Radical Resection of a Giant Facial Plexiform Neurofibroma in a Resource-Limited Setting.

Giant facial plexiform neurofibromas (PNFs) in neurofibromatosis type 1 are surgically challenging, particularly in low- and middle-income countries where patients may present late and repeated staged procedures may be impractical. We report a 20-year-old woman from rural Zimbabwe with a massive recurrent left facial PNF after three prior debulking procedures. She underwent single-stage radical excision of involved skin, mucosa, parotid tissue, and facial nerve branches, followed by immediate anterolateral thigh free flap reconstruction and static facial suspension. Two units of packed red blo

Cardiac Rehabilitation in Advanced Dilated Cardiomyopathy Within a Resource-Limited Setting.

BACKGROUND: Dilated cardiomyopathy (DCM) is a major cause of heart failure with reduced ejection fraction. Cardiac rehabilitation (CR) has been shown to improve functional capacity, but its use in advanced DCM remains limited. CASE SUMMARY: A 57-year-old man with idiopathic DCM and severe systolic dysfunction (left ventricular ejection fraction = 5%) presented with acute heart failure. Cardiac magnetic resonance revealed inferoseptal fibrosis and biventricular dilation. He was treated with optimal pharmacologic therapy and an 8-week supervised CR program. The 6-minute walk test improved by 11

The effect of allogeneic blood transfusion on burn mortality in a resource-limited setting.

INTRODUCTION: There are nearly 9 million new burn cases worldwide, with a disproportional burn burden in low- and middle-income countries. Patients with significant burn injury frequently require multiple blood transfusions; however, there is a paucity of data regarding the effect of allogeneic blood transfusion following burn injury in a resource-limited setting with a high anemia prevalence at baseline. This study aimed to determine the effect of blood transfusion on burn mortality. METHODS: We performed a retrospective review of patients presenting with burns between 2011 and 2019, using p

Challenges and perspectives of abdominal pregnancy in a resource-limited setting: A case series.

INTRODUCTION AND IMPORTANCE: Abdominal pregnancies, although rare, present unique challenges in both diagnosis and management. This case series reports on four cases of abdominal pregnancies diagnosed in a remote and underserved region of the Democratic Republic of Congo, highlighting the difficulties encountered in an under-equipped environment. CASE PRESENTATION: The cases were documented in a remote village of DRC, which lacks specialized medical personnel and relies heavily on traditional healers. We report here 4 cases of abdominal pregnancies, their clinical presentations, diagnostic mo

Modelling bodyweight to avoid anthelmintic underdosing of goats in resources-limited settings.

Improved management of livestock in resource-limited settings can provide a means towards improved human nutrition and livelihoods. However, gastrointestinal nematodes (GIN) are a significant production-limiting factor. Anthelmintics play a role in GIN management; however, few anthelmintic classes are available in many low-middle-income countries. Utilising a limited range of classes may increase selection for anthelmintic resistance; therefore, strategies to reduce other selective pressures are of heightened importance. Avoiding anthelmintic underdosing is one such strategy, but it can be cha

Diagnosing Cellulitis of the Penis with Point-of-Care Ultrasonography in a Resource-Limited Setting.

Cellulitis is a potentially serious bacterial skin infection. Penile cellulitis refers to the inflammation of the penile shaft and commonly occurs in uncircumcised, sexually active young adults. We reported the case of a 25-year-old heterosexual circumcised male patient with a two-day history of swelling and pain over the penile shaft. Local examination revealed a diffusely swollen penile shaft, erythematous, warm to the touch, and tender. The penile discharge culture was suggestive of a Streptococcus species infection. Ultrasonography of the penis showed increased echogenicity of the left sid

Experiences of rural nurses with emergency patient transport in a resource limited setting.

BACKGROUND: Registered nurses are primary care providers during most patient transfers from rural areas. Various local conditions and circumstances impact the provision of nursing care prior to and during transportation. These include clinic staffing, uneven access to functioning equipment and other necessary infrastructure across settings, the wide-ranging clinical need for specialty care, and complex social and interpersonal circumstances that play a role in care-seeking and transport decision-making. This study explored the experiences of nurses with emergency patient transport in rural hea

Characteristics of inpatients in dialectical behaviour therapy modified for a resource-limited setting.

BACKGROUND: Emotional dysregulation in psychiatric disorders contributes to morbidity, mortality and healthcare costs. Dialectical behaviour therapy (DBT) is effective in addressing this, but is complex and costly to implement. Recent literature indicates that DBT can be modified for use in resource-limited settings, but little is known about its implementation in African settings. AIM: To describe the demographic and clinical characteristics of participants in a modified DBT-ST (skills training) programme at a South African psychiatric hospital. SETTING: The study was conducted at Stikland

Liver abscess in children - Clinical profile and outcome in a resource-limited setting.

BACKGROUND: Liver abscess continues to be a major cause of morbidity in developing countries. There is no gold standard for management; it has to be tailor made for each child depending on availability of resources. We aimed to study clinical features, laboratory parameters, treatment, and outcome of children with liver abscess in resource-limited settings. MATERIALS AND METHODS: This is a retrospective observational cohort study of children less than 16 years admitted in pediatric ward with diagnosis of liver abscess during 4 years duration (2016-2019). Simplified intraoperative parathormone assay for primary hyperparathyroidism in a resource-limited setting.

Background: The optimum threshold of IOPTH decay remains a debate and numerous criteria have been described. In this study, we utilize a single-sample IOPTH taken 10 min post excision. Materials & methods: This 4-year query of a prospectively maintained database included primary hyperparathyroidism patients with pre-operative PTH done 1 week prior to surgery, and a 10-min post excision IOPTH value. Optimal cut-off for PTH and sensitivity/specificity were calculated. Results: A total of 93 patients had single-gland disease, of whom 79 (84.9%) were symptomatic. The 10-min post excision assay sen

Common presentation of an uncommon anaemia in a resource-limited setting: A diagnostic challenge.

A 38-year-old female in the reproductive age group presented with anaemia in a resource-poor setting. The patient was given multiple transfusions during the course of her treatment. After a proper work-up she was diagnosed with Coomb's negative autoimmune haemolytic anaemia (AIHA). She was later treated with steroid and she showed significant improvement. Early diagnosis in her case at the primary care level with other causes of anaemia in mind could have prevented unnecessary transfusions and morbidity. Proper history and approach to anaemia is the key in a resource-poor setting.

Comparison of clinical prediction rules for management of pharyngitis in settings with limited resources.

OBJECTIVE: To compare the effectiveness of several clinical prediction rules for culture-positive streptococcal pharyngitis in a single group of patients in a setting in which clinicians routinely treat all cases of pharyngitis presumptively, without laboratory data. STUDY DESIGN: A MEDLINE search identified clinical prediction rules for streptococcal pharyngitis in children. Each rule was applied analytically to data from 410 children in Cairo, Egypt with clinical pharyngitis, in whom throat cultures were performed. The diagnostic effectiveness of these rules for predicting a positive cultur

Hypoinsulinemic hypoglycemia with hemihypertrophy in a Filipino infant: management in a resource-limited setting.

Persistent infantile hypoglycemia is a medical emergency associated with significant risk of neurological injury. Although most commonly attributed to congenital hyperinsulinism, disorders of impaired insulin signaling with hypoketotic hypoglycemia despite low or undetectable insulin occur in fewer than 1 in 1 000 000 infants worldwide, leading to misdiagnosis and ineffective therapy. We report a Filipino infant with recurrent fasting hypoglycemic seizures and hemihypertrophy. Biochemical evaluation showed absent ketones with low to mildly detectable insulin, suggesting dysregulated insulin si
