Mastering the art of epicardial access in cardiac electrophysiology.

Access to the epicardial space is fundamental to several cardiac procedures. While traditional indications include catheter ablation of ventricular arrhythmias and accessory pathways, novel indications include left atrial appendage occlusion, esophageal protection, mapping and ablation during atrial fibrillation procedures, implantation of epicardial pacing leads, and phrenic nerve displacement to facilitate safe ablation of atrial and ventricular arrhythmias. Accessing the epicardial space safely is a major challenge requiring intimate knowledge of cardiac anatomy, extensive training, and exp

Positive contrast ventriculography in cats with experimental obstructive hydrocephalus.

Cerebrospinal fluid pathways were studied in both normal and experimental obstructed hydrocephalic cats by positive contrast ventriculography. Either water soluble or insoluble contrast material was injected into the lateral cerebral ventricles, and radiographs were taken of the head and spinal cord. In the normal cat, the contrast material freely flowed throughout the spinal fluid spaces. The contrast material accumulated in the cisterna magna, and from there extended into the cranial and spinal subarachnoid spaces. In the kaolin-induced hydrocephalic cat, the outlets from the fourth ventricl

cAMP increases K+ secretion via activation of apical IsK/KvLQT1 channels in strial marginal cells.

In the cochlea, K+ is secreted by electrodiffusion across the apical membrane of strial marginal cells via the IsK/KvLQT1 ('IsK') channel. This channel complex has been reported to be activated in other systems by adenosine 3',5'-cyclic monophosphate (cAMP). Since several reports had suggested that cAMP is a second messenger in the cochlea, the effect of the cAMP pathway on transepithelial K+ secretion by strial marginal cells of the gerbil was studied. Both the transepithelial current (Isc) and K+ flux (JK) across strial marginal cell epithelium were measured; Isc in a micro-Ussing chamber an

Neuritin promotes autophagic flux by inhibiting the cGAS-STING pathway to alleviate brain injury after subarachnoid haemorrhage.

BACKGROUND: Early brain injury (EBI) is closely associated with poor prognosis in patients with subarachnoid haemorrhage (SAH), with autophagy playing a pivotal role in EBI. However, research has shown that the stimulator of interferon genes (STING) pathway impacts autophagic flux. While the regulatory impact of neuritin on EBI and autophagic flux has been established previously, the underlying mechanism remains unclear. This study aimed to determine the role of the cGAS-STING pathway in neuritin-mediated regulation of autophagic flux following SAH. METHODS: A SAH model was established in mal

Cerebellin-neurexin complexes instructing synapse properties.

Cerebellins (Cbln1-4) are secreted adaptor proteins that connect presynaptic neurexins (Nrxn1-3) to postsynaptic ligands (GluD1/2 for Cbln1-3 vs. DCC and Neogenin-1 for Cbln4). Classical studies demonstrated that neurexin-Cbln1-GluD2 complexes organize cerebellar parallel-fiber synapses, but the role of cerebellins outside of the cerebellum has only recently been clarified. In synapses of the hippocampal subiculum and prefrontal cortex, Nrxn1-Cbln2-GluD1 complexes strikingly upregulate postsynaptic NMDA-receptors, whereas Nrxn3-Cbln2-GluD1 complexes conversely downregulate postsynaptic AMPA-re

Pyroptosis in urinary malignancies: a literature review.

Urinary neoplasms refer to malignant tumours occurring in any part of the urinary system, including the kidney, renal pelvis, ureter, bladder, prostate, etc. The worldwide incidence of urinary system tumours has been increasing yearly. Available methods include surgical treatment, radiotherapy, chemotherapy, endocrine therapy, molecular targeted therapy, and immune therapy. In recent years, emerging evidence has demonstrated that cell pyroptosis plays an important role in the occurrence and progression of malignant urinary tumours. Pyroptosis is a new type of cell death that involves inflammat

The anatomy of the limbus.

The limbus forms the border between the transparent cornea and opaque sclera, contains the pathways of aqueous humour outflow, and is the site of surgical incisions for cataract and glaucoma. Externally the epithelial cell border between conjunctiva and cornea possesses multipotential cells important for differentiation of the respective cell types. By the same token, the internal limbal border zone between corneal endothelium and anterior trabeculum appears to contain specialised cells some of which are activated to migrate and repopulate the trabecular meshwork after trabecular injury. The o

Intramural Outflow Tract Ventricular Arrhythmia With Preferential Conduction Mimicking a Left Ventricular Summit Origin.

BACKGROUND: Intramural ventricular arrhythmias with preferential conduction are challenging to ablate because the breakout site may be remote from the true site of origin. CASE SUMMARY: A 61-year-old man presented with drug-refractory premature ventricular contractions after failed catheter ablation. Electrocardiography suggested an epicardial outflow tract origin. Endocardial mapping identified a prepotential at the left-right coronary cusp junction with a perfect pace map but an extremely long stimulus-to-QRS latency, indicating activation via a preferential conduction pathway. Lactylation of MYH9 and its impact on FOXO3a/Bim signaling in sepsis-induced gut-vascular barrier injury.

The gut-vascular barrier (GVB) is the ultimate defense of the intestine, meticulously regulating the exchange of substances between the intestinal lumen and the circulatory system. Elevated expression of myosin heavy chain 9 (MYH9) has been implicated in blood-brain barrier dysfunction following cerebral ischemic/reperfusion injury and in lung endothelial barrier disruption during acute lung injury. However, the role of MYH9 in GVB dysfunction during sepsis remains unclear. We hypothesized that MYH9 contributes to GVB injury in sepsis. To test this hypothesis, we established a mouse model of s

Dihydromyricetin protects hippocampal neurons in sepsis-associated encephalopathy by reducing ER stress-mediated apoptosis and inflammation.

Sepsis-associated encephalopathy (SAE) involves hippocampal dysfunction and cognitive deficits, but its underlying mechanisms remain unclear, and effective treatments are lacking. Dihydromyricetin (DHM), a key flavonoid in Japanese raisin trees and Chinese Rattan tea, exhibits broad therapeutic potential for neurological disorders. This study investigated whether DHM improves hippocampal dysfunction in SAE rats by targeting endoplasmic reticulum (ER) stress-mediated apoptosis and inflammation. The SAE model was established by cecal ligation perforation (CLP) surgery in rats. We evaluated survi

Identification of altered miRNAs and their targets in placenta accreta.

Placenta accreta spectrum (PAS) is one of the major causes of maternal morbidity and mortality worldwide with increasing incidence. PAS refers to a group of pathological conditions ranging from the abnormal attachment of the placenta to the uterus wall to its perforation and, in extreme cases, invasion into surrounding organs. Among them, placenta accreta is characterized by a direct adhesion of the villi to the myometrium without invasion and remains the most common diagnosis of PAS. Here, we identify the potential regulatory miRNA and target networks contributing to placenta accreta developm

FTY720 Attenuates Cerebral Vasospasm After Subarachnoid Hemorrhage Through the PI3K/AKT/eNOS and NF-κB Pathways in Rats.

Cerebral vasospasm (CVS) is a major complication of subarachnoid hemorrhage (SAH). Inflammation and nitric oxide (NO) have become increasingly recognized as key pathogenic contributors to brain injury in this condition. We aimed to examine the role of FTY720 in CVS after SAH. Endovascular perforation was used to establish an SAH model. Seventy-five male Sprague-Dawley rats were randomly divided into five groups: sham, sham + FTY720, SAH + saline, and two SAH + FTY720 (0.5 and 1 mg/kg) groups. The results showed that FTY720 treatment in both the surgery and nonsurgery groups decreased the count

Multi-platform evidence of GSDMD-dependent pyroptosis in malaria with a potential role for TLR7.

BACKGROUND: Macrophage-mediated inflammation critically determines malaria pathology. Although pyroptosis contributes to host defense, excessive activation induces severe immunopathology. The precise pyroptotic pathways in splenic macrophages during Plasmodium infection, and the upstream regulatory mechanisms that govern them, remain unclear. METHODS: We integrated clinical transcriptomic analysis (GSE132050), Plasmodium yoelii infected mouse models, and in vitro macrophage assays to systematically investigate pyroptosis mechanisms and to evaluate the regulatory role of the Toll-like receptor

Post-Translational MYC Stabilization in Association with IGF-1 Signaling During Corneal Epithelial Repair.

Corneal ulcers are common in both humans and companion animals and involve epithelial or stromal defects that may progress to sight-threatening scarring or globe perforation. Both insulin-like growth factor-1 (IGF-1) signaling pathways and MYC-mediated cellular programs promote corneal epithelial proliferation and migration imperative for uncomplicated wound resolution; however, the role of IGF-1 in the regulation of MYC during corneal epithelial repair has not been characterized. The objective of this study was to examine IGF-1-mediated MYC expression in the corneal epithelium using telomeras

Erbin protects against sepsis-associated encephalopathy by attenuating microglia pyroptosis via IRE1α/Xbp1s-Ca2+ axis.

BACKGROUND: Microglia pyroptosis-mediated neuroinflammation is thought to be the crucial pathogenesis of sepsis-associated encephalopathy (SAE). Erbin has been reported to be associated with various inflammatory diseases. However, the role of Erbin in SAE and the relationship between Erbin and microglia pyroptosis are unknown. In this study, we investigated the promising role and underlying molecular mechanism of Erbin in the regulation of microglia pyroptosis. METHODS: WT and Erbin knockout mice underwent cecum ligation perforation (CLP) to induce SAE. Primary mouse microglia and BV2 cells w
