Diabetic papillopathy: two case reports in individuals with adult onset diabetes mellitus.

BACKGROUND: Diabetic papillopathy is a benign unilateral or bilateral optic neuropathy with transient optic disk edema and minimal reduction in visual function. The optic disk edema typically resolves in a few months with no resulting optic atrophy and minimal or no decrease in acuity. The exact etiology of the disk edema is unknown, but theories include retinal vascular leakage into and surrounding the optic nerve and disruption of axoplasmic flow resulting from microvascular disease of the optic nerve head vasculature. CASE REPORTS: Two adult patients receiving insulin for type II diabetes

Ocular manifestations of Mycoplasma pneumoniae infection.

Ocular manifestations of Mycoplasma pneumoniae infection, other than conjunctivitis, are uncommon. Optic disk swelling, optic nerve atrophy, retinal exudates and hemorrhages, and cranial nerve palsies have been infrequently reported. We describe a 15-year-old patient who developed bilateral optic disk edema and iritis during an acute infection with M. pneumoniae and review the world literature on findings associated with ocular manifestations of infection with this pathogen. Although our patient experienced complete resolution of iritis and optic disk edema after 6 weeks, several patients desc

[Papilledema and POEMS syndrome].

A new case of optic disk swelling associated with POEMS syndrome is described. This is a rare disease, but it should be suspected in cases of bilateral optic disk swelling when defining systemic components are present: polyneuropathy (P), organomegaly (O), endocrinopathy (E), monoclonal component (M) and skin changes (S). Usually visual acuity is maintained. However, loss of vision secondary to optic disk swelling and associated macular edema could be found.

Computed tomographic identification of calcified optic nerve drusen.

Four cases of optic disk drusen were accurately diagnosed with orbital computed tomography (CT). The radiologist should be aware of the characteristic CT finding of discrete calcification within an otherwise normal optic disk. This benign process is easily differentiated from lesions such as calcific neoplastic processes of the posterior globe. CT identification of optic disk drusen is essential in the evaluation of visual field defects, migraine-like headaches, and pseudopapilledema.

Fluorescein angiography in acute nonarteritic anterior ischemic optic neuropathy.

Fundus fluorescein angiograms from 41 patients with nonarteritic anterior ischemic optic neuropathy of less than three weeks' duration were compared with those from 43 age-matched control subjects. Patients with disease showed statistically significant delay in both the onset and the time to completion of prelaminar optic disk filling. Neither onset nor completion of peripapillary choroidal filling were markedly delayed when compared with control subjects. Frequency of occurrence of delayed filling within peripapillary choroidal watershed zones was not increased in patients with disease. There

Does optic nerve sheath diameter on MRI decrease with clinically improved pediatric hydrocephalus?

INTRODUCTION: Serial change in ventricular size is recognized as an imperfect indicator of ongoing hydrocephalus in children. Potentially, other radiographic features may be useful in determining the success of hydrocephalus interventions. In this study, optic nerve sheath diameter (ONSD), optic nerve tortuosity, and optic disk bulging were assessed as indicators of hydrocephalus control in children who underwent endoscopic third ventriculostomy (ETV) or posterior fossa tumor resection. METHODS: Sixteen children underwent ETV or tumor resection for treatment of hydrocephalus. T2-weighted axia

Morning glory disk anomaly: a computerized analysis of contractile movements with implications for pathogenesis.

Excavated optic disk anomalies, such as morning glory disk anomaly, peripapillary staphyloma, and optic disk coloboma, are rare. Contractile movement of a given lesion is even rarer. Only 4 patients with morning glory disk anomalies accompanied by contractile movement have been reported. We offer a computerized analysis of the contraction, observable on a video recording of a patient with this disorder, with an explanation of the possible pathogenesis of the disorder.

Malignant hypertension: ocular manifestations.

Malignant hypertension may be the first manifestation of systemic hypertension. We report a clinical case of a Caucasian 41-year-old man with no previous history of blood hypertension seen at casualty because of blurred vision. Fundus examination disclosed optic disk swelling, retinal hemorrhages and infarcts. The blood pressure was 220/130 mmHg. After the appropriate management of hypertension, optic disk and retinal edema resolved, leaving minor changes as mild optic disk pallor and hard exudates.

Foster Kennedy syndrome and optociliary shunt vessels in a patient with an olfactory groove meningioma.

A 48-year-old woman complained of acute loss of vision in her right eye. Ophthalmoscopically, the right optic disk appeared pale, and abnormally dilated vessels were noted on the disk. The left optic disk was reddish and swollen. Fluorescein angiography revealed abnormal vessels on the right optic disk that might be venous. The patient had right-sided anosmia. Computed tomography and magnetic resonance imaging showed a lesion in the frontal lobe that was deviated to the right and attached to the olfactory groove. A histopathologic study of the excised specimen disclosed a meningioma. We believ

The pathogenesis of reversible cupping in congenital glaucoma.

The pathogenesis of reversible cupping of the optic disk in congenital glaucoma was examined by two approaches. Human fetal, neonatal, and adult eyes were examined by histochemistry and electron microscopy to delineate the embryologic development of the optic nerve head. While the neural, glial, and vascular elements of the nerve head attain their adult configuration by midgestation, the connective tissues of the lamina cribrosa are incompletely developed at birth. The response of the optic disk cup to elevated intraocular pressure (IOP) was observed in enucleated infant and adult eyes. While

Reattachment of retina and retinoschisis in pit-macular syndrome by surgically-induced vitreous detachment and gas tamponade.

The origin of the subretinal fluid in pit-macular syndrome is unknown. Using optical coherence tomography, the authors observed that an optic disk pit is not a true pit but a cystic cavity covered with a superficial layer of the optic disk in a patient with retinal detachment and retinoschisis. The cyst was connected to the intraretinal space of the retina, which was split in several layers, or the retinoschisis in the papillomacular area. Vitrectomy and gas tamponade were performed to treat the patient. During surgically-induced vitreous detachment, strong vitreoretinal attachment at the disk

Pigment dispersion syndrome associated with optic nerve melanocytoma.

CASE REPORT: A 60-year old patient was referred for cataract surgery. The examination showed retrokeratic pigment in the left eye, which had an intraocular pressure of 24 mm Hg. The funduscopy showed a brown lesion on the left optic disk, with adjacent vitreous seeding of pigment. The patient was thus diagnosed with secondary pigment dispersion syndrome due to optic disk melanocytoma. DISCUSSION: Although melanocytoma is most commonly a benign, stationary tumor, it may present with major complications leading to significant visual loss. A patient with melanocytoma of the optic disk should be

[Clinically isolated Morning glory syndrome].

The Morning glory syndrome is an uncommon congenital optic disk anomaly. It is characterized by a funnel-shaped and enlarged dysplasic optic disk with white tissue, and retinal vessels arising from the periphery of the disk and running an abnormally straight course over the peripapillary retina. We report here the case of a 7-year-old girl with a delayed diagnosis of Morning glory syndrome discovered at a regular examination that showed amblyopia. The optic disk anomaly was isolated. The search for other associated morphological and/or functional abnormalities is mandatory (retinal and/or cere

[Qualitative morphologic characteristics of normal and glaucomatous optic papillae].

Using optic disk photographs, qualitative morphologic characteristics of the optic nerve head were determined in 251 nonselected normal eyes and 308 eyes with chronic primary open-angle glaucoma. These characteristics were correlated to morphometric (both intra- and parapapillary) and perimetric data. The highest degree of accuracy in distinguishing normal from glaucomatous optic disks was found with the characteristics "localization of narrowest point of neuroretinal rim outside the temporal horizontal optic disk sector" (85.5%) and "changes in the parapapillary retinal nerve fiber layer" (87

Disk drusen and angioid streaks in pseudoxanthoma elasticum.

Visual field loss secondary to optic disk drusen became evident before the development of angioid streaks in a patient with pseudoxanthoma elasticum. The incidence of optic disk drusen in cases of pseudoxanthoma elasticum is 20 to 50 times greater than that in the healthy population. We postulate that the abnormal aggregation of macromolecules with a high affinity for calcium (resulting in abnormalities in elastin in cases of pseudoxanthoma elasticum) also develops at the cribriform plate, disrupting axonal flow and leading to disk drusen formation. Pseudoxanthoma elasticum is associated with

Fibrovascular proliferation and retinal detachment after intravitreal injection of activated macrophages in the rabbit eye.

Injection of activated macrophages into the posterior vitreous of the rabbit induced vigorous fibrovascular proliferation over the optic disk and medullary rays, as demonstrated by 3H-thymidine autoradiography. One week after injection, endothelial cells and pericytes of the capillaries near the inner surface of the optic disk and rays were labeled; fibroblast-like cells, which were also labeled, migrated and formed vitreous strands. By the second week after injection, the fibrovascular tissue proliferated most actively, and traction medullary ray detachment and peripapillary retinal fold form

[Parapapillary retinal vessel caliber. II. Caliber reduction in eyes with glaucoma (a papillometric study of 309 eyes with chronic simple glaucoma compared with 264 normal eyes)].

The diameter of the temporal superior or inferior artery and vein was measured at the optic disk border and 2 mm from the disk center in 309 nonselected eyes with chronic primary open-angle glaucoma. The values obtained were compared with those of 264 nonselected normal eyes. The calibers of both vessels were significantly larger in the normal eyes than in the glaucomatous ones (p = 0.000 or p less than 0.01; Wilcoxon-Mann-Whitney test). Their diameters diminished significantly (p less than 0.001) with decreasing width and area of the neuroretinal rim as a whole and when divided into different
