Improving outcomes in adult patients who self-harm-evaluating a brief psychological intervention in emergency departments (ASSURED): protocol of a randomised controlled clinical trial.

BACKGROUND: Patients with self-harm and suicidal ideation are increasingly presenting in emergency departments (ED) in the UK. Self-harm is the strongest risk factor for suicide. Currently, there are no evidence-based interventions for self-harm offered in the context of general hospitals in the UK. This trial, funded by the National Institute for Health and Care Excellence (NIHR), aims to assess the clinical and cost-effectiveness of the ASSURED intervention. The ASSURED intervention includes up to five rapid follow-up contacts, comprising a narrative interview and enhanced safety planning se

Guanfacine as a Treatment Option for Persistent Mental Restlessness and Deliberate Self-harm Thoughts in a Patient with Attention-deficit/Hyperactivity Disorder, Autism Spectrum Disorder, Borderline Personality Disorder, and Post-traumatic Stress Disorder: Case Report.

Deliberate self-harm (DSH) in adolescents presents a significant clinical challenge, particularly when compounded by comorbidities such as attention-deficit/hyperactivity disorder (ADHD), autism spectrum disorder (ASD), borderline personality disorder (BPD), and post-traumatic stress disorder (PTSD). We describe a case of a 16-year-old transgender Māori male with ADHD, ASD traits, BPD, and PTSD, who presented with persistent mental restlessness experienced as intrusive DSH ideation. He had limited benefit and intolerable side effects with atomoxetine but responded to guanfacine modified-releas

Development of culturally-appropriate text message booster content to follow a brief intervention focused on reducing alcohol related harms for injury patients in Moshi, Tanzania.

Alcohol use is a risk factor for death and disability and is attributed to almost one-third of injury deaths globally. This highlights the need for interventions aimed at alcohol reduction, especially in areas with high rates of injury with concurrent alcohol use, such as Tanzania. The aim of this study is to create a culturally appropriate text messages as a booster to a brief negotiational intervention (BNI), to in the Emergency Department of the Kilimanjaro Christian Medical Centre, Moshi, Tanzania. Creation of text message boosters for an ED-based intervention expands the window of opportu

Management of patients at risk of harms from both continuing and discontinuing their long-term opioid therapy: A qualitative study to inform the gap in clinical practice guidelines.

BACKGROUND: Although long-term opioid therapy (LTOT) for chronic pain has declined, it remains common in the U.S. Providers do not have clinical practice guidelines for vulnerable LTOT patients, in whom both LTOT continuation and tapering to discontinuation pose risks of harm and in whom opioid use disorder (OUD) is absent. METHODS: To begin to meet the gap in guidelines, the study used a multiple case study approach. Five cases were constructed to systematically vary key elements of LTOT continuation and discontinuation harms among patients reporting LTOT's lack of efficacy (experience of pa

Using ambulance surveillance data to characterise blood-borne viral infection histories among patients presenting with acute alcohol and other drug-related harms.

OBJECTIVE: Preventable transmission of blood-borne viruses (BBV), including human immunodeficiency virus (HIV), hepatitis C virus (HCV) and hepatitis B virus (HBV), continue in at-risk populations, including people who use alcohol and drugs (AODs). To our knowledge, no studies have explored the use of ambulance data for surveillance of AOD harms in patients with BBV infections. METHODS: We used electronic patient care records from the National Ambulance Surveillance System for people who were attended by an ambulance in Victoria, Australia between July 2015 and July 2016 for AOD-related harms

Smoking may be more harmful to vasospastic angina patients who take antiplatelet agents due to the interaction: Results of Korean prospective multi-center cohort.

BACKGROUND: The interaction between smoking and the use of antiplatelet agents on the prognosis of vasospastic angina (VA) is rarely investigated. METHODS: VA-Korea is a nation-wide multi-center registry with prospective design (n = 1812). The primary endpoint was the composite occurrence of acute coronary syndrome (ACS), symptomatic arrhythmia, and cardiac death. Log-rank test and Cox proportional hazard model were for statistical analysis. Also, we conducted interaction analysis in both additive and multiplicative scales between smoking and antiplatelet agents among VA patients. For additiv

LVS-HARMED Risk Score for Incident Heart Failure in Patients With Atrial Fibrillation Who Present to the Emergency Department: Data from a World-Wide Registry.

Background Heart failure (HF) is a common complication to atrial fibrillation (AF), leading to rehospitalization and death. Early identification of patients with AF at risk for HF might improve outcomes. We aimed to derive a score to predict 1-year risk of new-onset HF after an emergency department (ED) visit with AF. Methods and Results The RE-LY AF (Randomized Evaluation of Long-Term Anticoagulant Therapy) registry enrolled patients with AF presenting to an ED in 47 countries, and followed them for a year. The end point was HF hospitalization and/or HF death. Among 15 400 ED patients, 9765 h

"Every Time I Go in There, It Gives Me Time to Reflect": A Qualitative Study of Patient Perspectives on Substance Use, Medications for Opioid Use Disorder, and Harm Reduction Following Hospitalization for Serious Injection-Related Infection.

BACKGROUND: Serious injection-related infections (SIRIs) have high morbidity and mortality, in part from incomplete antibiotic treatment, ongoing substance use and reinfection. Understanding how hospitalizations for SIRIs affect patient perspectives on substance use, harm reduction, and medications for opioid use disorder (MOUD) in the era of hospital-based addiction services will inform efforts to improve care. METHODS: We conducted qualitative interviews at Boston Medical Center with individuals hospitalized with SIRIs between 2020 and 2024. To ensure diverse experiences, we recruited quali

Aachen smoking cessation and harm reduction (ASCHR) trial study protocol - scientific evaluation of a psychological-telemedical counseling concept for smoking cessation in patients with vascular diseases.

BACKGROUND: Smoking is a major risk factor of cardiovascular diseases, notably peripheral arterial disease (PAD). Despite this link, research on smoking cessation interventions in PAD patients remains scarce and inconclusive regarding the efficacy of such interventions. Therefore, elucidating it is crucial and should address both individuals who smoke that are motivated to quit and individuals who smoke heavily lacking the motivation to quit. METHODS/DESIGN: The Aachen Smoking Cessation and Harm Reduction (ASCHR) trial is a prospective randomized controlled study (RCT) on the benefits of tele

Prescribing of Medication to Prevent Glucocorticoid Harms in Patients With Polymyalgia Rheumatica: A Cross-Sectional Study and Two Emulated Target Trials in the Clinical Practice Research Datalink Aurum.

OBJECTIVE: Polymyalgia rheumatica (PMR) is a common indication for long-term glucocorticoid (GC) treatment. Bone-protective and gastroprotective medications are recommended for those at high risk of adverse events from GCs, but no trials have evaluated their effectiveness in PMR. We describe bone-protective and gastroprotective medicine prescribing in people with PMR and evaluate its impact on adverse GC outcomes using a target trial approach. METHODS: A sample of >40,000 individuals aged ≥50 years with a coded PMR diagnosis from January 2010 to March 2022 who were prescribed GCs within 21 da

Curative intent therapy of stage I-III non-small cell lung cancer: a patient-centered precision approach to assess, measure, and interpret benefits and harms.

BACKGROUND: The number of people diagnosed with stage I-III non-small cell lung cancer (NSCLC) is increasing, in part due to greater implementation of lung cancer screening and earlier detection. Definitive surgery, radiation, or chemoradiation are increasingly utilized along with adjunctive therapies that include chemotherapy, radiation, immune checkpoint inhibitors (ICIs), and receptor tyrosine kinase inhibitors (rTKIs). However, remedial and adverse effects exist for each modality that must be accounted for in individual treatment plans with curative intent. The objective of this study was

Influence of study characteristics on harm estimates from randomised controlled trials in patients with inflammatory arthritis receiving biological or synthetic antirheumatic drugs: a meta-epidemiological study.

OBJECTIVE: To examine the association between study characteristics and the harms reported in randomised controlled trials (RCTs) on biological and targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs) in patients with inflammatory arthritis (IA). METHODS: We searched MEDLINE for all Cochrane reviews and for systematic reviews published since April 2015. RCTs were eligible if they included patients with IA receiving b/tsDMARD, compared with any comparator arm. Harms were evaluated based on number of withdrawals due to adverse events (WDdtAEs), total withdrawals (WDs), serious

Cash-Based Stem-Cell Clinics: The Modern Day Snake Oil Salesman? A Report of Two Cases of Patients Harmed by Intra-articular Stem Cell Injections.

CASE: The use of biologics is rapidly expanding. Over the past decade, there has been a significant increase in the number of cash-based "stem cell"/regenerative medicine clinics in the United States. These clinics provide cash-based services touting stem cell injections to cure a myriad of conditions. Largely, these clinics are unregulated and using injections in a non-Food and Drug Administration-approved manner. We report on 2 patients who presented with symptoms suggestive of septic arthritis following stem cell injections by cash-based local stem cell clinics. Case 1 involved a patient wh
