N2 as an index of inhibitory control, and P3 as an index of attentional allocation. Motivation [Sport Motivation Scale (SMS)] and negative affect [Depression, Anxiety, and

Bilateral anterior acetabular-superior pubic ramus junction fatigue fractures in a marathon runner.

Fatigue fractures occur due to repetitive submaximal mechanical stress on bone and are most common in the lower extremities of endurance athletes and military recruits. This report details a rare case of bilateral anterior acetabular-superior pubic ramus junction fatigue fractures in a marathon runner presenting with low back and hip pain. MR imaging of the pelvis was initially interpreted as normal. Further review by a sports medicine physician and a musculoskeletal radiologist prompted an addendum identifying bilateral mild marrow edema of the anterior acetabula with incomplete fracture line

(Ultra-)inflammation or adaptation? Comparison of different ultramarathon distances and their effect on the immune system.

BACKGROUND: Ultra-endurance sports like running over several hours or days exhibit great physical, psychological and metabolic strain on the respective athlete. Although the impact of ultramarathon running on the inflammatory/immunological system gained interest in the last years, there is no study that examined the effect of different running distances on inflammatory/immune system responses. METHODS: During a non-stop ultramarathon, blood and saliva samples were collected before (Pre) and after the race (Post), and analyzed for changes in blood cell variables (immune cells leukocytes/thromb

Effects of hip stability training versus standard care on Q-angle and pain in marathon runners with patellofemoral pain syndrome: A retrospective cohort study.

OBJECTIVE: Hip stability training may influence the quadriceps angle (Q-angle) and pain, but the clinical relevance of static Q-angle changes remains uncertain. This study compared hip stability training with standard care for Q-angle, pain, and function in marathon runners with Patellofemoral Pain Syndrome (PFPS). METHODS: We identified 487 marathon runners with PFPS, of whom 312 met the eligibility criteria. After 1:1 propensity score matching, 94 runners were retained in each group: hip stability training (Hip-STAB) and standard care (Control). Outcomes were assessed at baseline (W0) and a

Pacing and placing in 161-km ultramarathons: Effects of sex and age.

Ultramarathons are growing in popularity, owing especially to the participation of women and masters athletes. Pacing strategy, which can vary by sex and age, is a critical variable in determining finishing place in races up to the marathon distance. Whether this is true in 161-km ultramarathons is unclear. We tested the hypotheses that pacing is a determining factor in finishing place and that pacing differs by sex and age in 161-km ultramarathons. Publicly available data from 161-km races (n = 6) were analyzed for years 2012-2022 (n = 56). Linear regression was used to analyze the proportion

Persistent Improvements in Running Economy With Advanced Footwear Technology During Prolonged Running in Trained Male Runners.

This study evaluated the effects of a top-tier carbon plated shoe with highly responsive foam (CP-shoe) on changes in running economy (RE) and running speed at lactate threshold (LT) before, during, and after prolonged running. Ten male distance runners (half-marathon time: 75 ± 3 min) completed a series of tests. Initially, two graded treadmill tests were undertaken in a well-rested state while wearing either a CP-shoe or non-carbon plated shoe (NCP-shoe) to determine RE and LT-speed (LTCP and LTNCP). On separate days, participants then performed three 80-min runs in randomized order: twice a

Sex differences in ultramarathon performance in races with comparable numbers of males and females.

There is a prominent sex-based difference in athletic performance such that males outperform females by 7%-14% in races from 100 m to marathon. In ultramarathons, the difference is often much smaller, leading to speculation that females are "built" for the sport. However, data are confounded by the low number of female participants; just 10%-30% in any given race. This study compared data from two ultramarathons where males and females competed in comparable numbers. There were 116 and 146 starters in the 50 mile and 100 mile races, respectively (52% female). Finish times were compared using t
