BACKGROUND: Hemorrhage is one of the most significant complications of percutaneous nephrolithotomy (PCNL) and may lead to clinically significant persistent hematuria requiring digital subtraction angiography (DSA) and transcatheter arterial embolization (TAE). Identifying predictors of TAE may improve perioperative planning and risk stratification.
OBJECTIVES: To determine predictors of need for TAE in patients evaluated with DSA for persistent post-PCNL hemorrhage.
DESIGN: Single-center retrospective observational study.
METHODS: From 2021 to 2024, a total of 2947 tubeless PCNL procedures

OBJECTIVE: To preliminarily evaluate the effectiveness and safety of transrenal antegrade ureteral occlusion as an adjunct to mini-percutaneous nephrolithotomy (mPCNL), with particular attention to stone fragment migration and perioperative outcomes.
METHODS: We retrospectively reviewed patients who underwent mPCNL for renal calculi between December 2021 and December 2023 at a single tertiary center. According to intraoperative use of an antegrade ureteral occluder, patients were assigned to an occlusion-assisted mPCNL group (OA-mPCNL) or a standard mPCNL group. Group allocation was based on

OBJECTIVES: To evaluate a novel, simplified technique for TRUE supine percutaneous nephrolithotomy (PCNL) that omits the use of bolsters or stirrups, utilizing a novel anatomical landmark ("Dixit line") for initial puncture. The outcomes of this technique are compared with those of the traditional supine PCNL approach.
MATERIALS AND METHODS: A retrospective observational study was conducted on 400 patients who underwent supine PCNL for renal or upper ureteric stones between January 2020 and January 2024. All procedures were performed by a single urologist at a tertiary care center. Patients w

Urosepsis is one of the most severe complications after percutaneous nephrolithotomy (PNL). This study aimed to develop and validate a preoperative machine learning (ML) model for predicting post-PNL urosepsis using electronic medical record (EMR) data. In this retrospective study, we included 360 patients with urosepsis and 2,636 without urosepsis. Multimodal clinical parameters were collected, including demographic data, admission vital signs, imaging reports, and laboratory results. The cohort was randomly split into a training set (80%) and a validation set (20%). Eight ML algorithms were

PURPOSE: To identify predictors of sepsis following percutaneous nephrolithotomy in patients with neurogenic bladder, without investigating causal pathways.
METHODS: We retrospectively analyzed a consecutive sample of patients with neurogenic bladder who underwent percutaneous nephrolithotomy at our referral center between 2009 and 2020. We also systematically searched PubMed until February 2025 for studies that investigated neurogenic patients undergoing this procedure. We then performed a single-arm meta-analysis of postoperative sepsis, including our own data, and meta-regressions to searc

BACKGROUND: Kidney stones account for a high proportion of urological emergencies. The main objective of this paper is to evaluate the predictive ability of five scoring systems for overall stone-free status and postoperative complications after percutaneous nephrolithotomy and retrograde ureteroscopy.
MATERIALS AND METHODS: This study retrospectively analysed 312 cases of kidney stone patients between January 2021 and May 2022 at our centre. Multivariate logistic regression as well as ROC curves were applied to determine the ability to evaluate each scale to predict stone-free rates and post

INTRODUCTION AND IMPORTANCE: Percutaneous nephrolithotomy (PCNL) is one of the most commonly performed surgeries in urology. Due to blind nature of the procedure unexpected events are inevitable. Misplacement of percutaneous nephrostomy (PCN) during PCNL into the venous system is one of the rarest complications causing great stress to both physician and the patient. Due to scarcity of data, no standard treatment has been proposed. Here, we present a case with misplaced PCN into venous system moving up to the right atrium and discuss its management with a review of the literature.

INTRODUCTION: Massive haemothorax can occur following percutaneous nephrolithotomy (PCNL), which is a significant adverse event and a life-threatening condition.
PRESENTATION OF CASE: A 65-year-old male who presented with a full right staghorn stone was treated with PCNL. Two days later, he developed massive haemothorax and conservative management with intercostal drainage failed. The patient successfully underwent video-assisted thoracoscopic surgical decortication (VATSD).
DISCUSSION: PCNL is the mainstay procedure for complex renal stones. Because it is aggressive, it can also have seriou

OBJECTIVE: To define how the learning curve for success in ultrasound-guided percutaneous nephrolithotomy (PCNL) is impacted by body mass index (BMI). Previous research has shown ultrasound-guided PCNL to be an effective method of nephrolithiasis treatment comparable to fluoroscopy-guided PCNL. A common concern for the ultrasound-guided approach is potential imaging difficulty in the obese patient population.
METHODS: A prospective cohort study of consecutive patients undergoing PCNL with ultrasound guidance for renal tract access was performed. Clinical data collected included success in gai

OBJECTIVE: To develop a technique that helps avoid colonic injury during percutaneous nephrolithotomy (PCNL) in these patients.
PATIENTS AND METHODS: PCNL was prospectively performed in a cohort of adults with renal stones in a horseshoe kidney (HSK). PCNLs were done using a standardized technique in prone position in all patients. The colon was radiologically delineated by injecting air-through a catheter in the anal canal-to help localize its position in relation to the pelvicalyceal system (PCS). Patients were observed in hospital for 48 hours postoperatively to detect any potential compli

The purpose of this study is to prospectively identify factors that predict the chance of pleural injury (detected clinically or on postoperative X-ray chest) during percutaneous nephrolithotomy (PCNL). All patients with renal/upper ureteric stones, undergoing PCNL between January 2013 and June 2014, were evaluated for pleural injury. An erect chest X-ray on inspiration was done within 6 h of PCNL. The patients were divided into Groups A and B depending on whether they developed or did not develop pleural injury. Patient-, stone-, renal-, and procedure-related factors were compared between the
