Fatal case of inhalational anthrax mimicking intra-abdominal sepsis.

In this report, we discuss the second fatal case of inhalational anthrax related to the use of Bacillus anthracis spores as a biological weapon in the United States. This case highlights two of the major characteristics of inhalational anthrax: the fulminating nature of the infection and the difficulty of promptly establishing a diagnosis. In the patient discussed here, gastrointestinal symptoms and findings were so impressive that the patient was thought to have a primary intra-abdominal condition. In the current situation, in which bioterrorism is a real threat, any patient presenting with a

Community pharmacy services for people with drug problems over two decades in Scotland: Implications for future development.

BACKGROUND: In Scotland community pharmacies are heavily involved in service delivery for people with drug problems (PWDP) as documented through surveys of all community pharmacies in 1995, 2000 and 2006. A further survey in 2014 enabled trends in service demand/provision to be analysed and provides insight into future development. METHODS: The lead pharmacist in every Scottish pharmacy (n=1246) was invited to complete a postal questionnaire covering attitudes towards PWDP and service provision and level of involvement in services (needle exchange, dispensing for PWDP and methadone supervisio

Prevalence and causes of early-onset dementia in Japan: a population-based study.

BACKGROUND AND PURPOSE: Few studies are available that have addressed the prevalence of early-onset dementia (EOD), including early-onset Alzheimer disease and other forms of dementia in Japan. METHODS: A 2-step postal survey was sent to all of the 2475 institutions providing medical or care services for individuals with dementia in Japan's Ibaraki prefecture (population, 2 966 000) requesting information on EOD cases. Data were then reviewed and collated. RESULTS: We identified 617 subjects with EOD. The estimated prevalence of EOD in the target population was 42.3 per 100 000 (95% CI, 39.4

Parental and primary care physicians' views on the management of chronic diseases: a study in Italy. The Italian Collaborative Group on Paediatric Chronic Diseases.

A survey on the burden and quality of care and the parental and primary care physicians' views on management of eight chronic illnesses and disabilities was conducted from 1990 to 1993. Data were collected on 993 children and adolescents from family interviews and physicians' postal questionnaires. Approximately 70% of patients used two or more services for care management and 149 children were treated outside their region. Only 36% of the physicians were case managers and half of these agreed that better communication with other care providers could facilitate their role. A wide difference in

Using patients' experiences to identify priorities for quality improvement in breast cancer care: patient narratives, surveys or both?

BACKGROUND: Patients' experiences have become central to assessing the performance of healthcare systems worldwide and are increasingly being used to inform quality improvement processes. This paper explores the relative value of surveys and detailed patient narratives in identifying priorities for improving breast cancer services as part of a quality improvement process. METHODS: One dataset was collected using a narrative interview approach, (n = 13) and the other using a postal survey (n = 82). Datasets were analyzed separately and then compared to determine whether similar priorities for

The corporatization of community pharmacy: implications for service provision, the public health function, and pharmacy's claims to professional status in the United Kingdom.

BACKGROUND: Pharmacy has experienced both incomplete professionalization and deprofessionalization. Since the late 1970s, a concerted attempt has been made to re-professionalize pharmacy in the United Kingdom (UK) through role extension-a key feature of which has been a drive for greater pharmacy involvement in public health. However, the continual corporatization of the UK community pharmacy sector may reduce the professional autonomy of pharmacists and may threaten to constrain attempts at reprofessionalization. OBJECTIVES: The objectives of the research: to examine the public health activi

Applying the principles of isotope analysis in plant and animal ecology to forensic science in the Americas.

The heart of forensic science is application of the scientific method and analytical approaches to answer questions central to solving a crime: Who, What, When, Where, and How. Forensic practitioners use fundamentals of chemistry and physics to examine evidence and infer its origin. In this regard, ecological researchers have had a significant impact on forensic science through the development and application of a specialized measurement technique-isotope analysis-for examining evidence. Here, we review the utility of isotope analysis in forensic settings from an ecological perspective, concen

'Stressed, uncomfortable, vulnerable, neglected': a qualitative study of the psychological and social impact of the COVID-19 pandemic on UK frontline keyworkers.

OBJECTIVES: Non-healthcare keyworkers face distinct occupational vulnerabilities that have received little consideration within broader debates about 'essential' work and psychological distress during the COVID-19 pandemic. The aim of this study was therefore to explore the impact of the pandemic on the working lives and mental health and well-being of non-healthcare keyworkers in the UK. DESIGN: In-depth, qualitative interviews, analysed using a reflexive thematic analysis. SETTING: Telephone or video call interviews, conducted in the UK between September 2020 and January 2021. Cooperation between Single-Handed and Group Practices Ensures the Replacement of Gastrostomy Tubes and Tracheal Cannulas in Home Medical Care Settings.

In Japan, patients who require home medical care are increasing especially in the elderly. In home medical care settings, devices such as gastrostomy tubes, tracheal cannulas, and urethral catheters are usually replaced by visiting physicians or nurses. However, device replacement services are not always available in Japan. Unless device replacement services are sufficiently provided to patients at home, patients have to suffer various disadvantages, including a forced visit to a hospital for device replacement despite inability to walk. We therefore investigated background factors of clinics

An analysis of blood lead data in clinical records by external data on lead pipes and age of household.

This study examined the possibility that lead pipes in the drinking water distribution system were elevating the blood lead levels of children in London, Ontario, Canada. Based on their postal codes, 164 children admitted between 1984 and 1989 to an institution for the behaviorally disordered or developmentally challenged were categorized according to whether they lived in the area of the city known by the local Public Utilities Commission to be serviced by lead pipes. Analysis of covariance was used to obtain confounder-adjusted geometric means in each area. After adjusting for gender, year o

Measuring barriers to home bowel cancer screening in rural Australia: Development and validation of a survey instrument.

OBJECTIVE: This study aimed to develop and psychometrically validate an adapted version of the Barriers to Home Bowel Cancer screening Scale relevant to rural communities. METHODS: A combined scale assessing general and "rural-specific" barriers to bowel screening was developed and presented via a cross-sectional online survey to 526 screening-eligible Australians (78% rural). Participants rated the extent to which each barrier would deter them from participating in at-home bowel cancer screening. Exploratory factor analysis identified latent screening barriers among rural participants (n=416

Review of Decontamination Techniques for the Inactivation of Bacillus anthracis and Other Spore-Forming Bacteria Associated with Building or Outdoor Materials.

Since the intentional release of Bacillus anthracis spores through the U.S. Postal Service in the fall of 2001, research and development related to decontamination for this biological agent have increased substantially. This review synthesizes the advances made relative to B. anthracis spore decontamination science and technology since approximately 2002, referencing the open scientific literature and publicly available, well-documented scientific reports. In the process of conducting this review, scientific knowledge gaps have also been identified. This review focuses primarily on techniques

Impact of a Clinic- and Mail-Based Cervical Cancer Human Papillomavirus Self-Collect Screening Program.

Virtually all cervical cancers are caused by high-risk human papillomaviruses (HPVs), which can be accurately detected using deoxyribonucleic acid (DNA) testing from cervical or vaginal samples. Patient vaginal HPV self-collection (HPV self-collect) presents new opportunities to expand access to cervical cancer screening, improve uptake, and reach underrepresented populations. An integrated health system in Washington State implemented a mail- and clinic-based HPV self-collect program. Researchers partnered with health system leaders and clinicians to evaluate program implementation. This repo

Email recruitment for chronic pain clinical trials: results from the LAMP trial.

BACKGROUND: Recruitment for clinical trials and large-scale studies is challenging, especially for patients with complex conditions like chronic pain. Email recruitment has the potential to increase efficiency, to reduce costs, and to improve access for underrepresented patient populations. The objective of this study was to examine the effectiveness, efficiency, and equitability of email versus postal mail recruitment for the Learning to Apply Mindfulness to Pain (LAMP) study, a three-site clinical trial of mindfulness-based interventions for chronic pain. METHODS: Patients with chronic pain
