Is schizophrenia disappearing? The rise and fall of the diagnosis of functional psychoses: an essay.

BACKGROUND: The categories of functional psychoses build on views of influential professionals. There have long been four main categories - affective, schizophrenic, schizoaffective/cycloid/reactive/polymorphic, and delusional/paranoid psychoses. The last three are included in "psychotic disorders". However, this dichotomy and the distinctions between categories may have been over-estimated and contributed to lack of progress. TEN TOPICS RELEVANT FOR THE DIAGNOSIS OF FUNCTIONAL PSYCHOSES: 1. The categories of functional psychoses have varied with time, place and professionals' views, with mov

Paranoid schizophrenia: prognosis under DSM-II and DSM-III-R.

DSM-III-R subtype-specific criteria and Tsuang-Winokur demographic criteria for paranoid schizophrenia were applied to records of discharged schizophrenic inpatients diagnosed according to DSM-II. Those meeting DSM-III-R subtype-specific criteria for paranoid schizophrenia and those who did not, ie, they received more hopeful diagnoses, had equally good prognoses. Good outcome was associated with the presence of prior affective symptomatology, absence of formal thought disorder, or other negative-type symptoms as well as Tsuang-Winokur demographic criteria of onset after age 25, married or emp

Dopaminergic, Noradrenergic, Adrenal, and Thyroid Abnormalities in Psychotic and Affective Disorders.

BACKGROUND: This study aimed to assess hypothalamic-pituitary dopaminergic (DA), noradrenergic (NA), thyroid (HPT), and adrenal (HPA) activity in schizophrenia, in schizoaffective disorder, and in bipolar disorder. METHOD: We investigated a combined approach of hormone responses to (1) apomorphine (APO), a short-acting DA receptor agonist which decreases prolactin secretion (PRL), and stimulates secretion of growth hormone (GH), adrenocorticotropin (ACTH), and cortisol; (2) clonidine (CLO), an alpha 2-adrenoceptor agonist which stimulates GH secretion; (3) 8 AM and 11 PM protirelin (TRH) whic

Monosymptomatic hypochondriacal psychosis.

In brief, therefore, we are dealing with an illness characterised by a single delusional system - in this instance with hypochondriacal content - which can occur at any age from late adolescence onwards, appears to affect the sexes equally, and has a very poor prognosis without treatment. Its presentation appears to be relatively independent of cultural factors. A previous history, or a family history, of psychotic illness seems very uncommon. However, substance abuse and/or head injury seem to be background factors in a high proportion of younger patients and I would speculate about the role

Multiple sclerosis and Capgras' syndrome.

Psychotic disorders in patients with multiple sclerosis (MS), although reported in the literature, are quite rare. The maniac psychosis is increased in MS patients, especially after steroid use, but a pure paranoid (delusional) state is very uncommon. We report a case of a patient with MS complicated by Capgras' syndrome. This disorder, characterized by misidentification and also known as "illusion of double", was first described by the French psychiatrist Joseph Capgras in 1923. Our patient was a 36-year-old female, with a negative psychiatric history; the diagnosis of MS dated back to the ag

[Mental retardation as a risk factor to develop a psychotic disease].

INTRODUCTION: One of the main aims of research on schizophrenia has been to pinpoint the early symptoms and signals of the disease before its appearance. OBJECTIVES: We have examined the diagnoses previously given to patients before they were diagnosed of schizophrenia. METHOD: This is a case-control study in which we used a data register including the fields of minimum basic data set (MBDS) whose time period included 1999 to 2005. RESULTS: In our study, there was a 3.6% frequency of mental retardation and 2.1% one of behavioral and emotional disorders with onset usually occurring in childh

Forensic importance of jealousy.

The aim of the investigation is to define as clearly as possible specific forensic psychiatric characteristics of persons who committed homicide and or attempted due to jealousy (the nature and severity of psychopathology, the level of responsibility, danger for the community, intensity and nature of aggression, the victimologic dimension, the relation of alcohol and jealousy). A retrospective method based on forensic psychiatric expertises in the period 1975-1999 was used. They encompassed 200 examinees that committed murder or attempted it. The results show the connection of psychotic jealou

Diagnostic Task Specific Activations in Functional MRI and Aberrant Connectivity of Insula with Middle Frontal Gyrus Can Inform the Differential Diagnosis of Psychosis.

We constructed a novel design integrating the administration of a clinical self-assessment scale with simultaneous acquisition of functional Magnetic Resonance Imaging (fMRI), aiming at cross-validation between psychopathology evaluation and neuroimaging techniques. We hypothesized that areas demonstrating differential activation in two groups of patients (the first group exhibiting paranoid delusions in the context of paranoid schizophrenia-SCH-and second group with a depressive episode in the context of major depressive disorder or bipolar disorder-DEP) will have distinct connectivity patter

[Acute delusional disorder with dissociative symptoms--case report].

Delusions are amongst the main symptoms of schizophrenia. Delusional states may be caused by stress, somatic illness, or may be idiopathic, such as the persistent delusional disorder. Determination of a cause is usually difficult, but it is really important for the treatment. A case report of acute delusional disorder complicated by dissociative symptoms, suggesting an organic cause of the disorder is presented. Delirium caused by neuroleptic intoxication was an additional complication. Detailed informations gathered from relatives and psychological examinations made it possible to determine p

Childhood verbal abuse and risk for personality disorders during adolescence and early adulthood.

Data from a community-based longitudinal study were used to investigate whether childhood verbal abuse increases risk for personality disorders (PDs) during adolescence and early adulthood. Psychiatric and psychosocial interviews were administered to a representative community sample of 793 mothers and their offspring from two New York State counties in 1975, 1983, 1985 to 1986, and 1991 to 1993, when the mean ages of the offspring were 5, 14, 16, and 22 years, respectively. Data regarding childhood abuse and neglect were obtained from the psychosocial interviews and from official New York Sta

Race and self-reported paranoia: Increased item endorsement on subscales of the SPQ.

There is a dearth of research examining how individual-level and systemic racism may lead to elevated diagnostic and symptom rates of paranoia in Black Americans. The present study employed item response theory methods to investigate item- and subscale-level functioning in the Schizotypal Personality Questionnaire (SPQ) in 388 Black and 450 White participants across the schizophrenia-spectrum (i.e., non-psychiatric controls, individuals with schizophrenia, schizoaffective disorder, or schizotypal personality disorder). It was predicted that (1) Black participants would score significantly high

Tracing the Roots of Dementia Praecox: The Emergence of Verrücktheit as a Primary Delusional-Hallucinatory Psychosis in German Psychiatry From 1860 to 1880.

While the roots of mania and melancholia can be traced to the 18th century and earlier, we have no such long historical narrative for dementia praecox (DP). I, here, provide part of that history, beginning with Kraepelin's chapter on Verrücktheit for his 1883 first edition textbook, which, over the ensuing 5 editions, evolved into Kraepelin's mature concepts of paranoia and paranoid DP. That chapter had 5 references published from 1865 to 1879 when delusional-hallucinatory syndromes in Germany were largely understood as secondary syndromes arising from prior episodes of melancholia and mania i

Childhood trauma, psychosis liability and social stress reactivity: a virtual reality study.

BACKGROUND: Childhood trauma is associated with higher risk for mental disorders, including psychosis. Heightened sensitivity to social stress may be a mechanism. This virtual reality study tested the effect of childhood trauma on level of paranoid ideations and distress in response to social stress, in interaction with psychosis liability and level of social stress exposure. METHOD: Seventy-five individuals with higher psychosis liability (55 with recent onset psychotic disorder and 20 at ultra-high risk for psychosis) and 95 individuals with lower psychosis liability (42 siblings and 53 con

Relative potency of amphetamine isomers in causing the serotonin behavioral syndrome in rats.

High doses of amphetamine evoke a behavioral syndrome in man which is similar to paranoid schizophrenia. The similarities between the behavioral effects of amphetamine and the symptoms of an endogenous behavior disorder suggest a mechanistic relationship between the two states. This study deals with a specific set of behavioral signs in the rat which are caused by relatively high doses of amphetamine and are mediated by serotonin receptor activation. Experiments on the mechanism by which amphetamine evokes this behavioral syndrome reveal that the l-isomer, like the d-isomer, acts indirectly, i

[Personality disorders in a nonclinical sample of adolescents].

OBJECTIVE: To examine the comorbidity of borderline personality disorder and other personality disorders in a nonclinical sample of high-school students. METHOD: 311 high-school students who completed the French version of the CES-D (Center for Epidemiological Studies-Depression Scale), were asked to participate to interviews evaluating personality functioning: 60 subjects (19%) accepted to participate in the study. The mean CES-D score of these 60 subjects (16 boys, 44 girls, mean age=17.7 1.7) was significantly higher than the mean score of the whole sample (23.9 10.4 versus 16.7 9.8). Thus

[Diagnosis and therapy of paranoid syndromes in the elderly].

Paranoid syndromes in the elderly are nosologically unspecific. Delusional disorder is characterized by the presence of delusions, typically persecutory, commonly in their homes. Frequent risk factors are discriminating, insulting or threatening life events, social isolation as well as sensory impairments. Atypical antipsychotics are considered as first line treatment. Delusion of parasitosis and erotomania are presented as subtypes of delusional disorders.

Decline in the diagnosis of schizophrenia among first admissions to Scottish mental hospitals from 1969-78.

Annual age-standardised first admission rates from 1969-78 for Scottish mental hospitals were calculated for schizophrenia, paranoid states, reactive psychoses, all affective psychoses, mania, and depressive neuroses. Significant decreases were found in the diagnosis of schizophrenia (P less than 0.001) and, to a lesser extent, affective psychoses (P less than 0.01) and depressive neuroses (P less than 0.02). The incidence of paranoid states, reactive psychoses, and mania did not change significantly. Several factors possibly contributing to the decline in diagnoses of schizophrenia are discus
