Medicaid expansion and potential effects on Veterans Health Administration inpatient utilization.

BACKGROUND: The Affordable Care Act (ACA) allowed states to expand Medicaid to adults under 138 percent of the federal poverty line. Questions remain about how expansion influences health care utilization by Veterans already enrolled in the Veterans Health Administration (VHA). OBJECTIVE: To understand changes in VHA share of inpatient utilization among VHA-enrolled Veterans after Medicaid expansion. METHODS: Sample included Veterans aged 18-64 from 2011 to 2016 enrolled in VHA. Outcomes included changes in VHA share of total VHA + Medicaid inpatient days, proportion with VHA and Medicaid in

Medicaid Expansion and Overall Mortality Among Individuals With Renal Cell Carcinoma.

INTRODUCTION: Renal cell carcinoma (RCC) remains a major cause of cancer-related morbidity, mortality, and economic burden in the United States. Medicaid expansion under the Affordable Care Act increased access to insurance coverage for low-income adults, yet evidence regarding its impact on RCC survival remains limited. We evaluated the association between Medicaid expansion and overall and 5-year survival among adults aged 40-64 years diagnosed with RCC. METHODS: We conducted a retrospective cohort study using National Cancer Database data (2006-2021). States were classified as Medicaid exp

Comparing Approaches to Identify Mobile Crisis Services in Medicaid Administrative Claims.

BACKGROUND: Mobile crisis services offer a community-based alternative to law enforcement and emergency departments for individuals experiencing behavioral health crises. Medicaid claims represent a promising but underused tool for national surveillance of these services. Despite growing federal investment and expansion of crisis response systems, there is no standardized method for identifying mobile crisis encounters in administrative claims, limiting the ability to monitor access, evaluate implementation, or compare delivery across states. OBJECTIVE: To develop and evaluate claims-based ap

Medicaid Tobacco And Nicotine Cessation Treatment Rates Remained Low, 2019-24.

Tobacco use drives large health and fiscal costs for Medicaid, yet few enrollees who use these products receive cessation pharmacotherapy. We estimated that 10.4 percent of daily tobacco or nicotine users received cessation medications in 2024, and Medicaid programs spent $137.6 million on such treatments that year. State-level treatment rates ranged from 1.3 percent to 26.6 percent.

Rural-Urban Differences in Coverage Gains Under Continuous Postpartum Medicaid Eligibility.

BACKGROUND: Rural residents face higher rates of obstetric complications, yet are more likely to lose insurance after childbirth, limiting access to essential postpartum care. This study examined whether continuous Medicaid eligibility during the COVID-19 public health emergency (PHE) differentially impacted postpartum insurance coverage in rural versus urban populations. METHODS: Using data from the 2017-2022 American Community Survey, we evaluated how state-level variation in pregnancy-related Medicaid eligibility during the PHE influenced coverage among rural and urban residents in the pos

Medicaid Expansion and Buprenorphine Dispensing in Early vs Recent Expansion States.

IMPORTANCE: Patient Protection and Affordable Care Act (ACA) Medicaid expansion has been linked to improved uptake of medications for opioid use disorder (MOUD) access for Medicaid enrollees, but evidence is limited on the associations of post-2018 expansions with population-level treatment. Moreover, prior studies have not implemented modern staggered-adoption estimators or examined outcomes by payer type, risking biased and incomplete inference. OBJECTIVES: To evaluate the association of Medicaid expansion with buprenorphine treatment rates (overall and by payer) across all expansion states

Medicaid Accountable Care Organization Implementation and Behavioral Health Care for Children.

IMPORTANCE: Medicaid covers almost 40% of US children; state Medicaid programs are increasingly implementing financing and delivery system reforms through Accountable Care Organizations (ACOs). Little is known about how Medicaid ACOs are associated with outcomes for children with behavioral health conditions. OBJECTIVE: To examine the association between Medicaid ACO implementation and behavioral health care access, unmet mental health needs, and care experience for Medicaid-insured children with behavioral health conditions. DESIGN, SETTING, AND PARTICIPANTS: This repeated cross-sectional s

Comorbidities Associated With Chronic Hepatitis C Among Medicaid Beneficiaries, 2016-2020.

INTRODUCTION: Understanding comorbidities of chronic hepatitis C is crucial for improving care for affected persons. The prevalence of comorbidities associated with chronic hepatitis C among Medicaid beneficiaries is reported. METHODS: A descriptive retrospective cohort study was conducted using Centers for Medicare & Medicaid Services 2016-2021 data of Medicaid beneficiaries aged 18-64 years with a chronic hepatitis C diagnosis, not dually enrolled in Medicare, and with ≥12 months of continuous enrollment after diagnosis. Prescribing of extended release buprenorphine injection for Medicaid beneficiaries, 2018-2022.

BACKGROUND AND AIMS: Extended release buprenorphine injection (INJ-BUP) has been available in the United States since 2018. INJ-BUP has the potential to positively impact opioid use disorder (OUD) treatment outcomes by providing additional treatment options. As one of the largest payers of OUD treatment in the US, Medicaid coverage is important for access and uptake of INJ-BUP. Uptake of INJ-BUP among Medicaid beneficiaries has not been described since 2019 and variation in uptake by state has not previously been explored. We aimed to measure prescribing of INJ-BUP for Medicaid beneficiaries s

Impact of medicaid expansion on screenable versus non-screenable gastrointestinal cancers.

BACKGROUND: Medicaid expansion afforded increased healthcare access to low-income Americans contributing to a positive impact on cancer outcomes. However, it is unclear if these benefits were mainly due to enhanced access to cancer screening and earlier diagnosis versus access to cancer treatment METHODS: The National Cancer Database (NCDB) was queried between 2010 and 2021 for Medicaid and uninsured patients with GI malignancies. Patients were stratified by screenable (SGI) and non-screenable (NGI) cancers and expansion state (ES) categories: early (EES) and late (LES) adopters, and non-expan

Psychotropic Polypharmacy Combinations and Duration of Polypharmacy Among Medicaid-Enrolled Youths.

OBJECTIVE: This study evaluated psychotropic polypharmacy frequency and patterns of use among Medicaid-enrolled youths. METHODS: A cross-sectional study of a state Medicaid claims database (2015-2020) focused on youths (≤17 years old) with at least one psychotropic medication claim and ≥90 continuous days of Medicaid enrollment. Psychotropic polypharmacy (claims for three or more therapeutic classes of psychotropics for ≥90 consecutive days) was analyzed as average annual days and annual prevalence of class combinations. Multivariable negative binomial regression models assessed changes in an

State Variation in Severe Maternal Morbidity Among Individuals With Medicaid Insurance.

OBJECTIVE: To measure variation in delivery-related severe maternal morbidity (SMM) among individuals with Medicaid insurance by state and by race and ethnicity across and within states. METHODS: We conducted a pooled, cross-sectional analysis of the 2016-2018 TAF (Transformed Medicaid Statistical Information System Analytic Files). We measured overall and state-level rates of SMM without blood transfusion for all individuals with Medicaid insurance with live births in 49 states and Washington, DC. We also examined SMM rates among non-Hispanic Black and non-Hispanic White individuals with Med

Leveling the Playing Field: Mailing Pharmacotherapy to Medicaid Members Who Smoke.

INTRODUCTION: Smoking rates differ by insurance type; rates are often double for Medicaid and uninsured compared with that for Medicare or privately insured. State-funded tobacco quitlines' provision of free nicotine replacement therapy varies. In some states, Medicaid beneficiaries must obtain nicotine replacement therapy from a physician, whereas others get nicotine replacement therapy mailed to them. METHODS: This secondary analysis examined the differences in the source and use of cessation treatment by insurance type and their impacts on cessation. The parent trial excluded people who we

Mixed Method Evaluation of Louisiana's Prerelease Medicaid Enrollment Program, 2017-2019.

BACKGROUND: Over 600,000 people leave US prisons annually. Many are eligible for Medicaid upon release but may need support to enroll. Carceral facilities in nearly half of states have implemented systems to facilitate Medicaid access for those leaving incarceration, but there is limited information on program implementation models or outcomes. OBJECTIVES: To evaluate implementation and initial outcomes of Louisiana's prison-based Prerelease Medicaid Enrollment Program. METHODS: In this mixed-methods study, we assessed enrollment in Louisiana Medicaid at time of release from prison in the 2

Opioid prescribing restrictions and opioid use among the Louisiana Medicaid population.

BACKGROUND: Most states in the U.S. have enacted prescription opioid quantity limits to curb long-term opioid dependency. While several studies of these policies find reductions in subsequent prescriptions, others find mixed results in reducing overall opioid prescriptions and prescription length. Our objective was to examine three opioid restriction policies implemented in Louisiana Medicaid: (1) a 15-day quantity limit for opioid-naïve acute pain patients, (2) a subsequent further reduction to a 7-day quantity limit and a Morphine Milligram Equivalent Dosing (MME) limit of 120mg per day, and

Meeting compliance under Medicare and Medicaid emergency preparedness Final Rule requirements.

Nursing home residents experience death and injury at a disproportionately higher rate than other populations during and after a disaster. This paper addresses the complex chronic conditions of vulnerable residents that will become even more challenging due to compounding effects from continued disasters and rapid population aging. In 2016, the Centers for Medicare and Medicaid Services (CMS) established national emergency preparedness requirements for long-term care (LTC) and skilled nursing (SN) facilities. The problems facing nursing facilities created by an increasing population to serve,

Impact of Medicaid insurance on outcomes following endoscopic transsphenoidal pituitary surgery.

OBJECTIVE: Despite the rise of studies in the neurosurgical literature suggesting that patients with Medicaid insurance have inferior outcomes, there remains a paucity of data on the impact of insurance on outcomes after endonasal endoscopic transsphenoidal surgery (EETS). Given the increasing importance of complications in quality-based healthcare metrics, the objective of this study was to assess whether Medicaid insurance type influences outcomes in EETS for pituitary adenoma. METHODS: The authors analyzed a prospectively acquired database of EETS for pituitary adenoma from 2005 to 2018 at
