Efficacy of Polyhexamethylene Biguanide Antibacterial Gel Coating of Orthopedic Implants in Infection Control and Immune Modulation.

With the growth of the older adult population and the increasing incidence of traumatic injuries, the use of orthopedic devices has risen significantly. However, in addition to material-related limitations, one of the most serious challenges associated with these devices is the risk of infection. To address this issue, an innovative hydrogel is presented to prevent infection at the source during orthopedic surgery. This hydrogel combines polyhexamethylene biguanide (PHMB), a positively charged agent with proven broad-spectrum antimicrobial activity, with a thermosensitive polymer. The resultin

Analgesic Techniques for Managing Orthopedic Injuries: A Review for the Emergency Clinician.

BACKGROUND: Orthopedic injuries are commonly managed in the emergency department (ED) setting. Fractures and dislocations may require reduction for proper management. There are a variety of analgesic and sedative strategies to provide patient comfort during reduction. OBJECTIVE: This narrative review evaluates hematoma block, intra-articular injection, intravenous regional analgesia (IVRA) (also known as the Bier block), and periosteal block for orthopedic analgesia in the ED setting. DISCUSSION: Analgesia is an essential component of management of orthopedic injuries, particularly when redu

Frailty and social support in older orthopedic surgical patients: a cross-sectional analysis.

BACKGROUND: Frailty is a common and clinically significant condition among older adults undergoing surgical procedures and is associated with adverse postoperative outcomes. In addition to biological vulnerability, psychosocial factors such as social support may influence recovery processes. However, studies examining the relationship between frailty and perceived social support in older orthopedic surgical patients remain limited. AIM: This study aimed to examine the relationship between frailty and perceived social support in older orthopedic surgical patients and to determine how these var

Navigating ortho care amidst war crisis: insights from Al-Aqsa Martyrs' Hospital's orthopedic department at Gaza Strip.

The war in Gaza has triggered an unprecedented humanitarian crisis and severely impacted the healthcare system. It has overwhelmed orthopedic care at Al-Aqsa Martyrs' Hospital, the only public hospital serving the middle region of the Gaza Strip. With over a million displaced individuals and a sharp rise in war-related injuries, the department has been operating under extreme constraints, including shortages of medical supplies, staff, and infrastructure. The orthopedic department, initially small and under-equipped, has rapidly expanded and adapted to handle mass casualties. This has involved

Orthopedic problems in Lesch-Nyhan syndrome.

Lesch-Nyhan syndrome is an X-linked disorder of purine metabolism. The orthopedic problems and results of treatment of nine Lesch-Nyhan patients are reviewed. Associated orthopedic problems included hip subluxation or dislocation (nine of 18 hips), fractures (three), autoamputation, infections (three), minor scoliosis, and contractures. Lesch-Nyhan patients can safely undergo orthopedic procedures and the results of surgery are satisfactory and similar to those of patients with spastic cerebral palsy. All of the seven operated-on hips maintained good reduction at 6-year mean follow-up. With ad

National Trends in Utilization, Reimbursement, and Patient Demographics for Arthroscopic Subacromial Decompression, Biceps Tenodesis, and Distal Clavicle Excision in the Medicare Population.

BACKGROUND: Subacromial decompression (SAD), biceps tenodesis (BT), and distal clavicle excision (DCE) are frequently performed concomitant arthroscopic shoulder procedures. Randomized controlled trials questioning the benefit of SAD, combined with growing evidence supporting biceps tenodesis over tenotomy, have shifted clinical practice recently. This study evaluated national and regional trends in utilization, inflation-adjusted reimbursement, and patient demographics for these procedures among Medicare beneficiaries from 2013 to 2023. We hypothesized that SAD utilization would decline, BT u

Periprosthetic fractures: an unrecognized osteoporosis crisis.

Total joint replacement is common and increasing. Many of these patients have low bone mineral density preoperatively, and arthroplasty leads to bone loss. As falls are common before and after arthroplasty, it is unsurprising that periprosthetic fractures, defined as those associated with an orthopedic device, whether a joint replacement or other internal fixation devices, are not rare. These fractures engender morbidity and mortality comparable to osteoporosis-related hip fractures but remain largely unrecognized and untreated by osteoporosis/metabolic bone disease clinicians. Indeed, recent

Progressive implementation of patient blood management and declining transfusion rates in major orthopedic surgery. A single-centre experience.

INTRODUCTION: Patient Blood Management (PBM) has become an essential component of contemporary perioperative medicine. Major orthopedic surgery has historically been associated with substantial perioperative blood loss and high allogeneic transfusion rates. The aim of this study was to describe the temporal association between the progressive implementation of a multidisciplinary PBM programme and transfusion rates in major orthopedic surgery at our institution. MATERIALS AND METHODS: This retrospective observational single-centre study on annual institutional aggregated data included patient

Home Monitoring of High-Risk Individuals Receiving Opioids Post Orthopedic Surgery.

Background: Postoperative patients receiving opioids are at risk for morbidity and mortality caused by opioid-induced respiratory depression (OIRD). Guidelines advocate electronic monitoring for OIRD during postoperative hospitalization, but the utility of home monitoring following ambulatory surgery has not been assessed. We describe the utilization of capnography and pulse oximetry in an ambulatory orthopedic cohort to determine subject/home caregiver acceptance and utility of continuous monitors at home. The specific aims of this initiative were to (1) determine the subject/home caregiver a

A pictorial review of reconstructive foot and ankle surgery: elective lesser forefoot procedures.

This pictorial review focuses on basic procedures performed within the field of podiatric surgery, specifically for elective procedures of the lesser forefoot including the correction of hammertoes and lesser metatarsal deformities. Our goal is to demonstrate objective radiographic parameters that surgeons utilize to initially define the deformity, lead to procedure selection and judge post-operative outcomes. We hope that radiologists will employ this information to improve their assessment of post-operative radiographs following reconstructive foot surgeries. First, relevant radiographic mea

Redefining the Economics of Geriatric Orthopedics.

INTRODUCTION: The heath care system in the United States is in the midst of a transition, in large part to help accommodate an older and more medically complex population. Central to the current evolution is the reassessment of value based on the cost utility of a particular procedure compared to alternatives. The existing contribution of geriatric orthopedics to the societal burden of disease is substantial, and literature focusing on the economic value of treating elderly populations with musculoskeletal injuries is growing. MATERIALS AND METHODS: A literature review of peer-reviewed public

Orthopaedic surgery for patients with central nervous system lesions: Concepts and techniques.

Since ancient times, the aim of orthopedic surgery has been to correct limb and joint deformities, including those resulting from central nervous system lesions. Recent developments in the treatment of spasticity have led to changes in concepts and management strategies. The increase in life expectancy has increased the functional needs of patients. Orthopedic surgery, along with treatments for spasticity, improves the functional capacity of patients with neuro-orthopaedic disorders, improving their autonomy. In this paper, we describe key moments in the history of orthopedic surgery regarding
