Purple urine bag syndrome in palliative care.

Purple urine bag syndrome (PUBS) is a rare condition characterised by urine discolouration. The management of PUBS remains controversial. Four females (mean age 84.5±9.7 years) with palliative conditions (two cancer and two non-cancer cases) presenting PUBS were identified. Urine bags were changed in all cases. Urinary catheters were changed in three cases. Oral antibiotics were prescribed in two cases and used in one case. Urine discolouration was resolved in all cases. One patient (without antibiotic treatment) died on day 5 after presentation of PUBS. Three patients (one out of three cases

Palliative Care in End-Stage Heart Failure

Heart failure (HF) is the most common cause of death. The acute syndrome of HF, better known as congestive heart failure, accounts for at least 20% of medical admissions to all hospitals for patients older than age 65 years. HF tends to have a high symptom burden (dyspnea, fatigue, edema, pain, nausea, anxiety, and depression) and periods of decompensation often requiring hospitalization. There is a high level of prognostic uncertainty, leading to significant psychosocial distress for both patients and their caregivers. Advance care planning and complex shared decision-making become increasing

Palliative Care in End-Stage Neurological Disease

Neurological disorders are among the leading causes of morbidity and death worldwide. Although stroke is the third leading cause of death, after heart disease and cancer, in the United States, other neurological diseases have a more chronic course that leads to protracted disability, morbidity, and death. Unfortunately, for many of these disorders, such as Parkinson’s disease, amyotrophic lateral sclerosis (ALS), and multiple sclerosis (MS), no cure is currently available. Patients suffer from dysarthria, dysphagia, muscle spasm, bladder and bowel difficulties, pain syndromes, and fatigue. Ade

Palliative Care in End-Stage Liver Disease

End-stage liver disease (ESLD) is the final result of various pathophysiological disturbances that underlie chronic liver diseases. ESLD is associated with significant physical and emotional distress as a result of known complication such as ascites, spontaneous bacterial peritonitis, electrolyte abnormalities, hepatic encephalopathy (also known as portosytemic encephalopathy), esophageal varices, pruritus, hepatorenal syndrome, and hepatocellular cancer. This chapter discusses the key issues related to ESLD, including pain (opioid and non-opioid pharmacological management), symptom management

[Red blood transfusion in palliative care situation].

Anemia is frequent in oncology. We debate the decision-making process of erythrocyte transfusion in palliative care situation from a case report. A patient with a prostatic metastatic cancer was in palliative situation with asthenia and coronary symptom. We analyze, in this particular case that does not describe reality of normal practice, the decision-making process of erythrocyte transfusion. These transfusions were based, in this case, on the evaluation of oncology prognosis, the short-term vital threats, life project and clinical safety of the transfusion. The patient has received 5 erythr

Paediatric palliative care: intravenous methylnaltrexone relieves constipation.

Methylnaltrexone, a peripheral opioid μ-receptor antagonist is licensed for subcutaneous administration for the treatment of severe opioid-induced constipation in adults. We describe the use of intravenous methylnaltrexone in a 3-year-old boy receiving a subcutaneous diamorphine infusion for palliation from widely metastatic alveolar rhabdomyosarcoma. The patient, who had not opened his bowels for 3 weeks despite use of regular conventional laxatives, was given a 150 mcg/kg dose via indwelling central venous catheter. Constipation was relieved within minutes of the injection. There were no sid

Surgical palliative care: recent trends and developments.

Palliation has been an essential, if not the primary, activity of surgery during much of its history. However, it has been only during the past decade that the modern principles and practices of palliative care developed in the nonsurgical specialties in the United States and abroad have been introduced to surgical institutions, widely varied practice settings, education, and research.

Medically assisted hydration for palliative care patients.

BACKGROUND: Many palliative care patients have reduced oral intake during their illness. The management of this can include the provision of medically assisted hydration with the aim of prolonging the length of life of a patient, improving their quality of life, or both. OBJECTIVES: To determine the effect of medically assisted hydration in palliative care patients on their quality and length of life. SEARCH STRATEGY: Studies were identified from searching CENTRAL, MEDLINE (1966 to 2008), EMBASE (1980 to 2008), CINAHL, CANCERLIT, Caresearch, Dissertation abstracts, SCIENCE CITATION INDEX and

Real-World Study of the Implementation of Early and Systematic Palliative Care for Advanced Colorectal Cancer Care-The Palliative Care Early and Systematic Project.

PURPOSE: To develop and evaluate real-world interventions to move guidelines on timely palliative care utilization into practice. METHODS: A knowledge-to-action cycle theory-informed, process improvement project facilitated implementation of a clinical practice guideline by (1) systematically screening clinic patients for unmet palliative care needs and alerting the oncologist, (2) using a dedicated community-based palliative care nurse for each referral, and (3) using Shared-care letters (structured communication between oncologist, family physician, and patient) to enhance information excha

Implementing palliative reasoning in multiprofessional palliative care: A participatory action research in the Netherlands.

CONTEXT: In the Netherlands, two interventions exist to support healthcare professionals in assessing palliative care needs and providing multidimensional generalist palliative care. Separate use hindered communication, interprofessional collaboration, and multidimensional care. To address this, the two existing interventions were integrated into a single multiprofessional intervention, Palliative Reasoning. The Palliative Reasoning Working Method (PRWM) was developed to support its implementation in daily practice. OBJECTIVES: To iteratively adapt and implement the PRWM in primary care, hosp

Hospice and Palliative Care Experiences as Expressed on the r/hospice and r/palliative Subreddits.

Purpose: Hospice care is widely recognized as end-of-life support. Reddit, an anonymous online community forum, provides space for sharing medical information, seeking emotional support, and facilitating open communication. This qualitative study examined the most popular questions on the r/hospice and r/palliative communities on Reddit. Methods: Original posts from the r/hospice and r/palliative subreddits published between April and October 2024, with at least 10 comments, were analyzed. Posts were included if they asked questions or sought advice, included several standard keywords and phra

[Outpatient palliative care for non-oncological diseases].

The primary function of outpatient palliative care is to empower patients with life-threatening illnesses to optimize their quality of life by maximising their independence in their own home environment. In addition to addressing oncological diseases, the attention for palliative care towards non-oncological diseases is growing, a trend that is especially relevant for outpatient care. The early identification of the need for palliative care is of critical importance, as is the provision of effective palliative care within appropriate structures.

Psychedelic drugs in palliative care: research agenda.

Patients with life-limiting illnesses often experience profound emotional and spiritual challenges that conventional palliative care treatments may not fully alleviate. Psychedelics, with their potential to facilitate deep psychological insight, offer a promising approach to enhancing end-of-life care. Despite growing interest and emerging psychiatric applications, their use in palliative settings remains poorly understood, with existing studies limited by small sample sizes and low methodological quality. This article outlines a research agenda that prioritises adequately powered randomised c
