Pseudo-aneurysm of the posterior auricular artery after oncological neck dissection.

Pseudo-aneurysm of the posterior auricular artery after modified radical neck dissection is an extremely rare and potentially life-threatening complication. To our knowledge, this is the first reported case of post-neck dissection pseudoaneurysm presenting with bleeding from the orifice of Stensen's duct. A 46-year-old man underwent right submandibulectomy and modified radical neck dissection for carcinoma ex pleomorphic adenoma with cervical lymph node metastasis. On postoperative day 14, the patient presented with sudden and persistent intraoral bleeding. Examination revealed active bleeding

Making routine head and neck pathology Easy: From dissection to diagnosis.

Resections from the mobile tongue, the oropharynx and neck dissections constitute a large proportion of routine head and neck pathology workload. Histologically detected proximity to margins and prognostic factors like depth of invasion, perineural or lymphovascular invasion, or extranodal extension guide adjuvant radiotherapy and/or chemotherapy. This review discusses practical approaches to macroscopic examination of the various types of tongue and oropharyngeal resections and neck dissections. Differential inking and radial sections demonstrate proximity of tumour to margins. The macroscopi

Role of the neck dissection in early-stage lower lip cancers.

BACKGROUND: In early-stage lip cancer, spread to cervical lymph nodes is extremely rare. Elective neck treatment options include suprahyoid or supraomohyoid neck dissection, sentinel lymph node biopsy, or close follow-up. Aim: In this study, our aim was to investigate the effect of elective surgery on survival in patients operated for early-stage lip cancer. METHODS: Patients who underwent surgical treatment for lower lip squamous cell carcinoma between 2005 and 2020 were retrospectively analyzed. Age, gender, neck dissection status (yes/no), clinical and pathological T stage of the tumor, gr

Endoscopic Lateral Neck Dissection: A New Frontier in Endoscopic Thyroid Surgery.

BACKGROUND: Endoscopic thyroidectomy and robotic thyroidectomy are effective and safe surgical options for thyroid surgery, with excellent cosmetic outcomes. However, in regard to lateral neck dissection (LND), much effort is required to alleviate cervical disfigurement derived from a long incision. Technologic innovations have allowed for endoscopic LND, without the need for extended cervical incisions and providing access to remote sites, including axillary, chest-breast, face-lift, transoral, and hybrid approaches. METHODS: A comprehensive review of published literature was performed using

Effects of Central Neck Dissection on Complications in Differentiated Thyroid Cancer.

OBJECTIVE: It is still controversial whether performing central neck dissection (CND) in addition to total thyroidectomy (TT) increases the risk of complications. In the present study, we aimed to evaluate the effect of CND on the development of complications in differentiated thyroid cancer (DTC) compared to TT. MATERIAL AND METHODS: The data of 186 patients (136 females and 50 males) with a mean age of 48.73±14.78 (range, 17-82) whom were operated for DTC were evaluated retrospectively. The patients were divided into two groups; TT (Group 1) and CND±TT/Completion thyroidectomy±lateral neck

The incidence of stroke post neck dissection surgery and perioperative management.

BACKGROUND: Head and neck surgery encompasses major vessels, raising the concern of life-threatening complications such as stroke. METHODS: Databases at UPMC were queried to identify patients with both neck dissections and stroke from January 1st, 2004, to October 1st, 2020. A retrospective chart review was performed to identify patients who experienced a stroke within 30 days of a neck dissection. RESULTS: Search of a UPMC database for carotid artery stenosis (CAS), transient ischemic attack (TIA), and stroke identified 20,527 patients. After matching with the Head and Neck Tumor Registry p

Bilateral Central Neck Dissection via Transoral Approach in Papillary Thyroid Carcinoma.

BACKGROUND: The transoral endoscopic vestibular approach (TOETVA) for thyroidectomy is gaining popularity (Russell et al. in Thyroid 28(7):825-829, 2018; Le et al. in Surg Laparosc Endosc Percutan Tech 30(3):209-213, 2020; Liao et al. in Laryngoscope 130(6):1603-1608, 2020). TOETVA has been utilized successfully in performing thyroidectomy, parathyroidectomy, and neck dissection, via both endoscopic and robotic techniques (Razavi et al. in Head Neck 40(10):2246-2253, 2018; Otolaryngol Head Neck Surg 159(4):625-629, 2018; Ngo et al. in J ENT, 2020. https://doi.org/10.1177/0145561320943358 ; Ann

Elective neck dissection for T3/T4 cN0 sinonasal squamous cell carcinoma.

BACKGROUND: In locally advanced sinonasal squamous cell carcinoma (SNSCC), management of the clinically node-negative (cN0) neck is variable and elective neck dissection (END) remains controversial. METHODS: Patients with surgically treated T3/T4 cN0 M0 SNSCC were identified using the NCDB. Overall survival (OS) was assessed by Cox proportional hazard analysis in propensity score-matched cohorts. Factors associated with END were evaluated with logistic regression. RESULTS: Two hundred twenty patients underwent END (19.6%). END did not correlated with OS in propensity score-matched cohorts (H

Extent of neck dissection for patients with clinical N1 oral cancer.

BACKGROUND: No clear consensus has been reached on the indication of supraomohyoid neck dissection (SOHND) for clinically positive lymph-node metastasis. PATIENTS: Consecutive 100 patients with previously untreated oral cancer treated at Kobe University Hospital were included in this study. All patients were clinically staged as anyTN1M0 and underwent radical dissection of the primary site and level I-V neck dissection as the initial treatment. RESULTS: None of the 100 patients had pathological lymph-node metastasis (pLN) to level V. pLN to level IV was observed in two patients with tongue c

Neck Dissection for Cervical Lymph Node Metastases from Remote Primary Malignancies.

Background and Objectives: Patients with cervical lymph node metastases from remote primary tumours have poor prognoses because of the advanced stage of their cancer. Owing to recent progress in the nonsurgical management of various cancer types, options for surgical treatment to reduce tumour volume are increasing, and may help improve survival rates. For example, neck dissection may be a good option as a definitive therapy for some patients with resectable cervical metastases. We assessed patients who underwent neck dissection with curative intent and discuss the effectiveness of this approa

Evaluation of outcomes after reoperative neck dissection due to thyroid cancer.

AIM OF THE STUDY: Aim of the study was to assess the results of surgical treatment of cervical recurrences in patients with thyroid cancer. MATERIAL AND METHODS: We assessed 66 reoperations of thyroid cancer recurrences in 51 patients. Reoperative surgeries covered I-VII neck levels. RESULTS AND CONCLUSIONS: The localization of cervical recurrence and the number of removed nodes did not depend on the type of thyroid cancer. Metastatic spread was predominantly observed in the central neck. Bilateral changes tended to be observed more often in younger patients (p = 0.07). Radical neck dissecti

Feasibility of robot-assisted neck dissection followed by transoral robotic surgery.

Our aim was to evaluate the feasibility of robot-assisted neck dissection (RAND) followed by transoral robotic surgery (TORS) in treatment of cancers of the head and neck, which is expected to improve cosmesis and function. We studied 37 patients with biopsy-confirmed cNO or cN+ tumours of the oropharynx (n=22), hypopharynx (n=8), larynx (n=6), and oral cavity (n=1) who were treated by RAND then TORS from May 2010 to December 2012. Patients' characteristics and clinical details were recorded, together with operative complications and functional variables such as management of the airway and na

A role for ipsilateral, selective neck dissection in carotid body tumours.

INTRODUCTION: A reliable diagnosis of malignant carotid body tumour can only be made in the presence of metastatic disease, because the histological features of the primary tumour do not correlate with clinical behaviour. CASE REPORT: We report two cases of malignant carotid body tumour in which regional nodal biopsy at the time of excision of the primary tumour revealed unsuspected metastatic disease. DISCUSSION: Reoperation in the neck for recurrent metastatic carotid body tumour is difficult and potentially hazardous. The presence of occult metastatic disease is easily identified if a sel
