BACKGROUND: Time toxicity refers to the considerable time investment required by patients undergoing cancer treatment, including travel, waiting periods and treatment duration. It is increasingly recognised not only as a logistical burden but also as a psychological stressor, significantly affecting patient well-being.
OBJECTIVES: This study surveyed oncologists across Pakistan to assess their understanding of time toxicity and its impact on both patients and clinical practice.
METHODS: From August to October 2023, we conducted a cross-sectional study targeting a diverse group of cancer care

Prostate, bladder and kidney neoplasms are among the most prevalent genitourinary (GU) cancers worldwide. Significant therapeutic advancements in recent years have substantially improved patient outcomes. In response to this rapid progress, the Santiago de Compostela Health Research Institute (IDIS) has organized the annual 'Cambados Consensus Forum on Genitourinary Tumors' (Pontevedra, Spain) since 2018. This 2-day multidisciplinary meeting gathers Spanish medical oncologists, radiation oncologists, urologists, and hospital pharmacists to present and discuss the latest evidence in the field,

Spinal tumors pose significant treatment challenges for the physicians treating them. Understanding the location of the tumor within the intramedullary, intradural extramedullary, or extradural (epidural) space is not only critical in determining a differential diagnosis but may also provide important information about current and future neurologic deficits. Despite significant advances in the treatment of spinal tumors over the past few decades, these patients may still experience significant symptoms related to the tumor or its treatment, such as pain, weakness, impaired sensation, and bowel

Radiation oncologists treating patients with primary brain tumors are faced with two important issues: How to improve the cure rate over that which is achievable with the current radiotherapy modalities and how to decrease long-term morbidity while maintaining or improving the cure rate. Several new approaches are being studied, including interstitial implants, stereotactic radiosurgery, new radiation modalities, radiosensitizers, hyperthermia, and altered fractionation programs. It is nonetheless important to remember that such innovative radiotherapy must be incorporated into the overall mul

Radiation therapy, using both external beam and brachytherapy, is one of the mainstays of treatment for oral cavity cancers. For early lip, tongue, and floor of mouth lesions, radiation alone is highly effective and produces an excellent functional result. More advanced lesions are frequently treated with combined therapy or radiation alone with surgical salvage. Buccal mucosa and gingiva lesions are generally treated with surgery but can be managed by radiation therapy if they are early. More advanced lesions are treated by surgery plus postoperative radiation. Retromolar trigone lesions can

Radiation therapy often plays a critical role in the treatment of thymic malignancies. However, because of the location of these tumors, historically patients have been at a significant risk for radiation-related toxicity such as pericardial effusions, radiation pneumonitis, long-term pulmonary fibrosis, and occasional long-term esophageal stricture, particularly for unresectable thymoma. Recent advancements in technology have provided the treating radiation oncologist with the ability to more accurately target the region at risk while sparing normal structures. In this review, we provide an o

OBJECTIVES: To demonstrate that novice dosimetry planners efficiently create clinically acceptable IMRT plans for head and neck cancer (HNC) patients using a commercially available multicriteria optimization (MCO) system.
METHODS: Twenty HNC patients were enrolled in this in-silico comparative planning study. Per patient, novice planners with less experience in dosimetry planning created an IMRT plan using an MCO system (RayStation). Furthermore, a conventionally planned clinical IMRT plan was available (Pinnacle(3)). All conventional IMRT and MCO-plans were blind-rated by two expert radiatio

Esophageal large-cell neuroendocrine carcinoma (LCNEC) is a rare and aggressive malignancy that presents diagnostic and therapeutic challenges. We present the case of a 72-year-old female with LCNEC of the distal esophagus who was initially treated with chemotherapy and radiation therapy, followed by immunotherapy due to disease progression. Diagnostic imaging revealed extensive metastases to the abdominal lymph nodes. Histopathological examination confirmed the diagnosis of LCNEC with high-grade squamous intraepithelial neoplasia. The treatment complexities underscore the necessity of a multi

OBJECTIVES: This study evaluates the interobserver variation in parotid gland delineation and its impact on intensity-modulated radiotherapy (IMRT) solutions.
METHODS: The CT volumetric data sets of 10 patients with oropharyngeal squamous cell carcinoma who had been treated with parotid-sparing IMRT were used. Four radiation oncologists and three radiologists delineated the parotid gland that had been spared using IMRT. The dose-volume histogram (DVH) for each study contour was calculated using the IMRT plan actually delivered for that patient. This was compared with the original DVH obtained

The standard of care for transitional-cell carcinoma of the bladder with invasion to the muscularis propria is radical cystectomy with bilateral pelvic lymph node dissection. However, currently there is a tendency for organ preservation in selected cases of muscle-invasive bladder cancer. Trimodality treatment, including transurethral resection of the bladder tumor (TURBT), radiation therapy and chemotherapy, has been shown to produce 5-year and 10-year overall survival rates comparable to those of radical cystectomy. The current 5-year overall survival rates range from 50 to 67% with trimodal

BACKGROUND: Radiation dermatitis occurs to some degree in most patients receiving radiotherapy, with or without chemotherapy. Patients with squamous cell carcinoma of the head and neck (SCCHN) who receive radiotherapy in combination with epidermal growth factor receptor (EGFR) inhibitors, such as cetuximab, may develop a characteristic acne-like rash in addition to dermatitis.
DESIGN: An advisory board of 11 experienced radiation oncologists, medical oncologists and dermatologists discussed the management options for skin reactions in patients receiving EGFR inhibitors and radiotherapy for SC

Objective. We present a successful multidisciplinary treatment approach used for a patient with rapidly developing, high-volume, and high-risk squamous cell carcinomas (SCCs), addressing short-term treatment, long-term maintenance, and appropriate preventative strategies in a difficult patient population. Case report. An immunocompetent, 84-year-old, Caucasian man with at least 30 SCCs of the scalp, head, neck, and upper extremities, including a 4-cm SCC on the vertex of the scalp infiltrating to the periosteum, presented to a cutaneous oncology clinic. Initial physical examination revealed ne

Vaginal vault brachytherapy is a common treatment for endometrial cancer. Historically, applicator insertion has been the domain of a radiation oncologist (RO). This commentary outlines a project to improve efficiency and workforce utilisation by introducing a competency framework and training module allowing entitled radiation therapists to perform single-channel cylinder applicator insertions and treatment delivery under RO supervision for fraction one and without supervision for subsequent fractions. The rationale, relevant regulations, implementation process and barriers are explored.

Malignancies with a superficial involvement of the scalp present a unique technical challenge for radiation treatment planning. As an example of this, leukemic infiltration of the superficial skin as the only presentation of the disease is a rare entity. For such cases, radiation oncologists have typically treated with 3D conformal radiotherapy with matched electron fields, a technique that can lead to significant dose inhomogeneity. In this report, we describe the case of a patient with leukemia cutis with a superficial involvement of the scalp, face, and shoulders that was treated with volum

Nowadays, more than ever before, the treatment of cancer patients requires an interdisciplinary approach. Radiation therapy (RT) has become an indispensable pillar of cancer treatment early on, offering a local, curative treatment option, and symptom control in palliative cases. Due to technological advances in the last 30 years, the precision in RT planning has greatly improved, allowing the radiation oncologist to prescribe high radiation doses and thereby assuring high local tumor control while sparing healthy tissue, and thereby reducing side effects. One of the most important developments

In patients with known or suspected lung cancer, pathologic reports of cytologic "atypia" in the mediastinal nodal fine-needle aspirate samples pose a diagnostic and therapeutic dilemma. For the patents with non-operable disease, should the radiation oncologist empirically include lymph nodes with atypical findings in the radiation field, or should they exclude these mediastinal lymph nodes from the treatment field? Inclusion of an atypical finding in the management decision or radiation target would mean that the treating physicians are interpreting a pathologic report of "atypia" as probably
