The Application of Bone Allograft in Mastoid Obliteration After Mastoidectomy.

BACKGROUND: Mastoid obliteration after canal wall down mastoidectomy (CWDM) is recommended to reduce both complications and recurrence. In this study, the bone allograft was used for mastoid obliteration in large cavities and reconstruction of the external auditory canal (EAC) after retrograde CWDM. METHODS: This is a prospective, single-arm, open-label trial. Inclusion criteria were adult patients (>18 years old) who had undergone retro grade CWDM due to chronic otitis media with cholesteatoma. Bone allograft was used for mastoid obliteration in large cavities and for EAC reconstruction. RE

Mastoid obliteration with bone dust mixed with concentrated growth factor.

BACKGROUND: Mastoid obliteration is a useful surgical step in the treatment of chronic ear operation. The technique of mastoid obliteration involves filling the cavity with self-tissue and/or osseocartilaginous graft material, following removing the mastoid air cell. OBJECTIVE: The aim of this study is to show the advantages of mastoid obliteration using a mixture of CGF and bone dust. MATERIAL AND METHOD: Bone dust was collected with an apparatus from the healthy mastoid cortex. Collected bone dust was mixed with CGF to form a gel. Then, mastoid cavity and epitympanum obliteration were perf

Middle ear morphology and mastoid pneumatisation: a computed tomography study.

BACKGROUND: Mastoid pneumatisation is subject to numerous influencing factors including race, sex, and surrounding structures of the middle ear. This study aims to determine the mastoid air cell system (MACS) volume and its relationship with middle ear structures, and the influence of sex. MATERIALS AND METHODS: A cross-sectional study was performed analysing computed tomography (CT) scans in which MACS volume and the Eustachian tube length (ETL) were visible. MACS volume, ETL, and width and height of the aditus ad antrum were obtained. RESULTS: A total of 100 CT were included with a mean ag

Mastoid Abscess in a Child With Eustachian Tube Dermoid Cyst.

A dermoid cyst (DC) is a benign tumor caused by inclusion errors during embryogenesis. DC of the head and neck is a well-recognized entity both clinically and histologically; however, it rarely occurs in the Eustachian tube (ET). Due to its anatomical position, significant morbidity related to middle ear dysfunction may result from ET obstruction. In this report, we present a rare case of a girl child aged two years and nine months with persistent otorrhea, who was initially diagnosed with acute otitis media with mastoiditis, along with suspicion of congenital cholesteatoma. However, high-reso

Mastoid Obliteration with Synthetic Materials: A Review of the Literature.

Canal wall down mastoidectomy is a surgical technique used for the eradication of middle ear disease. The remaining large mastoid bowl is associated with a number of issues; one of the main techniques that have been developed in order to avoid such problems is the obliteration of the mastoid cavity. The materials used for this reason are either biological or synthetic. The purpose of this survey is to review the published literature related to the therapeutic value of mastoid obliteration with synthetic materials. We searched Web of Science, PubMed, and MEDLINE from 2008 to 2018 using the crit

Incidental mastoid opacification on computed tomography in the pediatric population.

OBJECTIVES: Incidental mastoid opacification (IMO) is a common radiologic finding and source of referral to otolaryngologists. Our objectives were to: 1) determine the rate of IMO in our pediatric population; 2) identify those with clinical mastoiditis; and 3) identify factors necessitating an otolaryngology consultation. METHODS: This was a retrospective chart review at an academic, tertiary, pediatric hospital in an urban setting. Pediatric patients with the keyword 'mastoid' on a computed tomography (CT) scan report were reviewed. The effect of age, gender, season and the use of contrast m

Concurrent Mastoid Cellulitis and Langerhans Cells Histiocytosis:
A Challenging Diagnosis.

Langerhans cell histiocytosis (LCH) is a rare proliferative disorder, which commonly arises in the bone and may involve other systems. To date, the diagnosis of temporal bone LCH remains a challenge as it may masquerade as a common ear infection. We report a case of a child who presented to us with persistent bilateral ear discharge for four months and was not responding to treatment. Her condition subsequently worsened, with clinical features and radiological findings suggestive of mastoid cellulitis. Nevertheless, further histopathology study revealed LCH.

Middle ear gas pressure regulation: the relevance of mastoid obliteration.

OBJECTIVES: To establish a mathematical model of middle ear gas pressure regulation and to discuss potential implications for pathophysiology-oriented theoretical approach to middle ear surgery, with particular attention to mastoid obliteration. BACKGROUND: Numerous studies support that small mastoid volume is associated with cholesteatoma. Latest studies show that mastoid obliteration is an effective technique to lower the recurrence rate in these ears. METHODS: A mathematical model was used to predict the development of gas pressure balance in the function of different middle ear volumes (
