Spin-Orbit Interaction Enabled Nonlinear Metasurface Holography.

Nonlinear optical metasurfaces have emerged as a powerful platform for efficient and multi-dimensional manipulation of harmonic waves, offering distinct advantages such as high-integration capability and phase-matching-free operation. Spin and orbital angular momentum (SAM and OAM) provide rich degrees of freedom for advanced light field control. While SAM- and OAM-multiplexing metasurface holograms have been realized in the linear optical regime, their performance is hindered by a low signal-to-noise ratio stemming from residual light mode conversion. Nonlinear OAM holography has recently bee

Dual-polarization interferometer using spin-orbit beams.

BACKGROUND: Colon cancer ranks as the third most common malignant tumor globally. Due to incomplete surgical resection and the multidrug resistance of tumor cells, it exhibits a high postoperative recurrence rate. Consequently, there is an urgent need to develop novel therapeutic strategies to inhibit postoperative recurrence of colon cancer. METHODS: Thermosensitive liposomes (Bu&Ap-Lip) co-loaded with bufalin (Bu) and apatinib (Ap) were prepared via the thin-film hydration method, with optimization of Prescription Processes. Bu&Ap-Lip was co-encapsulated with new indocyanine green (IR820) w

Multidisciplinary Management of Tumors of the Orbit.

The orbit is a complex anatomic space containing functionally important structures, including the eye, optic nerve, and extraocular muscles. Tumors of the orbit are notably varied in presentation and histologic type and can be challenging to identify, diagnose, and treat secondary to the functional importance of the eye and its surrounding structures. Multidisciplinary expertise from orbital surgeons, radiologists, oncologists, and pathologists is paramount in the timely and accurate management of orbital tumors. Fortunately, with the advancement of surgical approaches, radiation delivery, and

Juvenile Trabecular Ossifying Fibroma of the Orbit.

An 11-year-old male presented to the hospital with severe headache, nausea, and proptosis of the OD. Imaging revealed a 4 × 3 × 1 cm mass originating in the right orbital roof with adjacent bony remodeling, a calcific rim along the inferior extent of the lesion, and a heterogenous central component with hyper- and hypo-dense components. An orbitotomy with biopsy with near-total resection was performed, which yielded a diagnosis of juvenile trabecular ossifying fibroma, a tumor rarely found in the orbit. Two months later, given the concern for residual/recurrent disease based on increased signa

Venous malformation of the conjunctiva and orbit.

A 28-year-old male presented with a large conjunctival lobulated, nonpulsatile, red vascular lesion involving a large part of the temporal quadrant of the left globe. There was no proptosis or globe displacement, but the left eye abduction was limited. T2-weighted magnetic resonance imaging scan of the brain and orbit revealed an extensive contrast-enhancing lobulated lesion in the left half of the face involving the upper lip, cheek, oral cavity, extraconal space of left orbit, and nasal cavity. He underwent surgical excision of the conjunctival lesion with amniotic membrane reconstruction.

Supernumerary Medial Rectus Muscle of the Orbit.

Supernumerary extraocular muscles (EOMs) are relatively rare in humans compared to other species. Therefore, few cases are reported on pediatric patients with strabismus.  We describe a case of an incidental intraoperative finding of a right eye accessory medial rectus (MR) muscle in a child with normal ocular motility. This supernumerary muscle was found inserted underneath the original MR muscle and was of a similar size. In this article, we discuss the anomaly and its clinical relevance.

Elastoma of the Orbit: A Case Report.

A 78-year-old man presenting for revision ptosis surgery was found to have an asymptomatic left inferomedial orbital mass visible below the left lower eyelid on external inspection, and subconjunctivally on examination. This was subsequently diagnosed as an isolated elastoma. A mass in a similar location was excised 60 years previously. His other ophthalmological history included stable diplopia corrected with prism, left-sided ectropion, bilateral sequential phacoemulsification, and past bilateral ptosis which has been persistent on the left side despite surgical repair and revision. His exam

Misdiagnosis of fungal infections of the orbit.

OBJECTIVE: To evaluate the accuracy of the initial diagnosis in the case of fungal infections of the orbit and identify factors that may influence patient outcomes. METHODS: An institutional review board-approved retrospective chart review was conducted across 2 large academic centres to identify cases of fungal infections involving the orbit from January 1, 1998, to November 15, 2019. Data collected included patient demographics, past medical history, examination findings, diagnosis, treatment, imaging, and outcomes. RESULTS: Fifty cases of fungal infection involving the orbit were identifi

Transconjunctival and Transcaruncular Approaches to the Orbit.

Transcaruncular and transconjunctival approaches are commonly used by ophthalmic plastic surgeons to access various orbital compartments. These approaches are versatile and may be combined with transnasal or transcranial approaches to the orbit to gain optimal access to challenging locations. A major advantage of the transcaruncular and transconjunctival approaches is the lack of a visible skin incision with excellent postoperative cosmesis. As with all orbital surgery, an in-depth knowledge of orbital anatomy and physiology, as well as meticulous hemostasis, is needed to prevent complications

Conjunctival Melanoma Metastasis to the Contralateral Orbit.

BACKGROUND: Malignant melanoma predominantly occurs in whites, and is potentially fatal. Distant metastases often occur in lung, liver, brain, and could occur without prior regional disease. Conjunctival melanoma metastasis to contralateral orbit has never been reported. PATIENT PRESENTATION: A 60-year-old man who underwent a left primary conjunctival neoplasm resection 11 months ago presented for the evaluation of a conjunctival dark-colored mass with a bulging left lower eyelid that directly invaded the orbits bilaterally. A histopathologic examination and immunohistochemistry confirmed met

Definitive treatment of the negative vector orbit.

In a negative vector orbit, the most anterior globe portion protrudes past the malar eminence. As bulging eyes are considered unaesthetic, patients usually seek correction. However, most current correction techniques produce suboptimal results. Here, we present a surgical technique that sets back the globe and protrudes the malar bone using an intraoral approach. All five patients (aged 17-41 years) in our case series reported being satisfied with the improvement achieved using this technique. Based on our experience, a transoral approach to malar augmentation by valgisation osteotomy may addr

Invasive fungal sinusitis: treatment of the orbit.

PURPOSE OF REVIEW: To summarize diagnostic techniques for invasive fungal rhinosinusitis and provide a review of treatment options once disease has spread to the orbit. RECENT FINDINGS: Improved imaging criteria, polymerase chain reaction and other serologic tests show promise in advancing our ability to accurately diagnose invasive fungal disease. Currently, there exists three treatment options for infected orbital tissue: exenteration, conservative debridement and transcutaneous retrobulbar injection of amphotericin B. Exenteration, the most frequently reported intervention, has not been pr

Primary renal carcinoid metastatic to the orbit.

A 70-year-old man with a medical history of metastatic renal carcinoid tumor presented with several months of painless proptosis of the OS. The patient was receiving octreotide and everolimus chemotherapy. MRI revealed a well-circumscribed mass in the left lateral rectus muscle consistent with metastasis. Because the patient was asymptomatic, he was observed and maintained a stable amount of proptosis without diplopia or change in vision.

Sclerosing Lesions of the Orbit: A Review.

Orbital sclerosing inflammation is a distinct group of pathologies characterized by indolent growth with minimal or no signs of inflammation. However, contrary to earlier classifications, it should not be considered a chronic stage of acute inflammation. Although rare, orbital IgG4-related disease has been associated with systemic sclerosing pseudotumor-like lesions. Possible mechanisms include autoimmune and IgG4 related defective clonal proliferation. Currently, there is no specific treatment protocol for IgG4-related disease although the response to low dose steroid provides a good response

Primary extraosseous Ewing sarcoma of the orbit.

A 40-year-old man presented with painless, progressive vision loss and mild proptosis of the OD. CT revealed a right intraconal mass with slight penetration of the optic canal not contiguous with any bony structure. Incisional biopsy through a transfrontal orbitotomy revealed a diffuse growth of homogeneous, small, round cells. Immunohistochemical stains were positive for vimentin and MIC2 (CD99), and the translocation at EWS gene (22q12) was detected. Metastatic workup and a full-body bone scan were negative, confirming primary orbital extraosseous Ewing sarcoma. The patient received neoadjuv
