[Our experience with the dispensary observation and treatment of kidney transplant patients].

The authors report the results from the follow-up of 14 patients with transplanted kidney, three out of them with a lethal end--mycotic sepsis--1, purulent peritonitis--1 and of transplant lung-syndrome--1. The rest (11-78,5%) were in a good condition during 1 year and 8 months to 6,5 years (an average of 4 years and 10 days by April 30, 1980). Eleven of the patients had their transplantations performed by Prof. Sumakov--Prof. Levizkii in Moscow and three--by Prof. Hamburger, Prof. Crosnier, Prof. Lacomb in Necker Hospital, Paris. During the follow-up period those 14 patients had the following

Ultrasonic investigation of the kidneys.

Grey scale ultrasound provides a valuable non-invasive method of investigating the kidneys. It is usually a complementary examination to intravenous urography. When the ultrasonic findings are considered along with the clinical history, the results of the urogram and the results of any other relevant investigations then an accurate diagnosis can often be made. Ultrasound is of particular value in the investigation of renal masses, the non-functioning kidney, perinephric fluid collections, patients in renal failure and in the post-operative assessment of renal transplant patients.

[Emphysematous pyelonephritis].

Emphysematous pyelonephritis (EPN) is a rare, life-threatening infection of the kidney, characterized by production of gas within the renal parenchyma or perinephric tissue. Escherichia coli and Klebsiella are the most commonly isolated organisms. Most patients are diabetics. CT is diagnostic. Within half a year two patients with EPN were admitted. They were successfully treated with internal/percutaneous drainage (PCD), and antibiotics. Earlier, acute nephrectomy was first choice treatment, but recent studies point towards PCD and antibiotics as a better alternative.

Large posterior abdominal masses: computed tomographic localization.

Large posterior abdominal masses, particularly those in the right upper abdomen, may be difficult to localize correctly into the peritoneal or retroperitoneal compartments. The following signs were found to be reliable CT indicators of retroperitoneal location: obliteration of the perinephric fat outlining the psoas muscle; lateral displacement of the fat outlining the posterior right lobe of the liver; rotation of the intrahepatic portal veins to the left; anterior displacement of the inferior vena cava and renal veins; and anterior displacement of the ascending colon, descending duodenum, or

Calyceal Rupture Secondary to Nephrolithiasis: A Case Report Emphasizing Early Diagnosis and Management.

Calyceal rupture, defined as the extravasation of urine from the renal calyces into the perinephric or paranephric spaces, typically results from increased intrapelvic pressure due to urinary tract obstruction. This condition can lead to the formation of a perinephric urinoma and severe complications, such as infection, abscess formation, and impaired renal function. Timely diagnosis and management are crucial to prevent these adverse outcomes. Calyceal rupture often results from urolithiasis, with other causes including strictures, tumors, and congenital abnormalities. The rupture occurs when

It is easy and effective to locate adrenal gland during retroperitoneal laparoscopic left adrenalectomy by the landmark of left PFSV.

To evaluate the feasibility and clinical significance of the left perinephric fat sac vein (PFSV) as an anatomical landmark in locating left adrenal gland (LAD) during retroperitoneal laparoscopic left adrenalectomy (RLLA). In this study, a total of 36 patients who underwent RLLA were enrolled from February 2019 and March 2021. By following a vein vessel on the internal surface of perinephric fat sac (PFS), known as PFSV, LAD was searched finally along the upper edge of this vein. The demographic and clinical characteristics of these patients were acquired, including tumor features and periope

Imaging of pediatric renal transplants and their complications: a pictorial review.

Renal transplantation is the treatment of choice for end-stage renal disease in children. As a technically demanding surgery with complex medical management, it is associated with a number of complications. Anatomic imaging including ultrasonography with color and spectral Doppler and functional assessment with renal perfusion scintigraphy are complementary for the detection and characterization of posttransplant complications. Complications can be characterized by the time of appearance after transplantation (immediate, early, or late) or the anatomic site of origin (perinephric, vascular, ur

MDCT-based Grading of Perirenal Changes Secondary to Acute Unilateral Upper Urinary Tract Obstruction.

BACKGROUND: Unilateral upper ureteral obstruction is one of the most common causes of acute kidney function impairment. Grading perirenal changes secondary to acute unilateral upper urinary tract obstruction (AUUTO) with multidetector spiral computed tomography (MDCT) and exploring its association with kidney function are useful for diagnosing and assessing damage to the ipsilateral kidney. However, the correlation between renal function impairment and the severity of perinephric changes secondary to AUUTO has not been reported. OBJECTIVE: This study aimed to investigate the association of pe

Standard operating protocol for acute pyelonephritis.

Acute pyelonephritis ranges from uncomplicated infection to life-threatening emphysematous pyelonephritis, a fulminant necrotizing infection of the renal parenchyma and perinephric tissues. Most cases are caused by Gram-negative bacilli, predominantly Escherichia coli, with rising drug resistance. A structured, stepwise approach is essential for early recognition, risk stratification, and timely imaging-based intervention, particularly in acute care settings. Judicious empirical antimicrobial therapy, guided by illness severity, host factors, and local resistance patterns, is critical.

Unilateral page kidney in end-stage renal disease patient presenting as recurrent flash pulmonary edema: A rare association.

Page kidney refers to the extrinsic compression of renal parenchyma causing renovascular hypertension. We present the case of a 58-year-old female with refractory hypertension and recurrent episodes of flash pulmonary edema following trivial trauma. Despite initial management with percutaneous drainage of the perinephric hematoma, she eventually required a nephrectomy due to persistent symptoms. The symptoms resolved post nephrectomy. The case highlights the importance of early diagnosis and timely intervention in Page kidney to prevent long-term complications.

Unsuspected urinoma of an obstructed kidney detected in a bone scan in a patient with lung cancer.

A 60-year-old woman with an adenocarcinoma of the lung was referred to our department for a bone scan to assess the extension of the metastatic bone disease. The images showed several foci of radiotracer uptake in the bone consistent with malignant involvement. Besides, a considerable accumulation of tracer was shown in the right perinephric region as well as in the renal pelvis. SPECT/CT images demonstrated a superior perirenal urine leak due to rupture of the collecting system. The leakage was treated by urinary catheter decompression with complete recovery after 4 weeks.

Emphysematous pyelonephritis in a non-diabetic patient with non-obstructed kidney: an unknown entity.

Emphysematous pyelonephritis (EPN) is an acute necrotizing infection with evidence of gas inside the kidney, perinephric space, and/or urinary collecting system. This condition is usually encountered in an obstructed urinary system of diabetics or immunocompromised patients and carries poor prognosis. The gold standard for early diagnosis is computed tomography (CT) of the abdomen. Percutaneous/surgical drainage and urgent or delayed nephrectomy are the available treatment options. We report one case of EPN, which was diagnosed in an immunocompetent non-diabetic man with a non-obstructed urina

Combination of Surgical Drainage and Renal Artery Embolization: An Alternative Treatment for Xanthogranulomatous Pyelonephritis.

Conventionally, xanthogranulomatous pyelonephritis is treated with antibiotics and drainage of abscess followed by nephrectomy for definitive treatment. Surgical excision of the affected kidney carries risk of significant complications. An alternative treatment modality is described in the form of embolization of the renal artery to devascularise the renal parenchyma and ablate the renal tissue, thus avoiding a major surgical procedure and the significant risks involved.

Page Kidney Resulting From Traumatic Subcapsular Renal Hematoma: A Case Report.

Page kidney is an uncommon but clinically important cause of secondary hypertension resulting from external compression of the renal parenchyma, most often due to a subcapsular or perinephric hematoma, leading to reduced renal perfusion and activation of the renin-angiotensin-aldosterone system. We report a case of a 45-year-old Middle Eastern male who presented with acute left flank pain and newly diagnosed severe hypertension following minor blunt trauma. Initial evaluation revealed localized flank tenderness and elevated blood pressure in the absence of hemodynamic instability or significan

[Conservative Treatment of Renal Trauma with Arterial Laceration : A Successful Case Report].

An octogenarian male presented to the emergency department following a fall. On admission, his systolic blood pressure was in the 70s mmHg range, and his pulse was around 60 bpm. Initial resuscitation efforts included fluid administration with normal saline and vasopressor support with noradrenaline. A contrast-enhanced abdominal computed tomographic (CT) scan revealed multiple simple intraparenchymal lacerations in the right kidney, accompanied by perinephric hematoma, and peripheral extravasation of contrast at the right renal artery. There was also partial luminal narrowing of the main righ
