Parallel use of low-complexity automated nucleic acid amplification tests on respiratory and stool samples with or without lateral flow lipoarabinomannan assays to detect pulmonary tuberculosis disease in children.

BACKGROUND: Low-complexity automated nucleic acid amplification tests (LC-aNAATs) are molecular assays widely used to diagnose tuberculosis disease in children. The lateral flow urine lipoarabinomannan assay (LF-LAM) is recommended for use amongst children with HIV. Previous systematic reviews have assessed the diagnostic accuracy of LC-aNAATs and LF-LAM separately in children, but in clinical practice the tests may be used concurrently, i.e. in 'parallel'. OBJECTIVES: To compare the diagnostic accuracy of the parallel use of LC-aNAAT on respiratory and stool specimens in children, and with L

Evaluation of sample pooling for SARS-CoV-2 RNA detection in nasopharyngeal swabs and salivas on the Roche Cobas 6800.

The Roche Cobas SARS-CoV-2 test recently received an Emergency Use Authorization from the U.S. Food and Drug Administration UA for pooling of up to six nasopharyngeal swab samples (NPS). We evaluated the 6-pool approach on both NPS and saliva samples using 564 samples (20 positive NPS and saliva samples each and 262 negative NPS and saliva samples each). The sensitivity of the Roche SARS-CoV-2 RNA test for pooled NPS samples was 100 % (95 %CI: 83.2-100 %) and the sensitivity for pooled saliva samples was 90 % (95 % CI: 68.3-98.8 %). Given the high throughput of the Roche Cobas 6800, pooling of

Validation of SARS CoV-2 detection by real-time PCR in matched pooled and deconvoluted clinical samples before and after nucleic acid extraction: a study in tertiary care hospital of North India.

The diagnosis of coronavirus disease-19 (COVID-19) relies on the detection of severe acute respiratory syndrome coronavirus 2 (SARS CoV-2) RNA by real-time reverse-transcription polymerase chain reaction in respiratory samples. Rapid increase in the COVID-19 cases across the world requires fast and efficient testing as testing capacity is a bottleneck in diagnosis. In this context, pooling strategy can be opted for rapid testing in a cost-effective manner. In this study, the authors have optimized and compared the effect of pooling (5 and 10 samples) before and after nucleic acid extraction. I

Flexible upscaling of laboratory PCR testing capacity at the Robert Koch Institute during the SARS-CoV-2 pandemic.

BACKGROUND: Over the course of the COVID-19 pandemic, laboratories worldwide have been facing an unprecedented increase in demand for PCR testing because of the high importance of diagnostics for prevention and control of virus spread. Moreover, testing demand has been varying considerably over time, depending on the epidemiological situation, rendering efficient resource allocation difficult. Here, we present a scalable workflow which we implemented in our laboratory to increase PCR testing capacity while maintaining high flexibility regarding the number of samples to be processed. METHODS:

Prevalence and characteristics of asthma in the aquatic disciplines.

BACKGROUND: Despite the health benefits of swimming as a form of exercise, evidence exists that both the swimming pool environment and endurance exercise are etiologic factors in the development of asthma. The prevalence of asthma in swimmers is high compared with that in participants in other Olympic sport disciplines. There are no publications comparing the prevalence of asthma in the 5 aquatic disciplines. OBJECTIVE: The purpose of this study is to examine and compare the prevalence of asthma in the aquatic disciplines and in contrast with other Olympic sports. METHODS: Therapeutic Use Ex

Risk of malignancy in PTEN-altered thyroid nodules detected on preoperative FNA molecular testing: a systematic review and meta-analysis.

AIMS: Phosphatase and tensin homolog (PTEN) alterations are increasingly encountered on molecular testing of thyroid fine-needle aspiration (FNA) specimens in Bethesda III/IV nodules. Unlike high-specificity alterations (e.g., BRAF V600E), PTEN alterations can map to a broad morphologic spectrum and are influenced by the diagnostic pathway determining which nodules proceed to surgery. We performed a systematic review and meta-analysis to define risk of malignancy (ROM) for PTEN-altered thyroid nodules detected preoperatively. METHODS: Studies (2020-2025) reporting PTEN alterations detected pr

Active recall to increase HIV and STI testing: a systematic review.

BACKGROUND: Active recall can improve reattendance rates and could increase retesting rates and detection of HIV and sexually transmitted infections (STIs), but the best strategy remains uncertain. METHODS: We conducted a systematic review and meta-analysis of active recall for HIV and/or STI testing. We searched six electronic databases using terms for HIV, STIs, tests and active recall (defined as a reminder to retest for HIV/STIs) for randomised, non-randomised and observational English-language studies published between 1983 and 2013. Outcomes included reattendance/retesting rate and STI

Sputum scarcity among adolescents and adults with presumptive tuberculosis: a systematic review and meta-analysis.

BACKGROUND: The diagnosis of tuberculosis (TB) typically relies on being able to produce a sputum sample for microbiological testing. However, sputum scarcity, the inability to self-expectorate an adequate sputum sample for TB testing, is a well-known concern. Our systematic review and meta-analysis investigated the proportion of sputum scarcity among adolescents and adults being evaluated for presumptive TB in healthcare facilities. METHODS: We searched PubMed, Embase, Cochrane Library, Web of Science, and clinical trials databases with no language restrictions from January 2010 to October 2

Prevalence and antimicrobial resistance pattern of Clostridium difficile among hospitalized diarrheal patients: A systematic review and meta-analysis.

BACKGROUND: Clostridium difficile is the leading cause of infectious diarrhea that develops in patients after hospitalization during antibiotic administration. It has also become a big issue in community-acquired diarrhea. The emergence of hypervirulent strains of C. difficile poses a major problem in hospital-associated diarrhea outbreaks and it is difficult to treat. The antimicrobial resistance in C. difficile has worsened due to the inappropriate use of broad-spectrum antibiotics including cephalosporins, clindamycin, tetracycline, and fluoroquinolones together with the emergence of hyperv

Two-Stage Hierarchical Group Testing Strategy to Increase SARS-CoV-2 Testing Capacity at an Institution of Higher Education: A Retrospective Analysis.

Population testing for severe acute respiratory syndrome coronavirus 2 is necessary because of the potential for viral transmission from asymptomatic cases, yet the scarcity of reagents and equipment has increased the cost-prohibitive implementation of screening campaigns at institutions of higher education. Significant analytical sensitivities of nucleic acid amplification methods permit sample pooling to increase testing capacity. Statistical models compared optimal testing configurations for pools of 3, 5, and 10 samples. Assessment of pooling using the TaqPath COVID-19 Combo Kit multiplex

Can qPCR on pooled sheep milk detect brucellosis as part of herd brucellosis control and eradication programs?

BACKGROUND: Brucellosis is a zoonotic disease occurring worldwide. Brucella melitensis is the main cause of malta fever in humans and a major cause of abortion in sheep. In herd control and prevention programs, applying a suitable noninvasive method for accurate, rapid, and cost-effective monitoring of infected animals is a major concern. This study aimed to investigate lots of pooled sheep milk using qPCR to detect Brucella spp. infection in sheep. By calculating the limit of detection of Brucella in qPCR and microbial culture, the maximum number of pooled milk lots that retained the ability
