Tibial tuberosity-trochlear groove distance in Chinese patients with or without recurrent patellar dislocation.

PURPOSE: To compare the tibial tuberosity-trochlear groove (TTTG) distance in Chinese patients with or without recurrent patellar dislocation. METHODS: The TTTG distance of 14 knees in 5 men and 9 women aged 16 to 38 years with recurrent patellar dislocation were compared with that of 73 control knees using magnetic resonance imaging. RESULTS: The mean TTTG distance was significantly greater in patients with recurrent patellar dislocation than in controls (16.4 mm vs. 10.1 mm, p<0.001). Six (43%) knees with recurrent patellar dislocation had the TTTG distance greater than the cut-off thresho

Multidetector computed tomography evaluation of bony fragments and donor sites in acute patellar dislocation.

BACKGROUND: Patellar dislocation is frequently associated with bony fragments that are difficult to see on radiographs. MRI or MDCT are often used to rule out or characterize these bony fragments. PURPOSE: To assess the use of MDCT for locating bony fragments and donor sites in patients with acute patellar dislocation, and to test whether sizes and quantity of bony fragments differ between first-time dislocations and recurrent dislocations. MATERIAL AND METHODS: Retrospective data from two hospitals during a 96-month period were collected, and a total of 46 patients (mean age 27 years, range

Combination of medial patellofemoral ligament reconstruction with vastus medialis advancement for chronic patellar dislocation.

BACKGROUND: The medial patellofemoral ligament (MPFL) reconstruction is popular in clinical practice for chronic patellar dislocation; however, the combination with vastus medialis advancement is rare. The aim of this study was to evaluate the clinical outcome of the combination of MPFL reconstruction with vastus medialis advancement. METHODS: We retrospectively analyzed 69 patients with chronic patellar dislocation between July 2004 and October 2008: twenty eight cases with isolated MPFL reconstruction (group I), forty one cases with the combination of MPFL reconstruction with vastus mediali

Quadriceps pull test: an outcome predictor for lateral retinacular release in recurrent patellar dislocation.

Recurrent dislocation of the patella is a disorder with a complex aetiology. Several surgical procedures have been recommended for its correction, but there are few clinical or radiological guidelines to facilitate the selection of a particular procedure. The results of treating a series of 29 knees are presented with a follow-up of at least 2 years. A significant correlation has been found between the horizontal movement of the centre of the patella on quadriceps contraction (quadriceps pull test) and surgical outcome when lateral release alone is selected for the correction of recurrent pate

Isokinetic knee extension strength and pain before and after correction of recurrent patellar dislocation.

The maximal isokinetic knee extension strength was measured in 20 patients before and after surgical correction of recurrent dislocations of the patella and the pain evoked during the recordings was rated. The preoperative muscle strength was significantly higher and pain was less severe than in a previously studied group of patients with chondromalacia or patellofemoral osteoarthrosis. Twenty months after patella realignment by a new surgical procedure including an extensive lateral release and anteromedial displacement of the tibial tuberosity by an oblique osteotomy through the anterior cre

Locked patellar dislocation due to osteophytes: two case reports with a focused literature review.

INTRODUCTION: Locked patellar dislocation is a rare injury in which the patella becomes irreducible due to mechanical obstruction. Osteophytes have been increasingly recognized as a cause of irreducibility, but the mechanisms remain incompletely characterized. CASE PRESENTATION: Case 1: A 41-year-old female sustained a locked lateral, vertical, intra-articular patellar dislocation following a motorcycle collision. An inferomedial patellar osteophyte was impacted against the medial femoral condyle and entrapped within soft tissue, requiring open reduction. Case 2: A 57-year-old female presente

Extreme trochlear morphology in pediatric patients with fixed lateral patellar dislocation: a radiographic study.

BACKGROUND: Fixed lateral patellar dislocations (FLPD) are a rare subtype of chronic patellofemoral instability (PFI) where the patella remains dislocated in flexion and extension. The aim of this study was to describe radiographic morphological features in pediatric patients with FLPD and determine how they differ from those with acute patellar dislocation (APD) and unaffected controls. METHODS: Pediatric patients who underwent medial patellofemoral ligament reconstruction for FLPD or APD were identified and age- and sex-matched to controls without PFI. On axial magnetic resonance imaging, b

Patellar dislocation after total knee Arthroplasty: Stabilization technique using medial flap and suture anchors.

INTRODUCTION: Patellar dislocation following total knee arthroplasty (TKA) is an uncommon but functionally significant complication. Its management requires careful evaluation of prosthetic alignment, patellofemoral mechanics, and soft tissue integrity. The aim of this study was to describe a surgical stabilization technique using a medial retinacular flap fixed to the patella with suture anchors, combined with controlled lateral retinacular release, and to analyze its clinical and functional outcomes. MATERIALS AND METHODS: We conducted a retrospective case series between 2011 and 2023 in tw

Multimodal Data and Deep Learning-Driven Diagnostic and Therapeutic Assistance Framework for Patellar Dislocation.

This study aims to combine multimodal data and artificial intelligence algorithms to construct a model for identifying anatomical risk factors, predicting recurrence risk, and assessing postoperative outcomes. The proposed framework integrates preoperative, intraoperative, and postoperative information through a three-stage modeling framework. In the recurrence risk prediction module, we specifically incorporated lipid metabolism profiles and clinical variables, achieving an area under the curve (AUC) of 0.83, an F1-score of 0.68, and sensitivity of 0.82. Notably, younger patient age and eleva

Traumatic Popliteal Artery Occlusion Following Lower Extremity Crush Injury Presenting With Isolated Patellar Dislocation.

While traumatic popliteal artery injury historically has a low incidence, failure to acutely recognize the vascular insult poses a significant risk of limb loss and functional impairment. A 71-year-old male presented with left lower extremity pain in setting of a crush injury working underneath a vehicle resulting in an isolated lateral dislocation of his patella and complete occlusion of the distal popliteal artery. He was taken to the operating room for an in-situ bypass and four-compartment fasciotomy. His hospital stay included three staged washouts/debridements with eventual closure. He w

Patellar Dislocation in Adolescent Patients: Influence on Cartilage Properties Based on T1ρ Relaxation Times.

BACKGROUND: Adolescents who experience a patellar dislocation have an elevated risk of patellofemoral posttraumatic osteoarthritis. Magnetic resonance imaging (MRI)-based T1ρ relaxation times were measured for adolescents to evaluate patellofemoral cartilage after patellar dislocation. Long T1ρ relaxation times are an indicator of cartilage degradation. HYPOTHESIS: The primary hypothesis is that patellofemoral cartilage T1ρ relaxation times will be elevated in the acute phase after patellar dislocation. The secondary hypothesis is that T1ρ relaxation times will be higher for knees with multip
