Placebo and nocebo in the treatment of migraine: How much does real world effectiveness depend on contextual effects?

PURPOSE: Treatments in medicine impact individuals beyond their intended effects, due to phenomena such as the placebo and nocebo effects. The placebo effect arises from the positive expectation of a treatment being beneficial, while the nocebo effect stems from the negative expectation of a treatment causing harm. Both in real-world practice and clinical trials, treatments can lead to outcomes unrelated to their intended mechanism of action, which we categorize as placebo and nocebo responses. These responses, combined with the inherent fluctuation in a condition's natural progression, regres

[Effectiveness of and tolerance to intracavernous injection of moxisylyte in patients with erectile dysfunction: effect/dose relationship versus placebo].

The authors investigated the optima dose (efficacy and safety) of moxisylyte, an alpha-blocking agent, in a double-blind placebo-controlled crossover study in 30 patients. The origin of the erectile dysfunction was predominantly psychological in 14 patients and neurological in 16 patients. Each patient received 4 intracavernous injections in a randomized order (placebo, 10, 20, 30 mg of moxisylyte) at 7-day intervals. Regardless of the dose, moxisylyte induced significantly greater penile responses than placebo on all erection criteria. The frequency of responses allowing sexual intercourse ap

Effect of Gabapentin Administered With Prednisolone, Ciclosporin or a Placebo on Clinical Outcomes and Motor Activity in Cats With Atopic Skin Syndrome: A Prospective, Blinded, Placebo-Controlled Study.

BACKGROUND: Gabapentin reportedly decreases central sensitisation, a disorder associated with chronic pruritus in humans, although this is not well documented in cats. Its combined use with the standard antipruritic therapy for feline atopic skin syndrome (FASS) is not yet described. OBJECTIVES: To evaluate the impact of prednisolone, ciclosporin or placebo, with or without gabapentin, on lesional scores and actimetry in FASS cats. ANIMALS: Twenty-six cats from a laboratory colony with naturally acquired FASS. METHODS AND MATERIALS: Following a 12-week washout period, cats were allocated to

Placebo Effects in Modern Medicine: Mechanisms, Clinical Evidence, Limitations, and Future Directions.

This narrative review traces the historical background of placebo use in medicine, from its early role as a consolatory intervention prior to evidence-based practice to its scientific reframing with the emergence of randomized controlled trials and the pivotal contributions of mid-20th-century research. The article examines the mechanisms underlying placebo effects, focusing on neurobiological processes involving pain-modulating brain networks, endogenous opioid and dopaminergic pathways, and emerging evidence for hormonal and immune system involvement, alongside psychological mechanisms such

Placebo Effects Are Small on Average in the 7.5% CO2 Inhalational Model of Generalized Anxiety.

BACKGROUND: Anxiety disorders are highly prevalent and socio-economically costly. Novel pharmacological treatments for these disorders are needed because many patients do not respond to current agents or experience unwanted side effects. However, a barrier to treatment development is the variable and large placebo response rate seen in trials of novel anxiolytics. Despite this, the mechanisms that drive placebo responses in anxiety disorders have been little investigated, possibly due to low availability of convenient experimental paradigms. We aimed to develop and test a novel protocol for in

Treatment effects in pharmacological clinical randomized controlled trials are mainly due to placebo.

Источник пометил публикацию как отозванную

OBJECTIVES: The placebo response in clinical trials has four components: regression to the mean (RTM), measurement artifacts, natural tendency (NT) of the disease, and the genuine placebo effect. Our objective is to determine what contributes to the size of the placebo-effect in clinical trials by meta-regressions of randomized placebo-controlled clinical trials. STUDY DESIGN AND SETTING: We identified five diseases where data on the rates of NT were available to search for a sample of n = 150 (5x30) randomized controlled trials. We extracted various study descriptors and performed meta-regre

The Roles of Endogenous Opioids in Placebo and Nocebo Effects: From Pain to Performance to Prozac.

Placebo and nocebo effects have been well documented for nearly two centuries. However, research has only relatively recently begun to explicate the neurobiological underpinnings of these phenomena. Similarly, research on the broader social implications of placebo/nocebo effects, especially within healthcare delivery settings, is in a nascent stage. Biological and psychosocial outcomes of placebo/nocebo effects are of equal relevance. A common pathway for such outcomes is the endogenous opioid system. This chapter describes the history of placebo/nocebo in medicine; delineates the current stat

Biological Effects of Hydrogen Water on Subjects with NAFLD: A Randomized, Placebo-Controlled Trial.

Non-alcoholic fatty liver disease (NAFLD) is a liver pathology affecting around 25% of the population worldwide. Excess oxidative stress, inflammation and aberrant cellular signaling can lead to this hepatic dysfunction and eventual carcinoma. Molecular hydrogen has been recognized for its selective antioxidant properties and ability to attenuate inflammation and regulate cellular function. We administered hydrogen-rich water (HRW) to 30 subjects with NAFLD in a randomized, double-blinded, placebo-controlled manner for eight weeks. Phenotypically, we observed beneficial trends (p > 0.05) in de

Effects of Tyrosine on Parkinson's Disease: A Randomized, Double-Blind, Placebo-Controlled Trial.

Individuals with Parkinson's disease (PD) can suffer from orthostatic hypotension (OH) resulting from reduced levels of norepinephrine (NE), which inhibits the sympathetic nervous system. Levodopa reduces NE levels even further, leading to a greater decrease in blood pressure (BP) and increased OH. Tyrosine is a nonessential amino acid that is the major precursor to NE. Reduced levels of tyrosine have been shown after administration of l-dopa. This study was a single-center, randomized, double-blind, placebo-controlled trial to test the effects of supplementing l-tyrosine on BP, plasma tyrosin

Placebo effects on itch: a meta-analysis of clinical trials of patients with dermatological conditions.

Although placebo contributes to the effects of treatment for various symptoms and conditions, its effect on itch has rarely been investigated. In this meta-analysis, the magnitude of the placebo effect on itch was systematically investigated in clinical trials including patients with chronic itch due to atopic dermatitis, psoriasis, or chronic idiopathic urticaria. From searches in four databases, 34 articles were included in the quantitative analyses. Placebo treatment significantly decreased itch (1.3 out of 10, 95% confidence interval 1.02-1.61) compared with baseline itch (effect size 0.55

The effects of single-dose fexofenadine, diphenhydramine, and placebo on cognitive performance in flight personnel.

BACKGROUND: Sedation and functional impairments are side effects associated with the use of first-generation antihistamines that preclude their use in aviation. Selected second-generation antihistamines do not have such side effects and have been proposed for use in aircrew. METHODS: Forty-two healthy naval aviation personnel served as subjects in this double-blind, randomized, placebo-controlled crossover study. Subjective drowsiness, cognitive performance, and vigilance were measured under three conditions: 180 mg fexofenadine (F), 50 mg diphenhydramine (D) as a positive control, or placebo
