PURPOSE OF THE STUDY: A retrospective study of our experience with one-stage left colectomy for acute diverticulitis and obstruction with a review of the literature to more clearly define the indications of this procedure.
PATIENTS AND METHODS: 30 patients were operated for acute diverticulitis (group 1) and 47 for obstruction (group 2). Only 7 patients (23%) of group 1 had an intraoperative colonic lavage while this was performed for all the patients of group 2.
RESULTS: The postoperative morbidity and mortality for the patients of group 1 and 2 were 37 and 28%, and 7 and 11% respectively.

An 82-year-old man was diagnosed with pancreatic body cancer after worsening of his diabetes mellitus. Computed tomography and staging laparoscopy revealed an unresectable pancreatic cancer with peritoneal metastasis, and systemic chemotherapy was initiated. After 76 months of chemotherapy, including up to third-line treatment, continuation of chemotherapy was difficult because of adverse events. Small bowel perforation is a relative infrequent complication of blunt abdominal trauma. We reviewed the literature regarding this clinical entity and describe our experience in 22 patients. There were 17 male and 5 female patients; mean age was 34 years. Cause of the trauma was road traffic accident (18 cases) or fall (4 patients). We report a 2-year-old girl with spontaneous perforation of choledochal cyst. Preoperative diagnosis was possible by hepatobiliary scintigraphy. In view of emergency presentation and bile peritonitis, management was a staged procedure with peritoneal lavage and T-tube drainage of the biliary system, followed by excision of the cyst and Roux-en-Y hepatico-jejunostomy 3 months later.
