We have developed a new type of modified radical mastectomy, the method and clinical results of which are reported herein. In this operation, axillary dissection is performed by the following two approaches. Firstly, the axillary contents are dissected from the highest possible subclavicular point to the pectoralis minor muscle, after partially cutting the sternocostal origin of the pectoralis major muscle. The second approach is from the posterior aspect of the pectoralis minor muscle to the lateral portion of the latissimus dorsi muscle. Parasternal dissection can also be performed for stage

Psychosocial morbidity was examined in 90 consecutive patients with Stage I or II breast cancer aged under 70 treated by mastectomy. In the two years after surgery, the prevalences of depression and anxiety of clinical degree were generally under 10%--less than half of those reported in early controlled studies in the United Kingdom. However, cancer patients aged under 50 showed significantly higher prevalences compared with cancer patients aged 50 or above and compared with matched control subjects with benign breast disease. Social and work problems were common at all ages. Sexual problems w

It is possible to reconstruct the normal shape of the female breasts at the time of operation for carcinoma of the breast or benign lesions where the whole breast tissue is removed. This is done by the insertion of an inert silicone plastic prosthesis. This procedure avoids the mutilating effect normally associated with mastectomy. The criteria for this operation must be strictly applied if it is to be successful and if cure of the disease is not to be prejudiced. These principally involve local characteristics of the tumour and the nutrition of the skin flaps which will remain after the ablat

BACKGROUND: Breast cancer surgery has become less and less mutilating, however a mastectomy is required in the case of multicentric, large tumors or recurrences after conservative treatment. The removal of the nipple areola complex during the mastectomy dramatically increases the feeling of mutilation. To reduce this negative psychological impact, in cancers located outside of the central area of the breast, we propose a new type of nipple-sparing mastectomy associated with intraoperative electron beam radiotherapy (ELIOT) delivered on the region of the areola. The nipple-sparing mastectomy is

INTRODUCTION: The emphasis on aesthetic outcomes and quality of life (QoL) has motivated surgeons to develop skin-sparing or nipple-sparing mastectomy (SSM/ NSM) for breast cancer treatment or prevention. During the same operation, a so-called immediate breast reconstruction is performed. The breast can be reconstructed by positioning of a breast implant above (prepectoral) or below (subpectoral) the pectoralis major muscle or by using the patients' own tissue (autologous reconstruction). The optimal positioning of the implant prepectoral or subpectoral is currently not clear. Subpectoral impl

BACKGROUND: Patients who undergo radiotherapy following mastectomy may develop radiation-induced ulcers (RIUs) years later that are often refractory to healing. Reconstruction of these refractory ulcers remains a challenge for surgeons, and conventional surgical approaches can have disastrous outcomes.
METHODS: We present a detailed itemization and analysis of specific surgical methods-including local random-pattern flaps, pedicled flaps, and free flaps-as well as conservative treatments, along with their therapeutic outcomes, advantages, and disadvantages.
RESULTS: Chest wall reconstruction

BACKGROUND: Immediate reconstruction after mastectomy is a major surgical procedure of the breast associated with a significant risk of postoperative complications. To date, there is very little evidence supporting nutritional intervention as a potential risk-reducing factor. This study aimed to assess the effect of perioperative arginine supplementation on postoperative recovery.
MATERIALS AND METHODS: This was an open-label, non-randomised pilot cohort study including 94 patients. The intervention group received one oral supplement daily providing 5 g of arginine, 20 g of protein, and 100 k

Single-port robot-assisted nipple-sparing mastectomy (SPRNSM) has emerged as an innovative minimally invasive alternative to conventional and multi-port robotic techniques, aiming to improve cosmetic outcomes while maintaining oncologic safety. This systematic review evaluates the feasibility, safety, and current evidence supporting SPRNSM. A systematic search of PubMed, the Cochrane Library, and ClinicalTrials.gov was conducted from database inception to July 4, 2026. Studies involving adult women undergoing SPRNSM for therapeutic or prophylactic indications were included. Both clinical and c

Same-day surgery for mastectomy increased in Alberta from 1.7% to 73% in 2022, after implementation of a perioperative pathway in 2016. However, rates of unplanned visits to the emergency department (ED) remained >20%. Previous research explored reasons for this using administrative data but lacked patient-level data. Our study aims to explore patient-reported factors influencing unplanned ED visits after mastectomy. A survey study was conducted of patients who underwent mastectomy in Alberta between July 2021 and June 2022. Patients were identified from the Canadian Institute of Health Inform

A transgender (trans) man is a trans person who was assigned female sex at birth. Some trans men undergo gender-affirming surgical procedures, particularly mastectomy. We present a case of keloid formation after gender-affirming mastectomy that highlights the importance of choosing the most appropriate mastectomy technique in patients at risk of keloid and the most effective therapeutic strategies for anterior-chest keloids. A 40-year-old Japanese trans man developed keloids along inverted-T mastectomy-induced anterior-chest scars. The keloids were completely excised, and the defects were clos

BACKGROUND: Pain management following mastectomy is a significant challenge often requiring opioids. Nonopioid pain management utilizing nerve blocks has been shown in other fields to reduce postoperative opioid use and may be effective for postoperative pain in mastectomy patients. The primary purpose of this study was to compare postoperative opioid use, measured in morphine milligram equivalents (MME), between mastectomy patients who underwent interpectoral nerve block (IPNB) and a historical control group. Secondary outcomes included length of stay (LOS) and postoperative pain scores.

Breast reconstruction is recognized as an integrated part of breast cancer treatment today. Depending on tumor characteristics, different types of partial and total breast resections are indicated: tumorectomy, nipple/skin sparing or complete mastectomy. Patients' desires, general health status and body shape, as well as the necessity of adjuvant therapies, lead to the individual reconstruction plan. Next to implant-based reconstructions, autologous reconstructions play a great role, including local, pedicled and free flaps as well as autologous fat grafting. In cases of tumorectomy, oncoplast

The Wise-pattern skin-sparing mastectomy (SSM) is well known for its efficacy in large ptotic breasts, and its safety in facilitating immediate breast reconstruction. An unfortunate sequalae for all SSM techniques is mastectomy skin flap necrosis (MSFN) with a reported range of occurrence of 5%-30%. For the Wise pattern, the common area of wound dehiscence or necrosis is the T-junction. Different techniques have been described in the management of MSFN-ranging from primary closure to local and distant flaps. Full thickness MSFN results in wound breakdown and can expose a prosthesis, subsequent

Phyllodes tumour is a rapidly growing neoplasm with a propensity to involve the entire breast tissue. In large tumours, the treatment comprises a wide local excision or a mastectomy. A woman in her 20s from rural India presented with complaints of a recurrent left breast lump. The lump progressed to a large size, limiting her social activities and causing depression. On examination, she had a mass occupying almost the entire left breast, with stretched skin, dilated veins and pressure necrosis. There were no palpable axillary nodes. She was offered a wide local excision and reconstruction with

BACKGROUND: The breast reconstruction (BR) rate for women undergoing mastectomy for breast cancer management is 18% in Australia. The Australian Access to Breast Reconstruction Collaborative Group recommends that all women should have access to BR. This study presents BR uptake and outcomes from a breast surgical unit.
METHODS: A retrospective observational study identified women who had curative mastectomy for breast cancer between 1 January 2016 and 31 December 2021. Patient factors and surgical complications were compared between BR and no BR (NBR) patients.
RESULTS: Out of 929 women who
