[Postpartum psychiatry].

The four important syndromes that constitute postnatal psychiatry are chronologically: 1) Post-Traumatic Stress Disorder (PTSD), 2) Baby-blues, 3) Puerperal psychosis and 4) Postnatal depression. The PTSD (prevalence 1%) occurs from 24 to 48 hours after a difficult childbirth. Anxiety predominates in association with nightmares and recurrent images of the experience. Baby-blues (prevalence 80%) is a feeling of sadness that doesn't need to be treated. It occurs around the third day of post-partum. Its intensity is a predictive factor of postnatal depression. Puerperal psychosis (prevalence 0.2%

Calm after the storm? Clinical phenotypes and risk markers in suicidal patients: data from a liaison-consultation psychiatric setting.

INTRODUCTION: Suicidality represents an increasing public health matter and is more frequently associated with higher psychiatric vulnerability and with greater severity of psychopathological manifestations. Suicide risk may arise from a complex interplay of socio-environmental and clinical factors, which can be promptly explored during psychiatric consultations following a suicide attempt (SA). The present study aims at clarifying the correlates of suicidality in a liaison-consultation setting, in order to characterize risk profiles that could be targeted by preventive strategies. METHODS: T

Epigenetic Mechanisms of Suicidal Behavior Formation: A Review.

BACKGROUND: Suicidal behavior is a complex and multifactorial phenomenon rooted in both psychological and biological mechanisms. In recent years, there has been an increasing focus on epigenetic factors, which modulate the influence of environmental factors on the expression of genes associated with emotional and cognitive regulation. REVIEW OF THE LITERATURE: This review aims to provide a comprehensive analysis of contemporary scientific data concerning the epigenetic mechanisms implicated in the pathogenesis of suicidal behavior. It synthesizes and systematizes findings published between 20

Mental health among healthcare workers during COVID-19: a study to oversee the impact of the risk perception and relationship with inflammation from blood-based extracellular vesicles.

INTRODUCTION: Preventive measures have been implemented in hospitals during COVID-19, but how these guidelines affected mental health among healthcare workers (HCWs) remains to be determined. On another note, reliable psychological and blood-based markers are needed to promptly identify HCWs at-risk to develop distress. Extracellular vesicles (EVs) originating from brain cross the blood-brain barrier and are detectable in blood, giving them a highly valuable potential for biomarker discovery. In HCWs with or without psychological distress, we investigated how perceived stress during COVID-19 i

Factors associated with victimization in dual diagnosis patients.

BACKGROUND: Patients with a substance use disorder and co-occurring mental disorder are prone to victimization. There is a lack of research identifying variables related to violent and property victimization in this high risk group. The aim of this study was to identify factors associated with violent and property victimization in male and female dual diagnosis patients in order to identify targets for prevention. METHODS: In a cross-sectional study, victimization and demographic, clinical and psychological characteristics were assessed in 243 treatment-seeking patients with dual diagnosis. P

Psychometric assessment of dynamic risk factors for child molesters.

To explore the relationship between dynamic risk factors and recidivism in child molesters, we studied a sample of men (N=495) who completed an intensive, prison-based treatment program in New Zealand. During the follow-up period (M=5.8 years), 9.9% were reconvicted for a sexual offense. A self-report psychometric battery was administered at pre-treatment that assessed a range of variables related to sexual attitudes and beliefs, emotional functioning, and interpersonal competency. Factor analysis showed that individual differences in the battery could be described by four dimensions-Social In

Long-term effects of an intervention for families with parental severe mental illness-A 2.5-year RCT follow-up.

BACKGROUND: Children of parents with severe mental illness are at markedly increased risk of developing psychiatric disorders themselves. Interventions that target family-related risk and protective factors may represent a promising approach in preventive psychiatry. This study evaluated the long-term effects of the VIA Family intervention, a family-based program designed for children at familial risk of severe mental illness and their families. METHODS: We conducted a 2.5-year follow-up of a randomized clinical trial comparing the VIA Family intervention with treatment as usual (TAU). Eligib

Major Depressive Disorder and Cardiovascular Disease Risk in Children and Adolescents.

OBJECTIVE: Major depressive disorder (MDD) in youth is an independent risk factor for premature cardiovascular disease (CVD). While previous research has considered individual CVD risk factors in relation to depression, this study examined the association between depression and CVD risk among children and adolescents with MDD using a validated composite measure. METHODS: Youth with MDD were recruited through an outpatient psychiatry program and underwent semistructured psychiatric diagnostic assessment to confirm diagnosis. Healthy control participants were recruited from community settings.

Violence Against Healthcare Workers in a University Hospital of Central Italy: How Risk Management Interventions Can Help Change a Trend.

Background/Objectives: Violence against workers (physical, verbal, or psychological abuse), even if it affects all professional categories, is up to ten times higher in healthcare workers. A University Hospital of Central Italy has gradually implemented a series of preventive measures, which might have impacted the trend of the phenomenon. Methods: In order to monitor the episodes, an aggressive event reporting form was adopted throughout the hospital. Data extracted both from this form and the root cause analysis documentation regarding the period January 2019-December 2023 were used. Descrip

A qualitative study of perceptions related to family risk of bipolar disorder among patients and family members from India.

BACKGROUND: Over the past decade there have been rapid advances in psychiatry genetics and there is a growing concern about the clinical and practical relevance of this knowledge. Efforts are being made to know the perspectives of patients and family members regarding family risk and genetic counselling. However, the area remains largely unexplored in psychiatric research from Asian countries. AIM: The primary purpose of the study was to explore the perceptions related to family risk among Indian patients with bipolar disorder and their family members. The study also aimed to explore the rela

Clinical Utility of Early Intervention Including the 5-Step Precision Medicine Method in First-Episode Psychosis: Protocol for a Cohort Study With Nested Economic and Process Evaluations.

BACKGROUND: Psychotic disorders such as schizophrenia present a significant challenge to health care systems due to their high disability rates and treatment costs. With discontinuation rates for antipsychotics reaching over 40% in the first year and 80% after 3 years, it is crucial to tailor antipsychotic selection and dosing early in treatment. Personalized precision psychiatry, underpinned by pharmacogenetics, holds considerable potential in individualizing antipsychotic treatment for patients with first-episode psychosis. An internationally pioneering method called 5-step precision medicin

Risk assessment in offenders with mental disorders: relative efficacy of personal demographic, criminal history, and clinical variables.

Following the meta-analysis by Bonta, Law, and Hanson, (1998) this study examined the ability of personal demographic, criminal history, and clinical variables to predict reoffending in offenders in the United Kingdom who had mental disorders. The efficacy of each variable in predicting rate of general reoffending and violent reoffending was investigated. Age on admission, number of days hospitalized, and number of previous offenses were the most effective variables in predicting re-offending, with number of previous offenses being the strongest predictor. Clinical diagnosis was not predictive

A case of suicide of psychiatric patient in the Sicilian hospital: accidental or professional liability of "top management"?

BACKGROUND: In the hospital environment, any liability for organizational inadequacy and/or inefficiency, or again for defects in the facilities, or inadequacy of health equipment may be found in the work of the apex professional figures: Medical Director, the General Manager, and/or others. The responsibility for the internal organization of the department falls on the figure of the Director of the Complex Structure where the deficiencies have not been promptly and adequately reported to the top figures. Possible paths to increase detection of child sexual abuse in child and adolescent psychiatry: a meta-synthesis of survivors' and health professionals' experiences of addressing child sexual abuse.

BACKGROUND: Efforts are directed both towards prevention and early detection of Child sexual abuse (CSA). Yet, only about 50% of CSA survivors disclose before adulthood, and health professionals rarely are the first disclosure recipients. Increasing the detection rate of CSA within the context of Child and Adolescent Psychiatry (CAP) therefore represents a significant secondary prevention strategy. However, facilitating CSA disclosure when the survivor is reluctant to tell is a highly complex and emotionally demanding clinical task. We therefore argue that efforts to increase detection rates o
