Multidisciplinary pain facility treatment outcome for pain-associated fatigue.

OBJECTIVES: Fatigue is frequently found in chronic pain patients (CPPs) and may be etiologically related to the presence of pain. Fishbain et al. have recently demonstrated that chronic low back pain (LBP) and chronic neck pain patients are more fatigued than controls. The purpose of this study was to determine whether chronic LBP- and chronic neck pain-associated fatigue responded to multidisciplinary multimodal treatment not specifically targeted to the treatment of fatigue. DESIGN: A total of 85 chronic LBP and 33 chronic neck pain patients completed the Multidimensional Fatigue Inventory

Hyperpronation and foot pain: steps toward pain-free feet.

Primary care physicians often see patients who have foot pain. Although foot disorders may have many diagnostic possibilities, the majority can be explained via the pathologic biomechanics of hyperpronation and the resulting changes in the kinetic chain. Four common problems often associated with hyperpronation are plantar fasciitis, posterior tibial tendon dysfunction, metatarsalgia, and hallux valgus. Interventions that seek to reduce hyperpronation and strengthen foot muscles are often recommended for treating foot pain.

Pain description and severity of chronic orofacial pain conditions.

A multidisciplinary pain centre study of 120 consecutive chronic orofacial pain patients assessed pain description and intensity ratings, gender differences, prevalence of concurrent conditions, and interinstrument relationships of the McGill Pain Questionnaire and visual analogue scale. Pain words chosen by patients to describe conditions were predominantly sensory words, and patients with concurrent conditions often listed words indicating a substantial affective component. Results showed pain intensity ratings of chronic orofacial pain conditions have similar or higher pain ratings when com

[Chronic facial pain due to nerve injury. Deafferentation pain].

Patients presenting chronic facial pain are very common. After profound examination a significant cause for this pain often cannot be found. Some patients are suffering from this pain due to injury of a sensible nerve and then it is called deafferentation pain. This somatic disorder can give different complaints. Pain can develop as a result of formation of neuromas 'cross-talking' of nerves and alterations in the central nerve system. It seems to be difficult to make clinical features to diagnose deafferentation pain and differentiate it from pain disorders where the psychic suffering seems t

Pain processing and pain assessment in Huntington's disease: Study protocol of the Huntington's disease - PAIN study.

BACKGROUND: Huntington's disease (HD) is an autosomal dominant neurodegenerative disease that causes motor (e.g., chorea) and non-motor symptoms (e.g., neurocognitive, neuropsychiatric and autonomic disturbances). Pain is a frequently reported non-motor symptom in HD, with a prevalence of around 40% in the manifest stage. Despite its high prevalence, patients with HD seem to be at risk for undertreatment of pain, as reflected by the lower analgesic use in HD in the advanced stages. The relatively low use of analgesics may result from under-recognition and inadequate assessment of pain, particu

Pain catastrophizing, pain anxiety, and substance use among Black individuals with chronic pain who use opioids.

OBJECTIVE: Substance use is the leading cause of preventable deaths in the U.S. Chronic pain is associated with risky substance use. Black individuals experience substantial disparities in pain and substance use outcomes and treatment. Maladaptive psychological reactions to chronic pain, such as pain catastrophizing and pain anxiety, can increase substance use among White individuals. However, no research to date has tested this among Black individuals. This study is the first to test the relationships between pain catastrophizing, pain anxiety, and substance use among Black individuals with c

Pain resilience moderates the influence of negative pain beliefs on movement-evoked pain in older adults.

Negative pain beliefs are associated with adverse pain outcomes; however, less is known regarding how positive, adaptive factors influence pain and functioning. These relationships are especially important to examine in older adults with pain, given increased disability and functional limitations in this population. We investigated whether pain resilience moderated the relationships between negative pain beliefs (fear-avoidance, pain catastrophizing) and pain outcomes (functional performance, movement-evoked pain) in sixty older adults with low back pain. Higher pain resilience was associated

Psychophysiological responses to visceral and somatic pain in functional chest pain identify clinically relevant pain clusters.

BACKGROUND: Despite chronic pain being a feature of functional chest pain (FCP) its experience is variable. The factors responsible for this variability remain unresolved. We aimed to address these knowledge gaps, hypothesizing that the psychophysiological profiles of FCP patients will be distinct from healthy subjects. METHODS: 20 Rome III defined FCP patients (nine males, mean age 38.7 years, range 28-59 years) and 20 healthy age-, sex-, and ethnicity-matched controls (nine males, mean 38.2 years, range 24-49) had anxiety, depression, and personality traits measured. Subjects had sympatheti

Quantification of abdominal wall pain using pain pressure threshold algometry in patients with chronic pelvic pain.

OBJECTIVE: Chronic pelvic pain (CPP) is a syndrome involving 1 or more pain generating organs in the pelvis, which includes pain from the lower anterior abdominal wall. This entity has been termed myofascial pain syndrome (MFPS), but its characteristics, definition, and quantification have not been well described. In this study, pain pressure threshold (PPT) testing of the lower anterior abdominal wall in CPP patients was performed to determine the range and distribution of values at each site, and the clinical utility of using PPT in a definition of MFPS. METHODS: Fifty-six patients evaluate

Baseline Pain Summation Predicts Resting but Not Movement Pain Relief After Exercise in Low Back Pain.

BACKGROUND: Central modulatory pain mechanisms may influence improvements after exercise in low back pain (LBP). This observational study explored whether temporal summation of pain (TSP) and exercise-induced hypoalgesia (EIH) are associated with improvements in resting pain and movement-evoked pain, as it may improve the understanding of variation in effectiveness after an exercise program in individuals with LBP. METHODS: At baseline and after an 8-week exercise program, ratings of resting pain and movement-evoked pain on a 0-10 numerical rating scale (NRS) were assessed in 69 individuals w

Infant pain vs. pain with parental suppression: Immediate and enduring impact on brain, pain and affect.

BACKGROUND: In the short term, parental presence while a human infant is in pain buffers the immediate pain responses, although emerging evidence suggests repeated social buffering of pain may have untoward long-term effects. METHODS/FINDING: To explore the short- and long-term impacts of social buffering of pain, we first measured the infant rat pup's [postnatal day (PN) 8, or 12] response to mild tail shock with the mother present compared to shock alone or no shock. Shock with the mother reduced pain-related behavioral activation and USVs of pups at both ages and reduced Fos expression in

Utilization of PROMIS Neuropathic Pain Quality for Detection and Monitoring Neuropathic Pain in Heel Pain Patients.

BACKGROUND: Diagnosis and management of neuropathic pain (NP) in foot and ankle patients remain challenging. We investigated the plausibility of using Patient-Reported Outcomes Measurement Information System (PROMIS) Neuropathic Pain Quality (PQ-Neuro) as an initial screening tool to detect NP and track the treatment effects. METHODS: Patients with heel pain were prospectively recruited and grouped to no-NP, mild-NP, and severe-NP based on the initial PROMIS PQ-Neuro t scores. Pain Interference (PI), Physical Function (PF), and Self-Efficacy (SE) scores were evaluated at baseline, 30-day, and

Digital Pain Drawings Can Improve Doctors' Understanding of Acute Pain Patients: Survey and Pain Drawing Analysis.

BACKGROUND: Pain drawings (PDs) are an important tool to evaluate, communicate, and objectify pain. In the past few years, there has been a shift toward tablet-based acquisition of PDs, and several studies have been conducted to test the usefulness, reliability, and repeatability of electronic PDs. However, to our knowledge, no study has investigated the potential role of electronic PDs in the clinical assessment and treatment of inpatients in acute pain situations. OBJECTIVE: The aim of this study was to evaluate whether knowledge of the patients' electronic PD has the potential to improve t

Pain catastrophizing and pain acceptance are associated with pain severity and interference among methadone-maintained patients.

OBJECTIVE: The present study examined whether pain catastrophizing and pain acceptance, two important targets of psychosocial interventions for chronic pain, are uniquely associated with pain severity and pain interference among patients on methadone maintenance treatment (MMT). METHOD: A total of 133 MMT patients who reported experiencing some pain during the previous week completed a battery of self-report measures. Multiple regression was used to test whether pain catastrophizing and pain acceptance are related to pain severity and pain interference above and beyond covariates including de

Experimental manipulations of pain catastrophizing influence pain levels in patients with chronic pain and healthy volunteers.

Pain catastrophizing (PC) has been related to pain levels in both patients experiencing acute or chronic pain and in healthy volunteers exposed to experimental pain. Still, it is unclear whether high levels of pain catastrophizing lead to high levels of pain or vice versa. We therefore tested whether levels of pain catastrophizing could be increased and decreased in the same participant through hypnotic suggestions and whether the altered level of situation-specific pain catastrophizing was related to increased and decreased pain levels, respectively. Using the spontaneous pain of 22 patients
