A population-based multistate model for diffuse large B-cell lymphoma-specific mortality in older patients.

BACKGROUND: Despite effective therapies, outcomes for diffuse large B-cell lymphoma (DLCBL) remain heterogeneous in older individuals due to comorbid diseases and variations in disease biology. METHODS: Using the Surveillance, Epidemiology, and End Results (SEER)-Medicare database, the authors conducted a multistate survival analysis of 11,780 patients with DLBCL who were aged ≥65 years at the time of diagnosis (2002-2009). Cox proportional hazards models were used to specify the impact of prognostic factors on overall survival and cause-specific deaths, and the Aalen-Johansen estimator was u

Metabolic Signature of MELAS/Leigh Overlap Syndrome in Patient-specific Induced Pluripotent Stem Cells Model.

BACKGROUND: Mitochondrial myopathy, Encephalopathy, Lactic Acidosis, Stroke-like episodes/Leigh overlap syndrome (MELAS) is caused by defects in the mitochondrial respiratory chain. It is still largely unknown how these mitochondrial respiratory chain defects affect cellular metabolisms and lead to variable clinical phenotypes. Here, we analyzed metabolic signatures in a cellular model of MELAS/ Leigh overlap syndrome using untargeted gas chromatography coupled to mass spectrometry (GC-MS). . METHODS: We obtained fibroblasts from a MELAS/Leigh overlap syndrome patient carrying the heteroplasm

Patient-specific estimates of vascular and placental properties in growth-restricted fetuses based on a model of the fetal circulation.

INTRODUCTION: Intrauterine growth restriction (IUGR) due to placental insufficiency is associated with blood-flow redistribution in order to maintain perfusion to the brain. However, some hemodynamic parameters that might be more directly related to staging of the disease cannot be measured non-invasively in clinical practice. For this, we developed a patient-specific model of the fetal circulation to estimate vascular properties of each individual. METHODS: A lumped model of the fetal circulation was developed and personalized using measured echographic data from 37 normal and IUGR fetuses t

Mild anastomotic stenosis in patient-specific CABG model may enhance graft patency: a new hypothesis.

It is well known that flow patterns at the anastomosis of coronary artery bypass graft (CABG) are complex and may affect the long-term patency. Various attempts at optimal designs of anastomosis have not improved long-term patency. Here, we hypothesize that mild anastomotic stenosis (area stenosis of about 40-60%) may be adaptive to enhance the hemodynamic conditions, which may contribute to slower progression of atherosclerosis. We further hypothesize that proximal/distal sites to the stenosis have converse changes that may be a risk factor for the diffuse expansion of atherosclerosis from th

SU-E-J-58: Patient-Specific Biomechanical Head and Neck Models for Interfraction Dose Accumulation.

PURPOSE: In this abstract, we discuss a biomechanical head and neck model that will be able to represent patient setup variations as well as physiologic changes and subsequently enable dose calculations on the deformed anatomy. METHODS: We selected Multi Pose MRI as the imaging modality to aid in model development and validation. The MRI data allowed us to build a biomechanically predictive model that will enable accurate estimation of tumor position when seeded with CT data alone. The soft tissue contrast and lack of ionizing radiation when using MRI enabled us to acquire extensive imaging d

Estimation of accuracy of patient-specific musculoskeletal modelling: case study on a post polio residual paralysis subject.

For patients with patterns ranging out of anthropometric standard values, patient-specific musculoskeletal modelling becomes crucial for clinical diagnosis and follow-up. However, patient-specific modelling using imaging techniques and motion capture systems is mainly subject to experimental errors. The aim of this study was to quantify these experimental errors when performing a patient-specific musculoskeletal model. CT scan data were used to personalise the geometrical model and its inertial properties for a post polio residual paralysis subject. After having performed a gait-based experime

Multi-level patient-specific modelling of the proximal femur. A promising tool to quantify the effect of osteoporosis treatment.

Preventing femoral fractures is an important goal in osteoporosis research. In order to evaluate a person's fracture risk and to quantify response to treatment, bone competence is best assessed by bone strength. Finite-element (FE) modelling based on medical imaging is considered a very promising technique for the assessment of in vivo femoral bone strength. Over the past decades, a number of different FE models have been presented focusing on the effect of several methodological aspects, such as mesh type, material properties and loading conditions, on the precision and accuracy of these mode

Predicting Hospital Length of Stay in Orthopedic Trauma Patients Using Fracture-Specific Machine Learning Models: A Multicenter Retrospective Prediction-Modeling Study.

BACKGROUND AND AIMS: Hospital length of stay (LOS) is a key indicator for resource allocation, bed management, and discharge planning in orthopedic trauma care. Given the substantial heterogeneity of LOS across fracture types and the limitations of conventional approaches, this study aimed to develop and validate fracture-specific machine learning models for predicting short vs. long hospital stay. METHODS: In this multicenter retrospective prediction-modeling study, routinely collected hospital information system data from three trauma centers were analyzed. From 326,323 diagnosis-level reco

Improvement of deep learning prediction model in patient-specific QA for VMAT with MLC leaf position map and patient's dose distribution.

PURPOSE: Deep learning-based virtual patient-specific quality assurance (QA) is a novel technique that enables patient QA without measurement. However, this method could be improved by further evaluating the optimal data to be used as input. Therefore, a deep learning-based model that uses multileaf collimator (MLC) information per control point and dose distribution in patient's CT as inputs was developed. METHODS: Overall, 96 volumetric-modulated arc therapy plans generated for prostate cancer treatment were used. We developed a model (Model 1) that can predict measurement-based gamma passi

MR-based Bony 3D models enable radiation-free preoperative patient-specific analysis and 3D printing for SCFE patients.

OBJECTIVES: Slipped capital femoral epiphyses (SCFE) is a common pediatric hip disease with the risk of osteoarthritis and impingement deformities, and 3D models could be useful for patient-specific analysis. Therefore, magnetic resonance imaging (MRI) bone segmentation and feasibility of 3D printing and of 3D ROM simulation using MRI-based 3D models were investigated. METHODS: A retrospective study involving 22 symptomatic patients (22 hips) with SCFE was performed. All patients underwent preoperative hip MR with pelvic coronal high-resolution images (T1 images). Slice thickness was 0.8-1.2

Human gastric multi-regional assembloids for functional parietal maturation and patient-specific modelling of antral foveolar hyperplasia.

Patient-derived human organoids have the capacity to self-organize into more complex structures. However, to what extent gastric organoids can recapitulate differentiated cell types and mucosal functions remains unexplored. Here we report on how region-specific gastric organoids can self-assemble into complex multi-regional assembloids. These assembloids show increased complexity and cross-communication between different gastric regions, allowing for the emergence of the elusive parietal cell type that is responsible for the production of gastric acid and shows a functional response to drugs t

Rebuilding spinal circuit function after spinal cord injury through a patient-specific interneuron precision model.

Spinal interneurons constitute the physiological core of spinal circuitry, integrating excitatory and inhibitory inputs to generate the rhythmic patterns that drive locomotor, postural, and autonomic control. Their developmental logic, molecular diversity, and adaptive plasticity make them central determinants of functional recovery after spinal cord injury (SCI). Yet most regenerative strategies continue to emphasize cellular replacement rather than the restoration of the physiological integrity of spinal networks. In this article, we reframe spinal repair as the restoration of interneuron-me

Prenatal opioid exposure and maternal HCV infection impair microglia development and function: A patient-specific in vitro model.

Opioids are a class of pain-relieving drugs known to cross the placental and blood brain barriers, exposing the fetus in utero. Rates of opioid use disorder amongst pregnant individuals in the United States are on the rise, and intravenous routes of opioid administration are highly associated with hepatitis C (HCV) infection. Newborns with prenatal opioid exposure (POE) are more likely to be small for gestational age and have increased rates of neurodevelopmental delay. Microglia are brain-resident macrophages that originate from yolk-sac precursors that play critical role in neurodevelopment.

Hip joint center lateralization minimally affects the biomechanics of patient-specific flanged acetabular components: A computational model.

Patient-specific flanged acetabular components are utilized to treat failed total hip arthroplasties with large acetabular defects. Previous clinical studies from our institution showed that these implants tend to lateralize the acetabular center of rotation. However, the clinical impact of lateralization on implant survivorship is debated. Our goal was to develop a finite element model to quantify how lateralization of the native hip center affects periprosthetic strain and implant-bone micromotion distributions in a static level gait loading condition. To build the model, we computationally

Novel models by machine learning to predict the risk of cardiac disease-specific death in young patients with breast cancer.

BACKGROUND: With the tremendous leap of various adjuvant therapies, breast cancer (BC)-related deaths have decreased significantly. Increasing attention was focused on the effect of cardiac disease on BC survivors, while limited existing population-based studies lay emphasis on the young age population. METHOD: Data of BC patients aged less than 50 years was collected from the SEER database. A competing risk model was introduced to analyze the effects of clinicopathology variables on the cardiac disease-specific death (CDSD) risks of these patients. Further, an XGBoost prediction model was co

Patient-specific analysis of bicuspid aortic valve hemodynamics using a fully coupled fluid-structure interaction model.

Bicuspid aortic valve (BAV), the most common congenital heart disease, is prone to develop significant valvular dysfunction and aortic wall abnormalities such as ascending aortic aneurysm. Growing evidence has suggested that abnormal BAV hemodynamics could contribute to disease progression. In order to investigate BAV hemodynamics, we performed 3D patient-specific fluid-structure interaction (FSI) simulations with fully coupled blood flow dynamics and valve motion throughout the cardiac cycle. Results showed that the hemodynamics during systole can be characterized by a systolic jet and two co

Deforming Patient-Specific Models of Vascular Anatomies to Represent Stent Implantation via Extended Position Based Dynamics.

PURPOSE: Angioplasty with stent placement is a widely used treatment strategy for patients with stenotic blood vessels. However, it is often challenging to predict the outcomes of this procedure for individual patients. Image-based computational fluid dynamics (CFD) is a powerful technique for making these predictions. To perform CFD analysis of a stented vessel, a virtual model of the vessel must first be created. This model is typically made by manipulating two-dimensional contours of the vessel in its pre-stent state to reflect its post-stent shape. However, improper contour-editing can cau
