Choroidal metastasis as the presenting feature of lung chondrosarcoma.

A woman in her late 30s presented with a 3 week history of rapidly worsening vision in her right eye. Examination revealed a large amelanotic choroidal mass with subretinal fluid and progressive growth on serial imaging, raising suspicion of malignancy. Systemic workup identified a 14 cm lung mass, which was resected and histologically confirmed as grade II chondrosarcoma. Meanwhile, the choroidal lesion enlarged rapidly to a collar-stud configuration involving the posterior pole, suggesting this to be a chondrosarcoma metastasis. External beam radiotherapy was declined due to lack of histolog

Susceptibility of Primary Human Choroid Plexus Epithelial Cells and Meningeal Cells to Infection by JC Virus.

JC polyomavirus (JCPyV) establishes a lifelong persistence in roughly half the human population worldwide. The cells and tissues that harbor persistent virus in vivo are not known, but renal tubules and other urogenital epithelial cells are likely candidates as virus is shed in the urine of healthy individuals. In an immunosuppressed host, JCPyV can become reactivated and cause progressive multifocal leukoencephalopathy (PML), a fatal demyelinating disease of the central nervous system. Recent observations indicate that JCPyV may productively interact with cells in the choroid plexus and lepto

Indocyanine green angiographic and multifocal electroretinographic features in the diffuse and regional form of Bietti's crystalline retinopathy.

PURPOSE: To observe different features of indocyanine green angiography (ICGA) and multifocal electroretinography (ERG) in the diffuse and regional type of Bietti's crystalline retinopathy (BCR). METHODS: ICGA and the multifocal ERG were performed in two cases of the diffuse and regional type of BCR respectively. These data were compared with fluorescein angiography (FA), standard Ganzfeld ERG, and visual field testing. RESULTS: In the regional case, ICGA revealed reduced perfusion of the choroidal circulation in the early phase and multiple hypofluorescent spots in the posterior pole in the

Assessment of measurement methods of posterior inflammation in stromal choroiditis: the value of quantitative outcome measures versus the presently qualitatively based paradigm.

PURPOSE: To assess posterior inflammation using a fluorescein (FA)/indocyanine-green angiography (ICGA) scoring system, and compare them to the presently recommended outcome measure, the standardization of uveitis nomenclature vitreous haze score (SUN-VH) in stromal choroiditis. METHODS: This was a retrospective study on patients with a diagnosis of ocular sarcoidosis(OS), ocular tuberculosis(OT), Birdshot retinochoroiditis(BRC) and Vogt-Koyanagi-Harada disease(VKH) seen in the Centre for Ophthalmic Specialized Care, Lausanne, Switzerland. Angiography signs were quantified according to an est

Imaging-based Assessment of Choriocapillaris: A Comprehensive Review.

PURPOSE: Over the past two decades, advancements in imaging modalities have significantly evolved the diagnosis and management of retinal diseases. Through these novel platforms, we have developed a deeper understanding of the anatomy of the choroidal vasculature and the choriocapillaris. The recently developed tools such as optical coherence tomography (OCT) and OCT angiography (OCTA) have helped elucidate the pathological mechanisms of several posterior segment diseases. In this review, we have explained the anatomy of the choriocapillaris and its close relationship to the outer retina and r

Neurological complications of acute multifocal placoid pigment epitheliopathy.

Acute multifocal placoid pigment epitheliopathy (AMPPE) is an autoimmune chorioretinal disease that can be complicated by neurological involvement. There is limited information on this potentially treatable condition in the neurological literature. The objective of this patient series is to describe the neurological complications of AMPPE. We retrospectively identified patients with neurological complications of AMPPE seen at Auckland Hospital between 2008 and 2013 and summarised cases in the literature between 1976 and 2013. We identified five patients with neurological complications of AMPPE

Sympathetic uveitis after trauma and vitrectomy.

A case is describes in which the earliest clinical manifestations of sympathetic uveitis that occurred after trauma and pars plana vitrectomy were confined primarily to the retina, retinal pigment epithelium, and choroid posteriorly, and resembled focal lesions of acute posterior multifocal placoid pigment epitheliopathy. Similar focal lesions were noted on gross examination of the enucleated inciting eye. Histopathological examination revealed that these lesions were caused by focal choroidal granulomas and overlying Dalen-Fuchs nodules. Sympathetic ophthalmia was also noted in four other pat

[Contribution to the problem of central serous retinal detachment (author's transl)].

We described extensive and multifocal serous detachment of the neuroretina in cases of Harada's disease, optic head pitting, coloboma of choroid and optic nerve-head and bullous retinal detachment (Gass). Peripapillary serous detachment occurred in an additional case after branch occlusion of central retinal artery. Systemic steroids were useful in Harada's disease. The other cases required photocoagulation or argon laser treatment. Bullous retinal detachment did not heal until a buckling procedure was performed. The serous detachment of the neuroretina is considered as an uniform tissue-react

Presumed multifocal cryptococcol choroidopathy prior to specific systemic manifestation.

PURPOSE: Disseminated cryptococcosis is a major cause of morbidity and mortality in immunocompromised individuals, especially those with the acquired immunodeficiency syndrome (AIDS). Early diagnosis and treatment greatly improves the outcome, so clinical clues that lead to prompt diagnosis are important. METHODS: Three patients with AIDS in whom multifocal choroiditis and choroidal lesions were the initial signs of disseminated cryptococcosis were treated with systemic amphotericin B and flucytosine. All of the patients had a systemic work-up that included evaluation of the cerebral spinal f

A syndrome resembling acute posterior multifocal placoid pigment epitheliopathy in older adults.

PURPOSE: To describe clinical characteristics and visual and anatomic outcomes of a syndrome in older patients that is similar in its active stage to acute posterior multifocal placoid pigment epitheliopathy, but has a distinct clinical course. METHODS: We retrospectively reviewed medical records and photographic studies of consecutive patients over the age of 50 who presented with acute-onset visual symptoms associated with flat, gray-white lesions at the level of the retinal pigment epithelium reminiscent of acute posterior multifocal placoid pigment epitheliopathy. RESULTS: The cohort inc

A diagnostic conundrum.

We present a 26 year old male with a 6 year history of painful sensory symptoms, weakness and wasting in the legs alongside progressive facial weakness, slurred speech, dysphagia and ophthalmoplegia. There were no neurological symptoms or signs in the upper limbs. Previous medical history included traumatic corneal injury to the left eye in childhood and a cisterna magna choroid plexus papilloma which was removed surgically at age 22. Investigations localised the pathology to the cranial nerve and pre-ganglionic lumbar nerve roots. A dural meningeal biopsy demonstrated grade 1 choroid plexus p

Disciform detachment of the macula. II. Fluorescein and indocyanine green fluorescence angiographic findings in juvenile haemorrhagic macular choroidopathy.

Six patients with juvenile haemorrhagic mascular choroidopathy were studied with fluorescein and indocyanine green fluorescence (ICG) angiography, and red-light and red-free light photography in different stages of the disease. The primary lesion consisted of multifocal, whitish, dot-like areas of choroidal infiltration showing hyperfluorescence in the late phase of the fluorescein angiograms. Red-light photographs revealed depigmentation of the pigment epithelium overlying the choroidal lesion, and clearly demonstrated the subsequent pigment-ring lesion. Fluorescein angiograms revealed subret

[Transpupillary thermotherapy for choroidal metastases].

PURPOSE: To present three cases with choroidal metastases treated with transpupillary thermotherapy (TTT). PATIENTS AND METHODS: Five choroidal metastases of three patients were treated with infrared laser thermotherapy (TTT). In all cases the distorted vision, caused by serous retinal detachment joining the posterior pole metastasis, led the patients to an ophthalmologist. In our first case an intraductal breast carcinoma led to bilateral multifocal choroidal metastases. In the second case a squamous cell carcinoma of the hypopharynx caused a rapidly growing choroidal metastasis. The primary

Neurovascular Malformations in the Fetus and Neonate.

Vein of Galen malformations are the most common congenital neurovascular malformation and are a type of choroidal arteriovenous fistula involving the midline primitive choroidal venous circulation. The arteriovenous shunt zone of a VOGM may directly involve the embryonic precursor of the vein of Galen and/or its tributaries within the 3rd ventricle tela choroidea. Dural sinus malformations are characterized by dilated intracranial dural venous sinuses, some of which acquire multifocal arteriovenous shunts within the dural walls of these overgrown venous sinuses.Pial arteriovenous fistulae are

Digital indocyanine green videoangiography of central serous chorioretinopathy.

BACKGROUND: The pathogenesis of central serous chorioretinopathy (CSC) is poorly understood. Abnormalities in the choroidal circulation have been hypothesized to be causative factors. Fluorescein angiography has not been particularly useful in identifying specific choroidal defects in CSC, largely because of inherent limitations in imaging with this technique. Recent technologic advances in digital indocyanine green videoangiography allow enhanced imaging of the choroid and other subretinal structures in comparison with fluorescein angiography. METHODS: We performed digital indocyanine green

Patterns of uveitis in patients with proven systemic (pulmonary and extrapulmonary) tuberculosis.

PURPOSE: To report patterns of uveitis in patients with systemic tuberculosis. METHODS: Records of patients presenting at uvea clinic of a tertiary eye care centre were evaluated retrospectively, and 47 cases with proven systemic tuberculosis were analyzed for patterns of uveitis. Tuberculosis had been proven with a combination of radio imaging and detection of acid fast bacilli in body fluids. All patients had been reviewed by a specialist as applicable before diagnosing tuberculosis. These patients had undergone a thorough ocular workup. Pattern of uveitis was the primary outcome measure.
