High Complication Rate after Introduction of Transbronchial Cryobiopsy into Clinical Practice at an Academic Medical Center.

RATIONALE: Transbronchial cryobiopsy is an emerging technique for obtaining biopsies of lung parenchyma. Despite limited evidence of its safety and efficacy in direct comparison with other available biopsy procedures, pulmonologists are integrating this technique into clinical practice with the hope of avoiding the risks of surgical lung biopsy. OBJECTIVES: To report the rate of severe complications and diagnostic outcomes immediately after introduction of transbronchial cryobiopsy into the clinical practice of a single-center, high-volume, interventional pulmonary group at a large academic m

Feasibility and safety of outpatient medical thoracoscopy at a large tertiary medical center: a collaborative medical-surgical initiative.

BACKGROUND: Medical thoracoscopy (MT) is performed by relatively few pulmonologists in the United States. Recognizing that an outpatient minimally invasive procedure such as MT could provide a suitable alternative to hospitalization and surgery in patients with undiagnosed exudative pleural effusions, we initiated the Mayo Clinic outpatient MT program and herein report preliminary data on safety, feasibility, and outcomes. METHODS: All consecutive patients referred for outpatient MT from October 2011 to August 2013 were included in this study. Demographic, radiographic, procedural, and histol

Aerodigestive Considerations in Children with Neuromuscular Disorders.

Aerodigestive disorders affect the upper airway, respiratory and gastrointestinal system. These disorders are often related and pose a diagnostic and therapeutic challenge for patients with neuromuscular disorders. Ideally, this patient population should have a multidisciplinary team that includes a gastroenterologist, otolaryngologist, pulmonologist, and speech therapist to both diagnose and treat these aerodigestive conditions. Over time, these disorders can cause significant morbidity and mortality, but early treatment can improve quality of life.

Respiratory protection.

Respiratory protection is used as a method of protecting individuals from inhaling harmful airborne contaminants and in some cases to supply them with breathable air in oxygen-deficient environments. This article focuses on the use and types of personal respiratory protection (respirators) worn by individuals at workplaces where airborne hazardous contaminants may exist. Respirators are increasingly also being used in nonindustrial settings such as health care facilities, as concerns regarding infectious epidemics and terrorist threats grow. Pulmonologists and other clinicians should understan

International survey of physician recommendation for tracheostomy for Spinal Muscular Atrophy Type I.

The ethics of invasive mechanical ventilation for children with the neurodegenerative disease Spinal Muscular Atrophy Type I (SMA I) is highly debated, and wide variability in clinical outcomes exists internationally. We conducted this international survey to identify physician characteristics associated with recommendation for tracheostomy and ventilation for SMA I. A cross-sectional online survey was distributed to 1,772 pediatric pulmonologists and pediatric intensivists from online membership directories of American Thoracic Society, American College of Chest Physicians, and European Respi

Endobronchial ultrasound.

Complex technical problems interfered with the application of thoracic ultrasound (US) for studies and clinical research. Moreover, in contrast to radiologists, cardiologists, gastroenterologists, internists, obstetricians, gynecologists and others, pulmonologists were not trained in the basics of US images. However, endoscopic US methods were developed in the last 20 years and these methods also provided important results for pulmonologists. As soon as the technical problems interfering with US application in air-containing spaces were solved, endobronchial US (EBUS) became a valuable techniq

Tracheostomy in children: a narrative review.

Tracheostomy, a surgical procedure creating an artificial airway in the trachea, has evolved significantly since 1999 in pediatric care, driven by technological advancements and a deeper understanding of pediatric respiratory physiology. While once reserved for emergency situations or as a last resort for airway obstruction, its role has expanded to encompass long-term respiratory support, chronic aspiration management, and improved quality of life for children with complex medical conditions. The aim of this review, although not systematic, is to provide insight for pediatric surgeons, pulmon

The Usefulness of a Transbronchial Lung Cryobiopsy for Diffuse Bronchiolitis.

We herein report four cases of diffuse bronchiolitis proven by a transbronchial lung cryobiopsy (TBLC). Based on various aspects, including the pathological findings, we definitively diagnosed two patients with diffuse panbronchiolitis (DPB) and the other two with primary ciliary dyskinesia (PCD). One of the DPB patients had more severe peribronchiolar fibrosis than the other, and the disease course was refractory to macrolide therapy. One of the PCD patients was additionally diagnosed with combined constrictive bronchiolitis. This report highlights the importance of a TBLC in the differentiat

Persistent focal pulmonary opacity elucidated by transbronchial cryobiopsy: a case for larger biopsies.

Persistent pulmonary opacities associated with respiratory symptoms that progress despite medical treatment present a diagnostic dilemma for pulmonologists. We describe the case of a 37-year-old woman presenting with progressive fatigue, shortness of breath, and weight loss over six months with a progressively worsening right basilar infiltrate on chest imaging in spite of antibacterial therapy. Transbronchial cryobiopsy was used to establish the diagnosis after a bronchoscopy with traditional forceps biopsies was non-diagnostic. This case demonstrates the value of cryobiopsy as a second-line

Complications of Lung Transplantation: A Roentgenographic Perspective.

Lung transplantation is now an established treatment for a broad spectrum of end-stage pulmonary diseases. According to the International Society for Heart and Lung Transplantation Registry, more than 50,000 lung transplants have been performed worldwide, with nearly 11,000 lung transplant recipients alive in the United States. With the increasing application of lung transplantation, pulmonologists must be cognizant of common complications unique to the postlung transplant period and the associated radiologic findings. The aim of this review is to describe clinical manifestations and prototypi

Treatment of critical status asthmaticus in children.

Status asthmaticus is complex in its etiology and pathophysiology and may be associated with significant morbidity and mortality. Although there are many therapeutic options, specific inhaled beta 2-agonists, corticosteroids, and oxygen remain the mainstay of therapy. Several new drugs and some older drugs are being used in management; their exact role in treatment at present, however, relies largely on personal preferences. Innovative methods of providing ventilatory support are also emerging. What is quite clear is the fact that involvement of specialists (pulmonologists and intensivists) ea

[Pulmonary involvement in systemic diseases : Possibilities and limitations of radiology].

The diagnosis of pulmonary involvement in systemic diseases presents a major challenge. This challenge can only be met through close interdisciplinary collaboration and by discussing all available examination results, including radiological findings, in a comprehensive and integrative manner. The role of the radiologist is to possess in-depth knowledge of systemic diseases and their pulmonary manifestations. The radiological report should not be limited to a mere description of the morphological pattern. In appropriate contexts, the possibility of systemic disease should be indicated, and, whe
