Point-of-Care Ultrasound Identifies Pyomyositis Secondary to Intramuscular Testosterone Injection: Report of Two Cases.

BACKGROUND: Intramuscular (i.m.) injections are a commonly utilized route for medication delivery. Intramuscular-associated soft tissue infections are rare and can include pyomyositis and i.m. abscess. Intramuscular testosterone injections have not been previously implicated in causing pyomyositis. Point-of-care ultrasound is an important bedside tool that can identify pyomyositis and differentiate this infection from more common entities such as cellulitis. CASE REPORTS: We present two cases of i.m. testosterone-associated pyomyositis. In both cases, the physical examination features were co

Shoulder Pain after Fall, Septic Shock, and Pyomyositis Associated with Breast Cancer Chemotherapy and Lymphedema.

BACKGROUND: As a symptom of pyomyositis, sepsis usually follows local inflammation signs. Here, we report pyomyositis with lymphedema of upper extremity in which septic shock and poor local findings initially presented during chemotherapy for breast cancer. CASE REPORT: An 80-year-old woman presented with chronic right shoulder pain during chemotherapy for the recurrent disease. She had a history of postmastectomy lymphedema, diabetes mellitus, and repeated hyaluronic acid injections to the shoulder joint. The pain suddenly worsened with septic shock and no apparent local signs. Magnetic reso

Primary Pyomyositis of the Adductor and Pectineus Muscle in Patients With Uncontrolled Diabetes: Report of Two Cases.

Muscle amyotrophy and infarction are known to be rare complications of patients with diabetes mellitus and poor glycemic control. However, primary pyomyositis, an especially rare infection of the primary muscles, is also a problem that cannot be ignored. Primary pyomyositis typically begins as a subacute condition and is probably due to transient bacteremia. Here, we report on two patients with Type 2 diabetes mellitus and poor glycemic control who developed primary pyomyositis, which manifested as abscesses of the adductor and pectineus muscles. Both patients underwent surgical drainage, and

Diabetic myonecrosis complicated by emphysematous pyomyositis and abscess caused by Escherichia coli: a case report.

BACKGROUND: Necrotizing myopathies and muscle necrosis can be caused by immune-mediated mechanisms, drugs, ischemia, and infections, and differential diagnosis may be challenging. CASE PRESENTATION: We describe a case of diabetic myonecrosis complicated by pyomyositis and abscess caused by Escherichia coli. A white woman in her late forties was admitted to the hospital with a 1.5 week history of bilateral swelling, weakness, and mild pain of the lower extremities and inability to walk. She had a history of type 1 diabetes complicated by diabetic retinopathy, neuropathy, nephropathy, and end-s

Staphylococcus aureus Bacteremia With Disseminated Multiple Foci and Pyomyositis in an Immunocompetent Patient: A Case Report.

Pyomyositis is an uncommon primary infection of skeletal muscle resulting in muscle inflammation followed by pus formation. Pyomyositis is typically caused by Staphylococcus aureus (S. aureus), and most cases are associated with skin penetration and/or immunosuppressive conditions in tropical or even temperate climates. We report a previously healthy, immunocompetent 44-year-old man who presented with fever and right lower back pain. He had received an analgesic injection for his back pain 12 days prior to this visit. His clinical course was further complicated by the coexistence of multiple m

A Case of Multifocal Primary Pyomyositis Complicated With Toxic Shock Syndrome in a Non-tropical Region.

Primary pyomyositis, also known as tropical pyomyositis, is a primary bacterial infection of skeletal muscle following hematogenous infections. It is primarily caused by Staphylococcus aureus or Group A Streptococcus and predominantly affects children and young adults. Although rarely observed in temperate climates, its prevalence appears to be increasing. Here, we present the case of a 36-year-old male patient who manifested with persistent fever and inflammatory signs in multiple skeletal muscle locations following acute pharyngitis, further complicated by toxic shock syndrome within 48 h of

Pyomyositis Affecting the Paraspinal and Iliacus Muscles in a Patient With Staphylococcus aureus Urinary Tract Infection.

We report a case of a 63-year-old man who presented with a four-day history of fever, night sweats, and left lower back pain, which radiated down to his left buttock and leg. He also had a short-lived episode of chest pain and breathlessness. He had a medical history of chronic back pain, which had been diagnosed as sciatica three years ago. Initial investigations revealed raised inflammatory markers due to a Staphylococcus aureus urinary tract infection. Despite treatment, his inflammatory markers did not improve and the left lower back pain persisted. A magnetic resonance imaging scan demons

Pyomyositis and muscle abscess due to Salmonella enteritidis in a patient with Behçet's syndrome: a case report.

Salmonella infections usually present with gastrointestinal manifestations including enterocolitis especially in immunocompromised patients. Haematogenous dissemination and abscesses are very rare complications of Salmonella species. This case report documents a patient with Behçet's syndrome (BS) who has pyomyositis due to Salmonella species. A 43-year-old male patient with BS presented to the outpatient rheumatology clinic with bilateral acute-onset lower extremity pain. However, over a short time the pain gradually increased and was accompanied by fever. The magnetic resonance scans demonst

[Pyomyositis due to Streptococcus pyogenes in a patient with venolymphatic malformation. Report of one pediatric case].

Streptococcus pyogenes infections are common in the pediatric population in the form of tonsillopharyngitis or cutaneous disease. Less frequently, it presents with invasive forms such as pyomyositis or necrotizing fasciitis. Mortality in children is of 7 %. We present an 11-year-old patient with a subscapular venolymphatic malformation who consulted for febrile syndrome associated with pain and an acute increase in the size of the lesion. During the hospitalization, diagnosis of pyomyositis due to Streptococcus pyogenes was reached.

Fusobacterium emphysematous pyomyositis with necrotizing fasciitis of the leg presenting as compartment syndrome: a case report.

BACKGROUND: Fusobacterium necrophorum is a common agent of disease in humans, but the occurrence of primary infections outside the head and neck area is extremely rare. While infection with Fusobacterium necrophorum has a rather benign course above the thorax, the organism is capable of producing very severe disease when located in unusual sites, including various forms of septic thrombophlebitis. No infections of the leg have been documented before; thus, antibiotic coverage for Fusobacterium is currently not recommended in this area. CASE PRESENTATION: A 50-year-old homeless African-America

Pyomyositis of the obturator internus muscle extending to septic arthritis of the hip in a child: a case report.

We report a case of primary pyomyositis in the obturator internus muscle. Pyomyositis involving muscles around the hip needs to be differentiated from septic arthritis because these infections show similar symptoms. Management with antibiotics can avoid the need for surgical intervention. Uncontrolled pyomyosistis can cause sequelae such as septic shock, osteomyelitis of adjacent bone, and septic arthritis. Awareness of this condition will facilitate correct diagnosis and early treatment.

Pyomyositis of the iliacus muscle and pyogenic sacroiliitis after sacroiliac joint block -A case report-.

Sacroiliac joint block can be performed for the diagnosis and treatment of sacroiliac joint dysfunction. Although sacroiliac joint block is a common procedure, complications have not been reported in detail. We report a case of iliacus pyomyositis and sacroiliac joint infection following a sacroiliac joint block. A 70-year-old female patient received sacroiliac joint blocks to relieve pelvic pain. The patient was admitted to the emergency room two days after the final sacroiliac joint block (SIJB) with the chief complaints of left pelvic pain corresponding to a visual analogue scale (VAS) scor

[Pyomyositis of the M. quadriceps femoris in an 8-month-old boy following pneumococcal meningitis].

An 8-month-old boy developed pyomyositis of the M. quadriceps femoris following meningitis caused by Streptococcus pneumoniae. The child had no underlying immune defect. Conservative treatment with antibiotics led to complete clinical and radiological resolution of the infection. In Europe, pyomyositis is rarely seen in children. Pyomyositis should be considered in the differential diagnosis in children with fever and complaints of joint pain or myalgia. Ultrasound and MRI are helpful techniques for establishing the diagnosis and for differentiating pyomyositis from other pathological conditio

Physical signs in pyomyositis presenting as a painful hip in children: a case report and review of the literature.

The incidence of pyomyositis is on the increase worldwide among children and adults. When it involves muscles about the hip it needs to be differentiated from septic arthritis and transient synovitis among other diseases. Lack of awareness of the condition frequently leads to delay in correct diagnosis. We present one case of pyomyositis of the gluteus maximus and another rarer case of obturator internus pyomyositis that posed difficulties in initial diagnosis. While magnetic resonance imaging remains the imaging method of choice, we highlight important physical signs in the examination of pat

Pyomyositis presenting as myonecrosis secondary to methicillin-resistant Staphylococcus aureus bacteremia in chronic lymphocytic leukemia.

The incidence of pyomyositis in immunocompromised patients with HIV, diabetes, myelodysplastic syndromes, and acute lymphocytic leukemia is well documented. However, there are only a few reports of pyomyositis and myonecrosis in patients with chronic lymphocytic leukemia (CLL). We present a rare case of pyomyositis presenting as myonecrosis secondary to methicillin-resistant Staphylococcus aureus bacteremia in a 72-year-old patient with CLL. Pyomyositis, although rare, warrants increased provider awareness and management, especially among CLL patients who pose diagnostic and treatment challeng

Pyomyositis in Children: A 15-year Retrospective Study From a Tertiary Care Pediatric Hospital in Portugal.

BACKGROUND: Pyomyositis is a primary infection of the skeletal muscle, commonly occurring in tropical countries in children. Treatment is still controversial, with antibiotics plus drainage being the main option. METHODS: Retrospective, unicentric study of pyomyositis in a tertiary care Pediatric Hospital, from January 2008 to December 2022. Predisposing factors, clinical, laboratory and radiological results, treatment and evolution were analyzed. RESULTS: Twenty-nine patients were identified, 75.9% male, median age of 8 years old (0.8;17). The most common predisposing factors were intense p

Pyomyositis Secondary to Localized Cellulitis in a Dermatomyositis Patient: A Case Report and Review of Infectious Complications in Dermatomyositis.

Dermatomyositis (DM) is an autoimmune disorder characterized by proximal muscle weakness and distinct cutaneous features. Unfortunately, infection is a frequent and potentially life-threatening complication in patients with DM. Here, we present a case of pyomyositis in a patient with DM resulting from localized cellulitis. The patient also presented with subcutaneous calcification nodules and dermatomyositis-associated lipodermatosclerosis nodules. To our knowledge, there have been no reports of pyomyositis in patients with DM to date. Furthermore, we reviewed the infectious complications rela
