Characterization of patients with idiopathic normal pressure hydrocephalus using natural language processing within an electronic healthcare record system.

OBJECTIVE: Idiopathic normal pressure hydrocephalus (iNPH) is an underdiagnosed, progressive, and disabling condition. Early treatment is associated with better outcomes and improved quality of life. In this paper, the authors aimed to identify features associated with patients with iNPH using natural language processing (NLP) to characterize this cohort, with the intention to later target the development of artificial intelligence-driven tools for early detection. METHODS: The electronic health records of patients with shunt-responsive iNPH were retrospectively reviewed using an NLP algorith

Addressing diagnostic code variability in intimate partner violence surveillance through natural language processing: Evidence from substance use disorder populations.

BACKGROUND: Intimate partner violence (IPV) and substance use disorders (SUDs) represent major public health challenges, yet identifying IPV cases in electronic health records (EHR) remains difficult due to inconsistent documentation practices and variable billing procedures. METHODS: We constructed three patient cohorts based on SUDs or overdoses involving stimulants, opioids, or both from the EHR. Using the Open Health Natural Language Processing (OHNLP) toolkit, we developed a rule-based classifier to identify IPV in clinical notes. We validated classifier performance through manual chart

A modular pipeline for natural language processing-screened human abstraction of a pragmatic trial outcome from electronic health records.

BACKGROUND: Natural language processing allows efficient extraction of clinical variables and outcomes from electronic health records (EHRs). However, measuring pragmatic clinical trial outcomes may demand accuracy that exceeds natural language processing performance. Combining natural language processing with human adjudication can address this gap, yet few software solutions support such workflows. We developed a modular, scalable system for natural language processing-screened human abstraction to measure the primary outcomes of two clinical trials. METHODS: In two clinical trials of hospi

Natural language processing-driven knowledge graphs for transformative public health intelligence and research datasets in urgent and emergency care.

Urgent and emergency care generates large volumes of clinical data, yet much of the information most useful for research, surveillance, and service planning is recorded only in free-text clinical notes and never reaches the structured datasets used for analysis. As a result, the reasons patients present, the upstream pressures driving demand, and the care that was needed but unavailable remain largely invisible, with consequences that fall most heavily on people who move repeatedly across services. Existing initiatives connect previously separate records or predict future events, but neither r

Using natural language processing to identify patterns associated with depression, anxiety, and stress symptoms during the COVID-19 pandemic.

BACKGROUND: Combining data-driven natural language processing techniques with traditional methods using predefined word lists may offer greater insights into the connections between language patterns and depression and anxiety symptoms, particularly within specific stressful contexts. METHODS: Between 2020 and 2021, 1106 participants wrote narrative responses describing their experiences during the COVID-19 pandemic and completed the Depression Anxiety Stress Scale-21 (DASS). We investigated language patterns associated with DASS symptoms using established categories from Linguistic Inquiry a

Identifying opioid relapse during COVID-19 using natural language processing of nationwide Veterans Health Administration electronic medical record data.

Novel and automated means of opioid use and relapse risk detection are needed. Unstructured electronic medical record data, including written progress notes, can be mined for clinically relevant information, including the presence of substance use and relapse-critical markers of risk and recovery from opioid use disorder (OUD). In this study, we used natural language processing (NLP) to automate the extraction of opioid relapses, and the timing of these occurrences, from veteran patients' electronic medical record. We then demonstrated the utility of our NLP tool via analysis of pre-/post-COVI

Understanding patterns of loneliness in older long-term care users using natural language processing with free text case notes.

Loneliness and social isolation are distressing for individuals and predictors of mortality, yet data on their impact on publicly funded long-term care is limited. Using recent advances in natural language processing (NLP), we analysed pseudonymised administrative records containing 1.1 million free-text case notes about 3,046 older adults recorded in a London council between 2008 and 2020. We applied three NLP methods-document-term matrices, pre-trained embeddings, and transformer-based models-to identify loneliness or social isolation. The best-performing model, a bidirectional transformer,
