Delayed repair of obstetric injuries of the anorectum and vagina. A stratified surgical approach.

PURPOSE: We categorized the various types of post-obstetric injuries of the anorectum and vagina encountered in a five-year period. The operative procedures used to repair these injuries and the functional outcome after surgery were assessed. METHODS: Between 1986 and 1991, 52 patients were surgically treated for obstetric injuries of the anorectum and vagina; 48 patients were available for follow-up study. Four clinical injury types were identified: Type I, incontinent and sphincter (11 patients); Type II, rectovaginal fistula (16 patients); Type III, rectovaginal fistula and incontinent and

Open bite in adult patients.

Anterior open bite (AOB) is characterized by the lack of overlap or contact between maxillary and mandibular incisors, while the posterior teeth are in occlusion. Correction of this malocclusion is challenging due to difficulties in determining and addressing the etiologic factors, and the high relapse rate. A multidisciplinary approach may be necessary, with participation of Orthodontics, Surgery and Speech Therapy, to achieve adequate esthetic and functional results for long term stability. The present paper discusses the treatment options for AOB, their advantages and implications.

Body dysmorphic disorder. Diagnostic issues and related disorders.

This article reviews diagnostic issues about body dysmorphic disorder and discusses its relationship to other disorders. Diagnostic and symptomatic overlap with obsessive-compulsive disorder is explored; associated features, course of illness, presumed etiology, and treatment outcome are compared; and similarities and differences are reviewed. Other illnesses with similar features, such as anorexia nervosa, social phobia, and the disorders associated with cosmetic surgery are mentioned. These findings are discussed in light of the development of DSM-IV diagnostic criteria.

Perioperative nutrition-oriented nursing for older women undergoing cancer surgery: frailty screening, enhanced recovery, and survivorship care.

Older women undergoing cancer surgery commonly enter the perioperative period with overlapping frailty, malnutrition, sarcopenia, obesity, multimorbidity, and treatment-related symptoms. These vulnerabilities reduce physiologic reserve and amplify the catabolic response to surgery, increasing the risk of complications, functional decline, delayed adjuvant treatment, and persistent nutritional impairment. Nutrition-oriented perioperative nursing provides a practical framework for translating frailty assessment and cancer nutrition guidance into coordinated care before surgery, during enhanced r

Preoperative rehabilitation and in-hospital mortality in delayed hip fracture surgery: a nationwide cohort study with stratification by kidney function.

BACKGROUND: Early surgery within 24-48 h is recommended for hip fractures; however, the majority of older adults experience delays due to medical instability. Evidence is limited on interventions during this waiting period. In cardiovascular and acute care, rehabilitation initiated immediately after admission has been shown to improve outcomes. Whether similar early inpatient rehabilitation strategies could benefit orthopedic patients has not been well explored. We aimed to assess whether preoperative rehabilitation may reduce in-hospital mortality in patients undergoing delayed hip fracture s

Robot-Assisted Ileocecal Resection for Cecal Cancer with a Common Trunk of the Middle Colic and Ileocolic Arteries: A Case Report.

INTRODUCTION: Vascular variations around the superior mesenteric vessels increase the technical difficulty and risk of injury during right-sided colon surgery. We report a rare case of cecal cancer with a common arterial trunk formed by the middle colic artery (MCA) and ileocolic artery (ICA) treated by robot-assisted surgery. CASE PRESENTATION: A man in his 70s was diagnosed with cecal cancer during evaluation for anemia. Preoperative CT revealed that the MCA and ICA originated from a common arterial trunk. Robot-assisted ileocecal resection with lymphadenectomy was performed. After identifi

The Differential Diagnosis of Coma in the ICU: Hyperacute Postoperative Guillain-Barré Syndrome-A Case Report.

Guillain-Barré syndrome (GBS) is a rare, immune-mediated neurological disorder that can be challenging to diagnose in postoperative patients due to atypical manifestations and overlapping conditions. This case report highlights the diagnostic and therapeutic challenges of GBS following vascular surgery. We present the case of a 56-year-old man scheduled for elective femoro-popliteal bypass surgery. The initial neurological development of the patient's GBS was masked by delirium requiring sedation and culminated with cardio-respiratory arrest in the 72 h following admission in the post-operativ

Laparoscopic median arcuate ligament release: Successful management of Dunbar syndrome in a young male.

INTRODUCTION AND IMPORTANCE: Median arcuate ligament syndrome (MALS), also known as Dunbar syndrome, is a rare condition caused by compression of the celiac artery and/or ganglion by the median arcuate ligament. Its incidence is poorly defined, but it typically affects lean, middle-aged women. Due to its non-specific symptoms, MALS is often underdiagnosed. Although open surgery is conventional in resource-limited settings, laparoscopic approaches offer favorable outcomes. PRESENTATION OF CASE: We report a case of a 24-year-old male with chronic postprandial upper abdominal pain and significan

[Reconstruction of the extensor tendons in revision total knee arthroplasty and tumor surgery].

Reconstruction of the extensor mechanism in extended revision after total knee replacement and tumor surgery remains a clinically relevant problem. Due to large tibial bone defects with resection of the extensor insertion area, the specific problem of patella ligament refixation frequently arises. Several biological approaches and augmentation techniques have been published. Most of these are associated with a high rate of revision surgery because of failed replacement of the extensor mechanism and unsatisfactory functional outcome. Surgical reconstruction of these tendon defects is complicate

Effects on gender identity of prenatal androgens and genital appearance: evidence from girls with congenital adrenal hyperplasia.

To address questions about sex assignment in children with ambiguous genitalia, we studied gender identity in girls with congenital adrenal hyperplasia (CAH) in relation to characteristics of the disease and treatment, particularly genital appearance and surgery. A 9-item gender identity interview was administered to 43 girls with classical CAH ranging in age from 3-18 yr, 7 tomboys, and 29 sister control girls. Groups were compared on total score and on individual items. Results showed that, on the total gender identity score, 88% of girls with CAH had scores overlapping those of control girl

Implanted microelectrode arrays in reinnervated muscles allow separation of neural drives from transferred polyfunctional nerves.

Targeted muscle reinnervation surgery reroutes residual nerve signals into spare muscles, enabling the recovery of neural information through electromyography (EMG). However, EMG signals are often overlapping, making the interpretation of limb functions complicated. Regenerative peripheral nerve interfaces surgically partition the nerve into individual fascicles that reinnervate specific muscle grafts, isolating distinct neural sources for precise control and interpretation of EMG signals. Here we combine targeted muscle reinnervation surgery of polyvalent nerves with a high-density microelect

Adult chronic rhinosinusitis.

Chronic rhinosinusitis (CRS) occurs in >10% of the adult population in Europe and the USA and can be differentiated into CRS without nasal polyps and CRS with nasal polyps (CRSwNP). Both phenotypes are characterized by a high disease burden and an overlapping spectrum of symptoms, with facial pain and loss of smell being the most differentiating. Great progress has been made in the understanding of CRS pathophysiology: from the epithelium and epithelial-mesenchymal transition to innate and adaptive immunity pathways and, finally, on the role of eosinophils and Staphylococcus aureus in the pers

Perioperative metabolic acidosis: The Bradford Anaesthetic Department Acidosis Study.

Metabolic acidosis is considered deleterious but is common in post-surgical patients admitted to intensive care unit. We evaluated the prevalence and time course of metabolic acidosis in elective major surgery, and generated hypotheses about causes, by hourly arterial blood sampling in 92 patients. Metabolic acidosis began before incision and most had occurred by the next hour. Seventy-eight per cent of patients had a significant metabolic acidosis post-operatively. Two overlapping phases were observed. The early phase started before incision, characterised by a rising chloride and falling ani

Longitudinal Changes in Cerebellar and Thalamic Spontaneous Neuronal Activity After Wide-Awake Surgery of Brain Tumors: a Resting-State fMRI Study.

Hypometabolism has been observed in the contralesional cerebellar hemisphere after various supratentorial cortical lesions. It is unknown whether the consequences of the dee- and deafferentation subsequent to wide-awake surgery for brain diffuse low-grade glioma can be assessed within remote and unresected subcortical structures such as the cerebellum or thalamus. To answer this question, we have conducted several regional analyses. More specifically, we have performed amplitude of low-frequency fluctuations (neuronal activity magnitude) and regional homogeneity (local temporal correlations) a

Lesion symptom mapping at the regional level in patients with a meningioma.

OBJECTIVE: The majority of meningioma patients suffer from presurgical cognitive deficits. Since meningiomas do not directly damage the brain, this is presumably caused by a functional integrity reduction of the surrounding brain tissue through perilesional edema and/or mass effect of the tumor. Tumor location is a key feature in determining neurological symptoms in brain tumor patients, but the relationship between meningioma location and cognitive performance remains unclear. This study aimed to identify brain areas where the presence of a meningioma forms a potential risk factor for worse c

Resection zone prediction for parenchyma-sparing hepatectomy planning: a comparative study of three modeling paradigms.

PURPOSE: Parenchyma-sparing hepatectomy planning depends on accurate resection zones that preserve functional liver tissue without compromising oncological margins. This work investigates how different levels of anatomical and functional information complexity influence resection zone prediction for parenchyma-sparing surgical planning for primary liver cancer. METHODS: We compare three modeling paradigms: a geometric distance-based approach, an explicit perfusion-based method using vascular anatomy, and a deep learning-based model built on the U-Net architecture. All methods operate on segme
