Severe Maternal Morbidity and Maternal Mortality Associated with Pregnancies Among Women With Solid Organ Transplants.

OBJECTIVES: Solid organ transplants (SOT), including kidney, liver, heart, lung, and pancreas transplants, have become more common in the last few decades. However, few population-based studies have attempted to comprehensively quantify severe maternal morbidity (SMM) and maternal mortality associated with pregnancies among recipients of SOTs. This study aimed to determine the proportion of women with SOTs among all delivering women, and to quantify the rates of SMM and maternal mortality associated with SOT. METHODS: We conducted a retrospective cohort study of all hospital deliveries at ≥20

Maternal mortality, stillbirths, and neonatal mortality: a transition model based on analyses of 151 countries.

BACKGROUND: Maternal mortality, stillbirths, and neonatal mortality account for almost 5 million deaths a year and are often analysed separately, despite having overlapping causes and interventions. We propose a comprehensive five-phase mortality transition model to improve analyses of progress and inform strategic planning. METHODS: In this empirical data-driven study to develop a model transition, we used UN estimates for 151 countries to assess changes in maternal mortality, stillbirths, and neonatal deaths. On the basis of ratios of maternal to stillbirth and neonatal mortality, we identi

Perinatal mortality and maternal mortality at the Provincial Hospital, Quang Ngai, South Vietnam, 1967-1970.

The perinatal mortality, maternal mortality, infant mortality rates, and the complications of delivery at the Provincial Hospital of Quang Ngai, South Vietnam are described. The perinatal mortality is the only valid statistic available as the infant usually leaves the hospital within three days of delivery. Knowledge pertaining to the 4th to 28th day after birth is scanty and there is insufficient knowledge about the first year of life. Infant mortality is estimated at 277 per 1,000 live births. The perinatal mortality 64.6 per 1,000 live births, and maternal mortality, 106 per 10,000 live bir

Black Maternal Mortality: A Crisis in Need of Urgent Redress.

While the latest update from the National Center for Health Statistics of the Centers for Disease Control and Prevention (CDC) announced that the national maternal mortality rate for 2023 decreased to 18.6 deaths per 100,000 live births, in comparison to a rate of 22.3 in 2022, it noted that the maternal mortality rate for Black women was 50.3 deaths per 100,000 live births and that this was significantly higher than rates for White (14.5), Hispanic (12.4), and Asian (10.7) women. More troubling still, the maternal mortality rate for Black women has risen over the past five years from 2.5 time

Age and Type of Delivery as Risk Indicators for Maternal Mortality.

OBJECTIVE: This study assessed maternal mortality (MM) and related factors in a large-sized municipality in the Southeastern region of Brazil (Campinas, São Paulo) during the period 2000-2015. METHODS: This study consisted of two phases: 1. An analytical nested case-control phase that assessed the impact of individual and contextual variables on MM; and 2. an ecological phase designed to contextualize maternal deaths by means of spatial analysis. The case group consisted of all maternal deaths (n = 87) and the control group consisted of 348 women who gave birth during the same period. Data an

Maternal Mortality in the Remote Islands of India-Unraveling the Conundrum.

INTRODUCTION: Reduction in maternal mortality has been a top priority in low- and lower middle-income countries of Asia-Pacific region. India, being one of them, has a major responsibility of reducing maternal mortality especially in light of the commitment on the part of the Sustainable Development Goals. Despite massive efforts and availability of technology needed to avert maternal deaths, the maternal mortality ratio (MMR) in India continues to be high especially in remote areas. This study aims at collecting the MMR data and analyzing in the context of island infrastructure. MATERIAL AND

Impact assessment of particulate pollution on maternal mortality in Nigeria.

Recently, the World Health Organization reported that 20% of all global maternal deaths happened in Nigeria between 2005 and 2015. In developing countries, these maternal deaths are mainly from air pollution. Due to poor facilities and documentation, the extent of danger is not known. This research seeks to estimate the available pollutants and its direct and indirect impact on maternal mortality. Ten (10) years (2010-2019) datasets of black carbon, sulfur dioxide, dust, carbon monoxide, organic carbon particulates, sea-salts, and sulphate particulates were obtained from the second modern-era

Maternal Morbidity and Mortality: Are We Getting to the "Heart" of the Matter?

Cardiovascular disease (CVD), including hypertensive disorders of pregnancy (HDP) and peripartum cardiomyopathy, is a leading cause of pregnancy-related death in the United States. Women who are African American or American Indian/Alaskan Native, have HDP, are medically underserved, are older, or are obese have a major risk for the onset and/or progression of CVD during and after pregnancy. Paradoxically, women with no preexisting chronic conditions or risk factors also experience significant pregnancy-related cardiovascular (CV) complications. The question remains whether substantial physiolo

Women's Preferences for Maternal and Neonatal Morbidity and Mortality in Childbirth.

Purpose. To measure utility values that describe women's willingness to tradeoff maternal morbidity for fetal benefit among pregnant and nonpregnant women of reproductive age. Methods. We recruited English-speaking women aged 18 to 45 years in clinical and community-based settings. Eight health states were studied: 4 maternal (healthy, stroke, hysterectomy, death) and 4 neonatal (healthy, severe cerebral palsy [CP], severe mental retardation [MR], death). Utilities were assessed on a subset of 9 pairs of mom/baby delivery outcomes. Participants ranked the 9 pairs of outcomes in order of prefer

The Role of the Anesthesiologist in Preventing Severe Maternal Morbidity and Mortality.

Anesthesiologists are responsible for the safe and effective provision of analgesia for labor and anesthesia for cesarean delivery and other obstetric procedures. In addition, obstetric anesthesiologists often have a unique role as the intensivists of the obstetric suite. The anesthesiologist is frequently the clinician with the greatest experience in the acute bedside management of a hemodynamically unstable patient and expertise in life-saving interventions. This review will discuss (1) risks associated with neuraxial and general anesthesia for labor and delivery, and (2) clinical scenarios

The Role of Obstetrics/Gynecology Hospitalists in Reducing Maternal Mortality.

The United States experienced a 6.1% annual increase in the maternal death rate from 2000 to 2013. Maternal deaths from hemorrhage and complications of preeclampsia are significant contributors to the maternal death rate. Many of these deaths are preventable. By virtue of their continuous care of laboring patients, active involvement in hospital safety initiatives, and immediate availability, obstetric hospitalists are uniquely positioned to evaluate patients, initiate care, and coordinate a multidisciplinary effort. In cases of significant maternal hemorrhage, hypertensive crisis, and acute p

Eclapmsia: The Major Cause of Maternal Mortality in Eastern India.

BACKGROUND: Eclampsia is a very serious complication of pregnancy which is responsible for high maternal and perinatal mortality. Worldwide, it accounts for 50,000 maternal deaths annually. In spite of several global and regional interventions and initiatives from governments and other concerned agencies, maternal mortality is still very high in India, with eclampsia as a major cause. This study was conducted to determine the mode of deaths and incidence of maternal mortality associated with eclampsia and to assess how socio-demographic and clinical characteristics of the women influence the d
