Maintaining the Divide: The Legitimate Role for the Public in Health and Science Policymaking.

Many of the most pressing policy choices we face are matters of health and science. How should we prepare for the next global pandemic? How should we manage the disruption and promise of artificial intelligence? How should we address the challenge of climate change? These choices are permeated by intractable disagreement, especially disagreement over the moral and prudential values we rely on to decide. Nonetheless, we must decide. Proceduralism is one of the most widely endorsed answers to the challenge that such normative disagreement presents. The policy choices we face, however, also inclu

Strengthening research preparedness for crises: lessons from Norwegian government agencies in using randomized trials and quasi-experimental methods to evaluate public policy interventions.

During public health crises such as pandemics, governments must rapidly adopt and implement wide-reaching policies and programs ("public policy interventions"). A key takeaway from the coronavirus disease 2019 (COVID-19) pandemic was that although numerous randomized controlled trials (RCTs) focussed on drugs and vaccines, few policy experiments were conducted to evaluate effects of public policy interventions across various sectors on viral transmission and other consequences. Moreover, many quasi-experimental studies were of spurious quality, thus proving unhelpful for informing public polic

The Mexican experience in monitoring and evaluation of public policies addressing social determinants of health.

Monitoring and evaluation (M&E) have gradually become important and regular components of the policy-making process in Mexico since, and even before, the World Health Organization (WHO) Commission on Social Determinants of Health (CSDH) called for interventions and policies aimed at tackling the social determinants of health (SDH). This paper presents two case studies to show how public policies addressing the SDH have been monitored and evaluated in Mexico using reliable, valid, and complete information, which is not regularly available. Prospera, for example, evaluated programs seeking to im

Enhancing the Capacity of Policy-Makers to Develop Evidence-Informed Policy Brief on Infectious Diseases of Poverty in Nigeria.

BACKGROUND: The lack of effective use of research evidence in policy-making is a major challenge in most low- and middle-income countries (LMICs). There is need to package research data into effective policy tools that will help policy-makers to make evidence-informed policy regarding infectious diseases of poverty (IDP). The objective of this study was to assess the usefulness of training workshops and mentoring to enhance the capacity of Nigerian health policy-makers to develop evidence-informed policy brief on the control of IDP. METHODS: A modified "before and after" intervention study de

Factors shaping the global political priority of addressing elder abuse: a qualitative policy analysis.

BACKGROUND: Globally, 1 in 6 people aged 60 years and older experience elder abuse in the community annually, with potentially severe physical and mental health, financial, and social consequences. Yet, elder abuse remains a low global priority. We aimed to identify the factors accounting for the low global political priority of elder abuse. METHODS: We systematically searched relevant peer-reviewed literature and organisational reports in multiple databases and interviewed 26 key informants in the field of elder abuse. We used policy frameworks developed by previous research into the determi

Patient mobility within national borders. Drivers and politics of cross-border healthcare agreements in the Spanish decentralized system.

Cross-border patient mobility has become a topic of increasing interest for policy-makers and academic scholars. However, the focus on international dynamics hinders the fact that healthcare mobility takes place within national boundaries as well, particularly in countries characterized by decentralized health systems. This paper shifts the focus from the drivers of international patient mobility to the ones of policy-making on patient mobility within national borders, analyzing more than fifty policy arrangements adopted between Spanish Regions in the period 2000-2020. As the findings indicat

Producing the 'problem' of new psychoactive substances (NPS) in English prisons.

BACKGROUND: There has been a significant change in the types of substances consumed within English prison settings in the last eight years. There have been particular concerns regarding the acceleration in the use and availability of New Psychoactive Substances (NPS), mainly synthetic cannabinoids. Although NPS were identified as a 'problem' in prisons in 2011, government responses emerged only in 2015. As yet, there is no overarching policy document or strategy for dealing with NPS. This paper analyses the various strands of the response to NPS in prisons published from January 2015 to Decemb

Identifying and strengthening the structural roots of urban health in Canada: participatory policy research and the urban health agenda.

An urban health research agenda for health promoters is presented. In Canada, urban issues are emerging as a major concern of policy makers. The voices raising these issues are from the non-health sectors, but many of these issues such as increasing income inequality and poverty, homelessness and housing insecurity, and social exclusion of youth, immigrants, and ethno-racial minorities have strong health implications as they are important social determinants of health. Emphasis on these and other social determinants of health and the policy decisions that strengthen or weaken them is timely as

The judge as manager: the case of Judge Frank Johnson.

Judge Johnson's remarkable role in the reform of Alabama's mental health and prison facilities suggests that judges may have greater control over the agendas of policymaking than is normally assumed. In omnibus cases, the judge may be more interested in pushing state officials in the "right" direction than in securing full compliance with any specific set of absolute standards. The tenor and pace of implementation, moreover, may depend largely on the relationship of participants, the judge's personality, and related factors not susceptible to precise measurements.

The involvement of stakeholders in computational model development to support decision-making in public health: a scoping review.

BACKGROUND: The involvement of stakeholders in the development of computational models to support public health policy decisions has gained popularity, as it can enhance the quality of these models. This scoping review aims to synthesise the current body of literature on how stakeholders have been involved in the development of computational models in public health. METHODS: A literature search was performed in MEDLINE, APA PsycInfo and Scopus, from inception to October 2023. In addition, supplementary articles were identified through snowballing and a Google Scholar search. We searched for s

Housing for people with mental illness: update of a report to the President's New Freedom Commission.

A significant barrier to participation in community life for people with serious mental illness is the lack of decent, safe, affordable, and integrated housing of their choice linked with supportive services. The nation's affordable housing and mental health systems have historically failed to address consumers' housing needs and choices. The lack of housing has resulted in disproportionately high rates of homelessness and chronic homelessness. The author summarizes these issues, which were examined by the Subcommittee on Housing and Homelessness of the President's New Freedom Commission, and

A policy analysis of the three-month waiting period for new and returning residents to access provincial health insurance in Ontario, Canada.

BACKGROUND: Ontario's three-month waiting period (Regulation 552, subsection 5 (1) for health insurance eligibility under the Ontario Health Insurance Plan (OHIP) was implemented in 1994 amidst a severe economic recession. Justified by legislators as a cost-saving measure, the policy has affected newcomers during a vulnerable resettlement period. OBJECTIVE: This analysis examines the policy and political factors underpinning both the policy's implementation in 1994 and temporary suspension during the Covid-19 pandemic in 2020. METHODS: This is a descriptive health policy analysis using the 3

Developing a strategy for identifying recommendations prioritized for implementation in the Colombian health system.

BACKGROUND: Health systems guidelines (HSGs) developed by the WHO and clinical practice guidelines (CPGs) developed by the Colombian Ministry of Health can potentially support national and subnational policy-development processes. Barriers to implementation encountered when making necessary changes to the Colombian health system led to our research question: What prioritized recommendations are pending implementation in Colombia, and which require changes in health system arrangements? METHODS: A document analysis of all 58 Colombian CPGs and all 14 WHO HSG documents was performed in April 20

Informing prostate cancer screening policy makers in the European Union: lessons from cancer screening governance and policymaking.

Prostate cancer (PCa) poses a significant global health threat, with high incidence and mortality rates. In 2022, the Council of the European Union (EU) updated its screening recommendations, prioritizing PCa screening. This signals a crucial step towards establishing new early detection programmes in EU member states. This study investigates the role of policy makers and governance in cancer screening to inform the development of PCa screening. We had a mixed-method study design. First, a rapid review was conducted on policy making and governance in EU-funded cancer screening initiatives. Sec

Science for vaccine policy: Independent review of the September 2025 ACIP processes, deliberations and votes.

BACKGROUND: Since 1964, the Advisory Committee on Immunization Practices (ACIP) has shaped US vaccine policy but recently underwent significant structural and procedural changes affecting recommendation quality. This paper presents analyses of the September 2025 ACIP meeting by former ACIP voting members. METHODS: We examined ACIP policymaking maturity using a standardized tool for assessing national vaccine advisory groups' development and functionality. We also analyzed evidence presented, processes, and vaccine recommendations. RESULTS: Between April and September 2025, ACIP's policymakin

Facilitating knowledge transfer during Australia's COVID-19 vaccine rollout: an examination of 'Functional Dialogues' as an approach to bridge the evidence-policy gap.

BACKGROUND: Our interdisciplinary team initiated a project to inform the COVID-19 vaccination programme. We developed a novel research co-creation approach to share emerging findings with government. AIMS AND OBJECTIVES: We critically assess the 'Functional Dialogue' (FD) programme for future research translation practices in time-limited policy-making scenarios. We identify what factors helped us to put the FDs together and consider their effects on all aspects of the research programme. We draw out key moments of impact, weaknesses and challenges and identify how future FDs might be enhance
