The syndrome of acquired verbal auditory agnosia in childhood with mutism and epileptic discharges has been described in over 100 cases. An encephalitic etiology has often been postulated but never proved. We report two patients with this syndrome who were treated surgically. Despite careful search, no pathologic evidence of encephalitis was found. One patient, with the typical course, had no seizures but striking positive correlation between epileptic discharge and language disorder; the second, after classic onset, developed intractable temporal lobe epilepsy, a previously unreported outcome

A 44-year-old female presented with anaplastic astrocytoma of the right temporal lobe which had destroyed the skull base and extended extracranially. Histologically, the extracranial portion of the tumor was astrocytoma with desmoplastic reaction. She was treated by subtotal tumor removal, radiation therapy, and chemotherapy, but died of meningeal dissemination. Spontaneous extradural extension of malignant glioma is rare and always fatal.

Almost all supratentorial intracranial lesions which may effectively control chronic pain in man are ineffective for acute pain. There are at least 10 sites at which lesions producing no somatic sensory loss have often stopped the peculiarly agonizing chronic pain of advanced cancer. They include: (a) inferior posteromedial or subcaudate (preinnominate) frontal white matter; (b) supracallosal portion of cingulum; (c) thalamotomy of centrum medianum and parafascicularis nuclei; (d) thalamotomy of linear parasagittal type separating connections between lateral specific and medial nonspecific sen

In the United States, frontal lobe lesioning procedures have been uniformly linked to the neurologist Walter Freeman, although the prefrontal lobotomy was investigated in other institutions in the United States, the United Kingdom, Europe, Russia, Japan, and China, mostly in patients with psychosis, obsessive-compulsive disorder, and/or intractable pain syndromes. These procedures were based on earlier reports of improvement of psychiatric symptoms after surgical resection of frontal lobe tumors and led many to infer a causal relationship between frontal lobe dysfunction and abnormal behavior.

BACKGROUND: Psychiatric surgery, including deep brain stimulation and stereotactic ablation, is an important treatment option in severe refractory psychiatric illness. Several large trials have demonstrated response rates of approximately 50%, underscoring the need to identify and select responders preoperatively. Recent advances in neuroimaging have brought this possibility into focus. We systematically reviewed the psychiatric surgery neuroimaging literature to assess the current state of evidence for preoperative imaging predictors of response.
METHODS: We performed this study in accordanc

AIM: To assess the surgical outcomes of temporo-parieto-occipital (TPO) and parieto-occipital (PO) disconnection surgery for children with intractable posterior quadrantic epilepsy and a unilateral posterior quadrant lesion based on MRI and functional imaging abnormality in the TPO region on one side.
METHODS: A retrospective review of data of 12 children who underwent TPO or PO disconnective surgery was carried out from September 2009 to September 2012. Three-dimensional surface reconstructions of MRI scans and intraoperative electrophysiological monitoring were used during surgery. Drugs we

Deep brain stimulation (DBS) is the most rapidly expanding field in neurosurgery. Movement disorders are well-established indications for DBS, and a number of other neurological and psychiatric indications are currently being investigated. Numerous contemporary opinions, reviews, and viewpoints on DBS fail to provide a comprehensive account of how this method came into being. Misconceptions in the narrative history of DBS conveyed by the wealth of literature published over the last 2 decades can be summarized as follows: Deep brain stimulation was invented in 1987. The utility of high-frequenc

The effects of bilateral anterior capsulotomy were analyzed in 12 patients with Tourette's syndrome (TS). All of the patients underwent bilateral anterior capsulotomy. In 7 patients, the target was the anterior one third of the anterior limb of the capsula interna, while in 5 patients, the posterior one third of this structure was targeted. Radiofrequency thermolesions were created at 80 degrees C for 60 s. These surgeries resulted in a reduction in TS symptoms (motor and vocal tics and/or obsessive-compulsive behavior) of >50% in all patients, regardless of the target. A comparison of the two

Callosotomy has played a unique role in the treatment of epilepsy and in the understanding of human brain function. The pioneering work of Dejerine and Liepmann presenting the first findings of callosal lesion pathology at the turn of the 20th century was accepted but then quickly forgotten. Two schools resurrected the phoenix of callosal syndromes: Roger Sperry and Michael Gazzaniga leading in experimental neuroscience, and Norman Geschwind leading in clinical neurology. Callosotomy remains an effective technique to treat atonic, tonic, and tonic-clonic seizures, especially in patients with s

BACKGROUND: The aim of this study was to investigate the outcome of neurosurgery for mental illness in a cohort of Egyptian/Arab patients with treatment-resistant obsessive-compulsive disorder (OCD).
METHODS: Sixteen patients were recruited from various psychiatric clinics. All participants met DSM-5 criteria for OCD and had failed extensive trials of psychiatric medications and psychotherapies. Eligibility criteria focused on significant distress and functional impairment, as well as a Yale-Brown Obsessive Compulsive Scale (YBOCS) score greater than 30. Patients underwent either cingulotomy

Although deep brain stimulation (DBS) is an established treatment for Parkinson's disease, the long-term suppression of tremor is still a challenging issue. We report two patients with tremor-dominant Parkinson's disease (PD) treated with unilateral thalamotomy of the ventralis intermedius nucleus (Vim) combined with the subthalamic nucleus (STN)-DBS or the posterior subthalamic area (PSA)-DBS. One year after the surgery, thalamotomy of the area from the Vim to the PSA showed improvement not only in tremor but also in rigidity and akinesia. PSA- or STN-DBS with low intensity stimulation elimin

BACKGROUND: Despite the development of effective pharmacologic and cognitive behavioral treatments for obsessive-compulsive disorder (OCD), some patients continue to be treatment-refractory and severely impaired. Fiber tracts connecting orbitofrontal and dorsal anterior cingulate cortex with subcortical nuclei have been the target of neurosurgical lesions as well as deep brain stimulation in these patients. We report on the safety and efficacy of ventral gamma capsulotomy for patients with intractable OCD.
METHODS: Fifty-five patients with severely disabling, treatment-refractory OCD received

The history of prefrontal lobotomy is an interesting example of medicine regarding as useful a treatment method which present-day consensus evaluates in a contrary fashion. A pilot study of archives from the Swedish state mental hospital Umedalen shows that the frequency of lobotomies as well as postoperative mortality were higher than what has earlier been assumed. The majority of the 704 patients who underwent lobotomy at Umedalen hospital were women. One unexpected finding concerns the numbers of mentally retarded patients and children who were subjected to lobotomy. Case records and other

PERIOD OF CUSTODIAL CARE ONLY: The magnificent "Institute of Mental Health" has its history almost from 1795 when the East India company appointed Surgeon Valentine Conolly to be in charge of a "House for accommodating persons of unsound mind." After a few transitions, backed by a government order for the construction of a lunatic asylum in a 66 1/2 acre site, the asylum started functioning from 1871. The period of about six decades from its inception could be referred to as "the period of custodial care." However, the quality of care for the general medical problems gradually improved with th

Essential tremor (ET) is a disabling movement disorder that is most prevalent among the elderly. While deep brain stimulation surgery targeting the ventral intermediate nucleus of the thalamus is commonly used to treat ET, the most elderly patients or those with multiple medical comorbidities may not qualify as surgical candidates. Magnetic resonance-guided focused ultrasound (MRgFUS) constitutes a less invasive modality that may be used to perform thalamotomy without the need for a burr hole craniotomy. Here, we report on 2 patients over the age of 90 years who benefited significantly from MR

BACKGROUND AND IMPORTANCE: The ventral intermediate nucleus of the thalamus is a primary target of deep brain stimulation (DBS) in patients with essential tremor. Despite reliable control of contralateral tremor, there is sometimes a need for lead revision in cases of infection, hardware malfunction, or failure to relieve symptoms. Here, we present the case of a patient undergoing revision after ventral intermediate nucleus (Vim) DBS failed to control his tremor. During the electrode removal, the distal portion of the lead was found to be tightly adherent to tissue within the deep brain. Parti
