Confounding factors of transvaginal ultrasound accuracy in endometrial cancer.

Endometrial cancer is the most frequently diagnosed gynecological tumour. Transvaginal ultrasound has a leading role in the preoperative evaluation of endometrial cancer patients. The study aimed to identify factors that can worsen the diagnostic accuracy of transvaginal ultrasound in endometrial cancer patients. We retrospectively analysed 290 patients with histological diagnosis of endometrial adenocarcinoma. Two-dimensional (2D) gray-scale ultrasound and power Doppler imaging were performed. Age, menopause status, obesity, parity, Figo stage and benign uterine disorders were evaluated as po

Assessing Preoperative Opioid Use and Outcomes in Patients With Cystic Fibrosis After Lung Transplantation.

INTRODUCTION: Pain syndromes are well characterized in people with cystic fibrosis (pwCF). Recent evidence links preoperative opioid use to higher morbidity and worse survival in lung transplantation (LT) recipients, but this is not well studied in pwCF, who may require more opioids than other LT candidates. METHODS: We merged the Scientific Registry of Transplant Recipients with pharmacy claims databases and identified pwCF who underwent LT from 2011 to 2021. We defined pre-LT opioid use as filling an opioid prescription within six mo pretransplant. We assessed postoperative complications an

Combined use of iPTH and D/W ratio in preoperative diagnosis of parathyroid carcinoma.

PURPOSE: Parathyroid carcinoma is an extremely rare malignancy, with an unfavorable clinical outcome and a high recurrence rate after initial surgery. Because surgical removal without tumor cells is the only curative treatment, a preoperative diagnosis is important to develop a strategy for complete removal of tumor cells. This study aimed to examine preoperative methods to differentiate parathyroid carcinoma from parathyroid adenoma. METHODS: We retrospectively analyzed the clinical and pathological features of patients with parathyroid carcinoma or parathyroid adenoma who were undergoing tr

Effectiveness of promethazine on preoperative and intraoperative sequelae in cleft palate surgeries.

INTRODUCTION: Anxiety and nosocomial infection are the most common reported problems in children undergoing cleft surgeries. Research shows that there is an enigma in the use of antihistamine therapy in children for the management of upper respiratory tract infection. 'Promethazine' is a first-generation H1 receptor antagonist, and antihistamine also has strong sedative effects. Our study aims at evaluating the Effectiveness of Promethazine (Phenergan) in preoperative and intra operative sequelae in cleft surgeries. MATERIALS AND METHODS: This is a single-centre, parallel, randomized, double-

Impact of pre-operative factors on post-operative atrial fibrillation outcomes after cardiac surgery: systematic review with meta-analysis, trial sequential analysis, and meta-regression.

OBJECTIVES: Post-operative atrial fibrillation (POAF) is the most common arrhythmia following cardiac surgery. We conducted a systematic review and meta-analysis to quantify the association between POAF and adverse outcomes. METHODS: We searched PubMed, Embase, CENTRAL, and Web of Science through December 2024. Studies comparing outcomes between POAF and non-POAF patients after cardiac surgery were included. Random-effects meta-analysis was performed. Trial sequential analysis, Bayesian analysis, subgroup analyses, and meta-regression were conducted. Evidence certainty was assessed using the

Impact of preoperative mental health diagnosis on postoperative opioid use patterns in spine fusion surgery: A systematic literature review.

Opioids are frequently prescribed for patients undergoing procedures such as spinal fusion surgery for the management of chronic back pain. However, the association between a preoperative mental health illness, such as depression or anxiety, and opioid use patterns after spinal fusion surgery remain unclear. Therefore, we performed a systematic literature review in accordance with PRISMA guidelines to identify articles from the PubMed Database that analyzed the relationship between preoperative mental health illness and postoperative opioid usage after spinal fusion surgery on June 1, 2023. Preoperative Assessment of Cervical Involvement in Endometrial Cancer by Transvaginal Ultrasound and Magnetic Resonance Imaging: A Systematic Review and Meta-Analysis.

OBJECTIVE: To compare the diagnostic accuracy of transvaginal ultrasound (TVS) and magnetic resonance imaging (MRI) for detecting cervical infiltration by endometrial carcinoma using meta-analysis assessment. METHODS: An extensive search of papers comparing TVS and MRI for assessing cervical infiltration in endometrial cancer in the same set of patients was performed in Medline (Pubmed), Web of Science, and the Cochrane Database. Quality was assessed using QUADAS-2 tool (Quality Assessment of Diagnostic Accuracy Studies-2). Quantitative meta-analysis was performed. RESULTS: Our extended sear

Utility of Preoperative Electrodiagnosis Together with Peripheral Nerve High-Resolution Ultrasound: A Complex Case Report of Neurofibromatosis Type I.

Our case report underscores the importance of electroneuromyography (ENMG) combined with peripheral nerve high-resolution ultrasound (HRUS) in the evaluation of neurofibromatosis type 1 (NF1). A 49-year-old woman affected by NF1 came to our attention because of new-onset left arm weakness and atrophy. Debulking of a cervicothoracic C7-T1 neurofibroma had been performed 8 years earlier. On current admission, magnetic resonance imaging disclosed increased lesion volume that was thought to cause the neurologic deficits by compressing the C8 root. Findings from intraoperative neurophysiologic moni

Prophylactic cognitive enhancers for improvement of cognitive function in patients undergoing electroconvulsive therapy: A systematic review and meta-analysis.

OBJECTIVE: Cognitive enhancers, including cholinesterase inhibitors and memantine, are used to treat dementia, but their effect for reducing post-electroconvulsive therapy (post-ECT) cognitive side effects is unclear. We conducted a systematic review and meta-analysis to assess the effectiveness of cognitive enhancers in the prevention of cognitive side effects due to ECT. METHODS: We identified relevant studies by searching electronic databases (e.g., PubMed, EMBASE, Web of Science, Cochrane Library). Only studies published up to October 2019 comparing cognitive enhancer vs placebo for cogni

Localization of Ventricular Arrhythmias for Catheter Ablation: The Role of Surface Electrocardiogram.

The surface ECG is a valuable mapping tool in patients with idiopathic and scar-related ventricular arrhythmias (VAs). A detailed analysis of 12-lead ECG can provide useful information in localizing the VA site of origin. This might help tailoring the ablation strategy to optimize procedural duration, increase the probability of success, and prevent complications. The aim of this article is to review the ECG features of both idiopathic and scar-related VAs and discuss their potential implications for optimizing the ablation strategy.

Research progress of asymptomatic bacteriuria before arthroplasty: A systematic review.

BACKGROUND: A high prevalence of asymptomatic bacteriuria exists in patients prior to arthroplasty, and urinary tract infection is considered to be a source of postoperative superficial wound and prosthetic joint infections. There is no consensus whether to screen for and treat asymptomatic bacteriuria before arthroplasty. OBJECTIVE: To summarize the association between asymptomatic bacteriuria and complications after arthroplasty and to evaluate the clinical benefits of treating asymptomatic bacteriuria prior to arthroplasty. METHOD: We systematically searched PubMed, Embase, and the Cochra

Nutrient deficiencies prior to bariatric surgery.

PURPOSE OF REVIEW: The purpose of this review is to provide an update of recent additions to our understanding of the prevalence of nutrient deficiencies and the potential role of preoperative weight loss in contributing to these deficiencies in obese individuals planning to undergo bariatric surgery. RECENT FINDINGS: Recent reports that have included bariatric surgery candidates from sites around the world have shown consistent deficiencies in a variety of nutrients. Although protein-energy malnutrition is uncommon preoperatively, micronutrient deficiencies occur commonly with multiple defic

The impact of cleft lip and palate repair on maxillofacial growth.

Surgical correction is central to current team-approached cleft treatment. Cleft surgeons are always concerned about the impact of their surgical maneuver on the growth of the maxilla. Hypoplastic maxilla, concaved mid-face and deformed dental arch have constantly been reported after cleft treatments. It is very hard to completely circumvent these postoperative complications by current surgical protocols. In this paper, we discussed the factors that inhibit the maxillofacial growth on cleft patients. These factors included pre-surgical intervention, the timing of cleft palate and alveolae repa

Preoperative and postoperative nipple-areola sensibility in patients undergoing reduction mammaplasty.

Objective data on nipple and areola sensibility are scarce. For women with macromastia, there is little published information available indicating the incidence and intensity of postoperative nipple and areola sensibility. This prospective study was undertaken to evaluate nipple and areola sensibility in "small-breasted" control subjects as well as in patients with macromastia before and after reduction mammaplasty. Preoperative and postoperative Semmes-Weinstein pressure threshold testing was performed on 84 breasts in 43 patients and on 12 breasts of A or B cup size in the control group. The

Present state of intentional hemodilution.

Preoperative intentional hemodilution is induced by isovolemic exchange of whole blood with colloid solutions in order to gain autologous blood while maintaining normovolemia. The basic mechanism that compensates for the fall of oxygen capacity of the diluted blood is the rise in cardiac output, and organ blood flow, factors that result from the improved fluidity of blood at lower hematocrits. Normal tissues maintain adequate oxygenation during hemodilution through the enhanced redistribution of blood. In ischemic tissues this effect is enhanced, and causes an increase in the oxygenation of is

[Congenital diaphragmatic hernias].

The chances of survival of infants with congenital diaphragmatic hernia have improved only slightly in recent years. Hence, surgery has little to offer towards improvement of prognosis. For this reason, new avenues must be explored which guarantee the functioning of post-foetal circulation and prevent a relapse into the foetal circulation. The emphasis is on continuous registration of the preductal and post-ductal arterial oxygen content, of the pressure conditions in the pulmonary artery and the extent of a right-left shunt. Among the most important therapeutic measures are high-frequency ven
