Complex regional pain syndrome and chronic pain management in the lower extremity.

CRPS is a difficult clinical entity to manage; it requires close and regimented follow-up to ensure that progress is being made. Early recognition is important so that treatment plans to prevent a long-standing or permanent disability made be instituted. Treatments such as physical therapy, medications, regional anesthesia, and neuromodulation may decrease their morbidity and return the patient to a more functional status.

Pain intensity and home pain management of children with sickle cell disease.

The purpose of this study was to measure the pain intensity of children with sickle cell disease during vaso-occlusive episodes (VOEs) and to describe their home pain management techniques. This research was guided by Orem's (1991) Self-Care Deficit Theory of Nursing. The instruments used were the African American Oucher Scale (Denyes & Villarruel, 1990) and a pain diary. The study's convenience sample consisted of 30 4- to 18-year-old children attending the Sickle Cell Center at a large Midwestern children's hospital. At the onset of the VOE, 43% of the children reported intense pain levels,

Pain management in chronic pancreatitis: beyond opioids.

Chronic pancreatitis is characterised by complex, multifactorial pain affecting 80-90% of patients. Pain arises from overlapping nociceptive and neuropathic mechanisms, often accompanied by CNS sensitisation and reorganisation that amplify symptoms (nociplastic pain). Opioid use is highly prevalent in chronic pancreatitis despite its many side-effects and addictive potential, and there is a need for alternative treatment strategies. Non-opioid pharmacological treatments, including neuromodulators, target specific pain mechanisms, with gabapentinoids showing the strongest evidence. Other interv

Postoperative pain management strategies in brain surgery.

The management of postoperative pain after brain surgery is challenging due to the vulnerability of neuroanatomic structures and differences in the perception of pain among different individuals. Pain management is essential for the patient's quick recovery. The main aim of the review is to assess the various pharmacological and non-pharmacological techniques for managing pain in patients after brain surgery, highlighting the potential benefits and safety. This is a focused narrative review of literature published between 2016 and 2024. A targeted search of major databases: PubMed/MEDLINE, EMB

Music-based interventions for chronic pain management.

This is a protocol for a Cochrane Review (intervention). The objectives are as follows: Primary objective To evaluate the benefits and harms of music-based interventions (MBIs) for chronic pain management in adults with pain lasting more than 12 weeks, comparing MBIs with the following comparators: active care, placebo, or no treatment/waitlist control. Specifically, we aim to assess the effects of MBIs on critical outcomes, such as pain intensity, pain-related distress, and pain interference with daily functioning. Secondary objectives To evaluate the effects of MBIs on important outcomes, in

Groin Pain and Injuries: Evaluation and Management.

Groin pain is a common symptom in athletes and the general population. Etiologies include musculoskeletal conditions involving the pubic rami, hip joints, pelvic floor, and abdominal wall, as well as hernias and genitourinary conditions. Differentiating musculoskeletal from nonmusculoskeletal symptoms and identifying a specific cause can be a diagnostic challenge. A systematic approach to physical examination can focus the differential diagnosis and guide additional evaluation and specific management. Physical examination should include direct palpation and testing for the major musculoskeleta

Establishing Buprenorphine for Acute Postoperative Pain Management.

Opioids continue to be relied on to treat postoperative pain and continue to result in harms ranging from pruritis to overdose. The partial agonist opioid buprenorphine was synthesized in the 1960s as result of a search for a safe and effective opioid analgesic. While formulations of buprenorphine are approved for the treatment of pain, it is more commonly known as a medication for opioid use disorder. However, there is increasing interest in employing buprenorphine as a front-line perioperative opioid analgesic. We review the continued reliance on full agonist opioids and associated harms, hi

Acute Low Back Pain: Diagnosis and Management.

Acute low back pain falls into two causal categories: specific and nonspecific. Specific causes can be intrinsic to the spine, from systemic disease, or referred pain from other organs. However, acute low back pain typically is nonspecific. Aside from recent trauma, most patients with acute low back pain do not require imaging unless history reveals red flag findings. Those with red flag findings require immediate evaluation and treatment, including imaging and specialty referral or consultation. For patients with nonspecific low back pain, first-line treatment involves maintaining activity, u

Diagnosis and Management of Noncardiac Chest Pain.

Noncardiac chest pain is a challenging condition often encountered by primary care providers, emergency medicine physicians, and gastroenterologists. It is frequently accompanied by persistent symptoms, diagnostic uncertainty, decreased quality of life, and high health care burden. Gastroesophageal reflux disease is the most common esophageal cause followed by functional chest pain, and at least half of patients with noncardiac chest pain have psychiatric comorbidities such as anxiety or depression. Management is focused on identification of an underlying cause to target treatment and address

Perioperative Pain Management for Distal Radius Fractures.

Distal radius fractures have a high incidence among both young and elderly patients, and in many instances require operative intervention. When operative intervention is employed, adequate pain management is essential to decrease postoperative complications, such as chronic pain and disability, while minimizing the risk of prolonged opioid use and dependence. Strategies to optimize pain management include regional anesthesia, preoperative dosing of medication, multimodal regimens, long-acting selective opioids at the time of surgery, corticosteroids, and non-pharmacologic therapies.

OTC Analgesics vs Opioids for Pain Management.

The use of opioids in acute pain may be appropriate in some situations, but there are opportunities to reduce exposure to opioids with equally effective monotherapy and combination therapy over-thecounter (OTC) medications. There are a number of OTC analgesics that are readily accessible and costeffective options to treat pain. The American College of Rheumatology Osteoarthritis Guideline "strongly" recommends the use of topical nonsteroidal antiinflammatory drugs (NSAIDs) and oral NSAIDs to treat arthritis pain, and it conditionally recommends against the use of opioids (other than tramadol).
