The clinical outcomes of imaging modalities for surgical management Cushing's disease - A systematic review and meta-analysis.

INTRODUCTION: Cushing's disease presents major diagnostic and management challenges. Although numerous preoperative and intraoperative imaging modalities have been deployed, it is unclear whether these investigations have improved surgical outcomes. Our objective was to investigate whether advances in imaging improved outcomes for Cushing's disease. METHODS: Searches of PubMed and EMBASE were conducted. Studies reporting on imaging modalities and clinical outcomes after surgical management of Cushing's disease were included. Multilevel multivariable meta-regressions identified predictors of o

Long-term efficacy and safety of osilodrostat in patients with Cushing's disease: results from the LINC 4 study extension.

OBJECTIVE: To evaluate the long-term efficacy and safety of osilodrostat in patients with Cushing's disease. METHODS: The multicenter, 48-week, Phase III LINC 4 clinical trial had an optional extension period that was initially intended to continue to week 96. Patients could continue in the extension until a managed-access program or alternative treatment became available locally, or until a protocol amendment was approved at their site that specified that patients should come for an end-of-treatment visit within 4 weeks or by week 96, whichever occurred first. Study outcomes assessed in the

Treatment of Cushing Disease With Pituitary-Targeting Seliciclib.

CONTEXT: Preclinical studies show seliciclib (R-roscovitine) suppresses neoplastic corticotroph proliferation and pituitary adrenocorticotrophic hormone (ACTH) production. OBJECTIVE: To evaluate seliciclib as an effective pituitary-targeting treatment for patients with Cushing disease (CD). METHODS: Two prospective, open-label, phase 2 trials, conducted at a tertiary referral pituitary center, included adult patients with de novo, persistent, or recurrent CD who received oral seliciclib 400 mg twice daily for 4 consecutive days each week for 4 weeks. The primary endpoint in the proof-of-conc

[Cushing's disease in pregnancy and treatment with cabergoline: obstetric and neonatal results].

INTRODUCTION:: Cushing's disease (CD) is rare during pregnancy and is associated with significant maternal and fetal complications. The aim was to report the case of a patient with CD who achieved pregnancy and delivery without complications after treatment with low doses of cabergoline. METHODS:: A 29-year-old woman diagnosed with CD (ACTH-secreting macro-tumor that produces displacement of the optic chiasm, infiltrates the right cavernous sinus and includes the internal carotid artery). She undergoes transsphenoidal surgery with incomplete tumor resection. After a year of clinical stability

Diurnal Range and Intra-patient Variability of ACTH Is Restored With Remission in Cushing's Disease.

CONTEXT: Single ACTH measurements have limited ability to distinguish patients with Cushing's disease (CD) from those in remission or with other conditions. OBJECTIVE: To investigate the changes in ACTH levels before and after transsphenoidal surgery (TSS) to identify trends that could confirm remission from CD and help establish ACTH cutoffs for targeted clinical trials in CD. DESIGN: Retrospective analysis of CD patients who underwent TSS from 2005 to -2019. SETTING: Referral center. PATIENTS: CD patients (n = 253) with ACTH measurements before and after TSS. INTERVENTIONS: TSS for CD.

Altered hippocampal volume and functional connectivity in patients with Cushing's disease.

INTRODUCTION: Stress-related brain disorders can be associated with glucocorticoid disturbance and hippocampal alteration. However, it remains largely unknown how cortisol affects the structure and function of hippocampus. Cushing's disease (CD) provides a unique "hyperexpression model" to explore the effects of excessive cortisol on hippocampus as well as the relation between these effects and neuropsychological deficits. METHODS: We acquired high-resolution T1-weighted and resting-state functional magnetic resonance imaging in 47 CD patients and 53 healthy controls. We obtained the volume a

The hypothalamic-pituitary-gonad axis in male Cushing's disease before and after curative surgery.

OBJECTIVE: Gonadal and sexual disturbances are commonly encountered in patients with Cushing's disease. Nevertheless, the prevalence of hypogonadism in male Cushing's disease, the risk factors as well as the recovery time have been scarcely reported. Therefore, we aimed to explore the prevalence of hypogonadism at baseline and its determinants. In addition, the recovery time of hypogonadism and risk factors for unrecovered gonadal axis in male Cushing's disease with biochemical remission were investigated. METHODS: We reviewed medical records of males with Cushing's disease managed between 20

Multiple enlarging hepatic and retroperitoneal myelolipomas in the setting of Cushing disease.

Myelolipomas are benign tumours typically occurring in the adrenal glands, made up of fat and trilineage haematopoeitic cells resembling bone marrow. Their aetiology is not well understood; however, they have a clear association with elevated serum adrenocorticotropic hormone (ACTH). Extra-adrenal myelolipomas are rare, and to our knowledge there are no previously reported cases of multiple enlarging hepatic and retroperitoneal myelolipomas in the setting of Cushing disease. We present the case of a patient with an ACTH-producing pituitary adenoma who developed multiple enlarging fat containin

Patient-Centered Outcomes with Pituitary and Parasellar Disease.

Over the last 2 decades, advances in the diagnosis and management of pituitary diseases have made it possible to attain an endocrine "cure" in a large proportion of patients. In other words, tumors can be excised or controlled with drugs, mass effects of the lesion on surrounding structures can be solved, and pituitary deficiencies can be substituted with all relevant hormones. While this is considered a satisfactory outcome for health care providers, patients often suffer from an aftermath of prior endocrine dysfunction exposure, with irreversible effects, both physically and psychologically,

A Case of Cushing Disease Masked by Symptoms of Bipolar Disorder.

BACKGROUND: Cushing disease is a rare but serious endocrine disorder. It involves increased cortisol levels, which can damage the function of various systems throughout the body, including the central nervous system, affecting cognition, memory, and emotion. However, it is rare that Cushing disease presents as a mental disorder. This paper reports the complete diagnosis, treatment process, and follow-up of a patient with Cushing disease whose main symptom was bipolar disorder. CASE PRESENTATION: A 31-year-old woman was diagnosed with bipolar disorder 9 years ago and was admitted to hospital s

Pasireotide - Mechanism of Action and Clinical Applications.

BACKGROUND: Pasireotide (SOM230) is a multi-receptor ligand somatostatin analogue (SSA) developed as the successor of the first-generation SSAs. Currently, pasireotide is recommended for the treatment of patients with Cushing's disease in whom surgery was unsuccessful, and patients with acromegaly who either remain uncontrolled after surgical therapy or in whom tumor resection is not possible. METHODS AND RESULTS: Phase II and III clinical trials have shown pasireotide efficacy in these diseases, with a similar rate of adverse events when compared with first-line SSA, although higher incidenc

Management of endocrine disease: Mortality remains increased in Cushing's disease despite biochemical remission: a systematic review and meta-analysis.

The aim of this systematic review and meta-analysis was to investigate whether mortality is increased in patients biochemically cured after initial treatment for Cushing's disease. This is a systematic review and meta-analysis of follow-up studies in patients cured from Cushing's disease after initial treatment was performed. Eight electronic databases were searched from 1975 to March 2014 to identify potentially relevant articles. Original articles reporting the standardized mortality ratio (SMR) for patients cured of Cushing's disease were eligible for inclusion. SMRs were pooled in a random

Cushing's disease presenting as cholestatic hepatitis.

Here we report an adolescent male with Cushing's disease secondary to a pituitary adenoma who was admitted with cholestatic hepatitis. A detailed work-up for hepatitis was negative except for a novel heterozygous ABCB4 gene mutation encoding multidrug resistance type III (MDR3) protein. Upon resection of the pituitary adenoma, the hepatic biochemical abnormalities gradually improved. This case highlights that heterozygous ABCB4 defect may represent a genetic predisposition for nongenetic factors such as hormones or other metabolites to decrease normal MDR3 allele expression. It is known to act

Current approaches to the pharmacological management of Cushing's disease.

If treatment of Cushing's disease (CD) by surgery is not successful, medical therapy is often required. Long-term use of metyrapone is limited by hirsutism and hypertension and escape because of increased ACTH levels. Although ketoconazole can normalize cortisol levels in 50%, liver toxicity limits its use. Mitotane, an adrenolytic agent, has had minimal use for benign disease. Etomidate is useful when rapid reduction in cortisol levels is needed. Cabergoline can normalize cortisol levels in CD in about one-third of patients and is well tolerated. Pasireotide can normalize cortisol levels in C

Mitotane reduces human and mouse ACTH-secreting pituitary cell viability and function.

Medical therapy for Cushing's disease (CD) is currently based on agents mainly targeting adrenocortical function. Lately, pituitary-directed drugs have been developed, with limited efficacy. Mitotane, a potent adrenolytic drug, has been recently investigated for the treatment of CD, but the direct pituitary effects have not been clarified so far. The aim of our study was to investigate whether mitotane may affect corticotroph function and cell survival in the mouse pituitary cell line AtT20/D16v-F2 and in the primary cultures of human ACTH-secreting pituitary adenomas, as an in vitro model of

Glucose metabolism in anterior pituitary adenomas.

Glucose metabolism alterations are frequently observed in patients with secretory pituitary adenomas. The most commonly secreted hormones in these tumors include prolactin, growth hormone (GH), adrenocorticotropic hormone (ACTH), and thyroid-stimulating hormone (TSH), all of which can disrupt glucose homeostasis through distinct pathophysiological mechanisms. Prolactin stimulates pancreatic β-cell proliferation, enhances insulin gene transcription, increases intracellular insulin content, and augments glucose-induced insulin secretion. GH promotes lipolysis and hepatic glucose production while

Pregnancy in a case of Nelson's syndrome.

A woman suffering from Cushing's disease from the age of 17 who had been treated consecutively with pituitary irradiation, bilateral partial adrenalectomy and o,p'-DDD (Mitotane, USP) presented the clinical picture of Nelson's syndrome (hypoadrenalism with secondary hypersecretion of ACTH and MSH) at the age of 32. Under substitution therapy with corticoids she became pregnant for the first time at the age of 38. The course of the pregnancy was normal and at term she was delivered of a normal child by Cesarean section. The materno-fetal relationship, the increased risk of pituitary infarction
