Fellowship at Home: A Model for Expanding Neurosurgical Capacity in Low- and Middle-Income Countries.

BACKGROUND AND OBJECTIVES: Short-term global neurosurgical missions and off-site fellowships rarely achieve durable independence in low-resource settings. Sustainable capacity building requires longitudinal, locally embedded training that integrates technical skill with system development. The objective of this study was to evaluate whether a locally embedded "Fellowship at Home" model could establish sustainable independent cerebrovascular surgical capacity and identify factors associated with success. METHODS: We established a longitudinal partnership (2022-present) between Brigham and Wome

Perspectives on Using Pharmacogenomics to Guide Tobacco Cessation: Survey Results From an American Indian Community.

Pharmacogenomics research has predominantly focused on populations of European ancestry, limiting the application to diverse populations such as American Indian and Alaska Native (AIAN) communities. Our community-centric study aims to understand perspectives on utilizing pharmacogenomics to guide tobacco cessation in an AIAN community using a survey with qualitative and quantitative components. We assessed participant (n = 273) tobacco usage and cessation history, pharmacogenomics knowledge, and perceptions of utilizing pharmacogenomics in the context of tobacco cessation. We found that the ma

Nutrition guidance in The Netherlands: the role of the GP in the translation from population strategy to individual approach.

An important pitfall of nutritional guidance in medical practice is how to deal with the prevention paradox: a nutritional advice that is good for the population as a whole is not necessarily proven effective for the individual patient. Evidence-based guidelines are needed to support GPs to translate these advices to the individual patient. We illustrate this with two examples: obesity and undernutrition. The Dutch Ministry of Health started a national partnership on overweight. The role of the Dutch College of GPs (NHG) in this process is to insert the GP's perspective and to 'translate' the

Syndemic anemia in British Malaya: An early global health encounter with hookworm and malaria co-infections in plantation workers.

With the establishment of the International Health Commission in 1913, the Rockefeller Foundation sought governmental partnerships overseas to combat hookworm disease and improve feces disposal practices. In the Madras Presidency in British India, the India Research Fund Association carried out hookworm surveys but failed in its educational efforts to improve feces disposal practices. In British Malaya, the Uncinariasis [Hookworm] Commission to the Orient discovered a syndemic of anemia among Tamil plantation laborers from the Madras Presidency and Chinese laborers from southern China who suff

Providing care for children and adolescents facing homelessness and housing insecurity.

Child health and housing security are closely intertwined, and children without homes are more likely to suffer from chronic disease, hunger, and malnutrition than are children with homes. Homeless children and youth often have significant psychosocial development issues, and their education is frequently interrupted. Given the overall effects that homelessness can have on a child's health and potential, it is important for pediatricians to recognize the factors that lead to homelessness, understand the ways that homelessness and its causes can lead to poor health outcomes, and when possible,

Developmental venous anomalies and cerebral cavernous malformations: Partners in crime.

Developmental venous anomalies (DVAs) are benign congenital veins that collect normal brain drainage into a single outlet. Cerebral cavernous malformations (CMs) are clusters of thin-walled capillary cavities prone to bleeding. When both lesions coexist, the DVA's altered venous pressure and flow can promote CM formation or rupture. Detecting a DVA abutting an otherwise unexplained intracerebral hemorrhage can therefore raise suspicion of an occult CM as a likely cause, a clue which may be invaluable for daily clinical practice. The main focus of this review is to acknowledge the hallmark imag

Sustaining presence under depletion: A grounded theory study of family-centered care, compassion fatigue, and nursing-sensitive quality outcomes in pediatric settings.

BACKGROUND: Pediatric nurses face challenges delivering family-centered care while managing compassion fatigue, with potential consequences for care quality and patient safety. AIM: This study explored the social processes through which pediatric nurses navigate family-centered care and compassion fatigue, and how these were perceived to shape nursing-sensitive quality outcomes. METHOD: A constructivist grounded theory design was employed. Twenty-two pediatric and neonatal intensive care nurses participated in semi-structured interviews. Data collection and constant comparative analysis occu

Scaling Mindfulness for Youth Well-Being: A District-Community Partnership Model.

BACKGROUND: Youth in the United States face unprecedented levels of emotional distress, highlighting the need for comprehensive school-based approaches to well-being. Mindfulness-based programs (MBPs) are developmentally appropriate strategies for fostering resilience, yet large-scale implementation remains challenging. CONTRIBUTIONS TO PRACTICE: A long-term partnership between a public school district and a community behavioral health agency supported the expansion of mindfulness programming across elementary schools through a facilitator-supported delivery model and intensive instructor pre

25 Years of Progress: ELNEC and AACN Transforming Palliative Nursing Education.

In the year 2000, leaders with the American Association of Colleges of Nursing joined with Dr. Betty Ferrell and her colleagues at City of Hope to address gaps in how nurses are educated to care for patients at the end-of-life and their families. The collaborators launched a far-reaching train-the-trainer initiative, the End-of-Life Nursing Education Consortium (ELNEC), which resulted in competencies, guidelines, and nursing curricula developed to prepare nurses to provide evidence-based, skilled, and compassionate palliative care. The 25th anniversary of ELNEC celebrates the tremendous effort

Advocacy around abortion in a politically charged climate.

PURPOSE OF REVIEW: To examine the evolving role of clinician advocacy in abortion care and to highlight key domains through which obstetrician-gynecologists can influence policy, clinical practice, and patient outcomes. RECENT FINDINGS: Increasing state-level abortion restrictions have significantly altered the landscape of reproductive healthcare in the USA. Emerging evidence demonstrates worsening barriers to care, delays in treatment, and increased morbidity, particularly for marginalized populations. Clinicians are responding through a range of advocacy strategies, including interpersonal

Podcast on Cross-speciality Perspectives on Practical Management of Atopic Dermatitis-Associated Ocular Surface Diseases.

There is a growing awareness among dermatology providers of ocular comorbidities in patients with the chronic inflammatory skin disease atopic dermatitis (AD). For example, the prevalence of ocular surface diseases (OSD) such as conjunctivitis is higher in patients with AD than in the general population, and the use of some AD treatments may be associated with OSD. In a recent review published in the Journal of the American Academy of Dermatology, dermatologists and ophthalmologists provided an overview of the different types, etiology, pathophysiology, and practical management of OSD associat

The generative potential of mess in community-based participatory research with young people who use(d) drugs in Vancouver.

Community-based participatory research (CBPR) is increasingly standard practice for critical qualitative health research with young people who use(d) drugs in Vancouver, Canada. One aim of CBPR in this context is to redress the essentialization, erasure, and exploitation of people who use(d) drugs in health research. In this paper, we reflect on a partnership that began in 2018 between three university researchers and roughly ten young people (ages 17-28) who have current or past experience with drug use and homelessness in Greater Vancouver. We focus on moments when our guiding principles of

Real world challenges in delivering person-centred care: A community-based case study.

Community nurses face many challenges when trying to practice evidence-based, person-centred care. Ongoing concerns regarding the impact of the 2013 Francis Report ( Ford and Lintern, 2017 ) suggest that individualised and holistic care is an impossible dream, one made harder when the client appears uncooperative. This paper presents a case study that sets out how some of these challenges were met in a potentially difficult situation experienced by a student nurse and her mentor in practice, in which the student was supported to further examine and explore issues that may have influenced the s

Breech delivery in twenty years of practice.

A major area of controversy and concern in current obstetrics practice is the management of breech presentation. Studies showing a threefold to fourfold increased morbidity rate in breeches delivered vaginally have led to striking increases in cesarean section rates. The present study comprises a review of all breech deliveries in a partnership practice through the years 1957 to 1976 to permit a follow-up period of at least 2 years. One hundred forty-one breech presentations (excluding twin pregnancies) from a total of 5,320 viable deliveries (2.6%) were managed by one of four obstetrician-gyn

Innovations in plans of safe care implementation for infants with prenatal substance exposure.

INTRODUCTION: Federal legislation requires states to notify child welfare agencies of all newborns affected by prenatal substance exposure and to develop a plan of safe care (POSC). However, there is substantial variation in how states implement POSC and no best-practice standards. This study explored innovative state practices to understand how POSC policies can best support affected children and families across policy, health system, and child welfare contexts. METHODS: We conducted qualitative interviews with national experts on perinatal substance use policy, as well as state officials an

Prioritising Strategies to Strengthen Domiciliary Oral Care for Older Adults in Malaysia: A Stakeholder Co-Creation Study.

INTRODUCTION: Domiciliary oral care services (DOMS) are important for older adults who are frail, homebound or dependent on others for access to care. In Malaysia, implementation remains limited despite growing oral health needs in an ageing population. This study aimed to co-create and prioritise practical strategies to strengthen DOMS for older adults in Malaysia. METHODS: A qualitative participatory co-creation design was used. Twelve purposively selected stakeholders from academia, clinical practice, professional bodies and community-related sectors participated in a multi-stage process i

Exploring Children's Physical Activity Experiences in After-School Programs: Insights from a University-Community Partnership Initiative.

This study aimed to comprehensively understand how children engaged with physical activity in after-school programs developed through a university-community partnership initiative. The program was designed to enhance physical activity opportunities for elementary school students by offering a variety of structured and unstructured activities, facilitated by physical education teacher education (PETE) major students serving as mentors. A mixed-methods approach was employed, using both quantitative and qualitative data. Seventy children completed the Physical Activity Enjoyment Scale (PACES) bef

Collaborative development of an innovative virtual research recruitment strategy through an academic/clinical partnership.

PURPOSE: Recruitment for research studies is the crucial first step and often the most challenging one. A major shift in recruitment methods for research was necessitated by the onset of the COVID-19 pandemic. Our goal is to describe lessons learned and the success rate of virtual research recruitment compared with other research recruitment strategies employed by our Academic/Clinical Partnership research team. METHODS: A descriptive design was employed to assess the success of in-person, mailed introductory letters with follow-up telephone calls and virtual recruitment strategies. The poten
