Adaptive time-frequency decomposition informer for pathological rest tremor sequence prediction.

Pathological tremor is a common symptom of various neurological disorders. Pathological tremor signal prediction is important to the tremor suppression equipment. However, how to improve the accuracy of multi-step prediction of tremor motion is a difficult problem. In this paper, an adaptive time-frequency decomposition informer (ATFDI) method is proposed to predict the signal of pathological tremor. For this method, an adaptive oscillator is used to model the tremor signal, and the tremor signal is decomposed into multiple sub-signals with narrow band and single main frequency. After eliminat

Kinetic-rest tremor dominance for differentiating dystonic tremor from Parkinson's disease.

BackgroundDifferentiating dystonic tremor (DT) from Parkinson's disease (PD) remains challenging, particularly when dystonic signs are subtle, and DT is a major contributor to scans without evidence of dopaminergic deficit. Although dopamine transporter imaging can support the distinction, it is costly and not universally accessible. Simple diagnostic indicators less reliant on subjective clinical interpretation are needed.MethodsIn a dopamine transporter imaging-supported cohort (n = 102; DT, 51; PD, 51), we used accelerometry to characterize ordinal patterns of tremor amplitude across rest,

EEG Spectral Analysis in Chronic Pain During Rest and Cognitive Reasoning.

Chronic pain (CP) represents a multidimensional condition in which cognitive and emotional factors shape the individual experience from perception to action. The purpose of this study was to characterize the functional significance of alterations in neural oscillatory dynamics underlying the transition from resting-state to cognitive load across distinct CP phenotypes. Continuous electroencephalographic data were acquired from patients with headache, migraine, and spine-related pain, as well as healthy controls, during rest and three visual-cognitive-motor (VCM) tasks: reaction time, working m

Adventitial Cystic Disease of the Popliteal Artery with Nocturnal Rest Pain.

Arterial cystic disease (ACD) affecting the popliteal artery (PA) is a rare form of non-atherosclerotic vascular disease. This cystic tumor is defined by the accumulation of a mucinous substance in the adventitia. Treatment options include percutaneous cyst aspiration, percutaneous transluminal balloon angioplasty, the evacuation of the cyst through a surgical approach, and resection of the affected artery segment followed by arterial reconstruction using autologous venous or prosthetic grafting. Our hospital received a 36-year-old man who had an intermittent claudication and periodically noct

Lumbosacral spinal cord functional connectivity at rest: From feasibility to reliability.

In the past decade, exploration of spontaneous blood-oxygen-level-dependent (BOLD) signal fluctuations has expanded beyond the brain to include the spinal cord. While most studies have predominantly focused on the cervical region, the lumbosacral segments play a crucial role in motor control and sensory processing of the lower limbs. Addressing this gap, the aims of the current study were twofold: first, confirming the presence and nature of organized spontaneous BOLD signals in the human lumbosacral spinal cord; second, systematically assessing the impact of various denoising strategies on si

Reverse endothelial scraping technique (REST) for an infectious corneal endothelial plaque.

A male patient in his 20s, suffering from a persistent, infection-related corneal endothelial plaque (EP) was urgently referred to our tertiary medical centre for therapeutic penetrating keratoplasty (TPK). Over the preceding month, he had been undergoing treatment with both topical and oral antifungal medications due to clinical suspicion of fungal keratitis. At our centre, an endothelial scraping was performed using a reverse Sinskey hook to obtain samples for microbiology and revealed septate branching fungal hyphae. After targeted therapy with intracameral and intrastromal voriconazole inj

Personality matters in extremely demanding environments: A bed rest performance study.

INTRODUCTION: Personality is a rather neglected aspect in bed rest studies. The aim of the study was to clarify which specific personality pattern may predict the performance of bed rest study participants. MATERIALS AND METHODS: Personality traits were correlated with participants' performance rated by the team running the study. The sample consisted of N = 68 participants who took part in one of four different studies. A broad set of personality traits correlated with different performance aspects (stability, perseverance, modesty, flexibility, compliance, likability, social adaptation). R

Fetuses with deceleration of growth improve their growth following maternal rest.

OBJECTIVE: The purpose of this study was to determine if fetuses with deceleration of growth velocity resulting in an EFW <10th percentile increase their growth above the 10th percentile following 2 weeks of maternal rest in the left lateral recumbent position. METHODS: This was a retrospective observational study of 265 fetuses with the prenatal diagnosis of an EFW <10th percentile. Fetuses were classified by four definitions of abnormal growth velocity: (1) a growth velocity less than 20 g/day, (2) 30 percentile decrease in the EFW, (3) 50 percentile decrease in the EFW, and (4) abnormal gr

Rest-Activity Rhythm Phenotypes in Adults with Epilepsy and Intellectual Disability.

OBJECTIVE: Sleep and rest-activity rhythms (RARs) are perturbed in many forms of neuropsychiatric illness. In this study, we applied wrist actigraphy to describe the extent of RAR perturbations in adults with epilepsy and intellectual disability ("E+ID"), using a cross-sectional case-control design. We examined whether RAR phenotypes correlated with epilepsy severity, deficits in adaptive function and/or comorbid psychopathology. METHODS: Primary caregivers of E+ID adults provided informed consent during routine ambulatory clinic visits and were asked to complete standardized surveys of overa

Renal transplantation: the last iteration of the rest of the world.

PURPOSE OF REVIEW: Renal transplantation systems across the world aim to achieve an optimal balance between fair access to deceased donor kidney transplants (equity) and efficient use of organs (utility). Kidney allocation systems are measured across a host of metrics, and there is no single agreed upon definition of success, which looks different for each system depending on the desired balance between equity and utility. This article evaluates the United States renal transplantation system's efforts to balance equity and utility while drawing comparisons to other national systems. RECENT FI

Unexpected cardiorespiratory findings postictally and at rest weeks prior to SUDEP.

INTRODUCTION: Mechanisms underlying sudden unexpected death in epilepsy (SUDEP) are unclear, but autonomic disorders are thought to play a critical role. However, those dysfunctions have mainly been reported in the peri-ictal context of generalized tonic-clonic seizures. Here, we explored whether heart rate variability (HRV), heart rate (HR), and breathing rate (BR) changes could be observed perictally during focal seizures with or without impaired awareness as well as interictally to assess the risk of SUDEP. We report the case of a 33-year-old patient with drug-resistant bilateral temporal l

Guardians of Rest? Investigating the gut microbiota in central hypersomnolence disorders.

In recent years, there has been an increased interest in elucidating the influence of the gut microbiota on sleep physiology. The gut microbiota affects the central nervous system by modulating neuronal pathways through the neuroendocrine and immune system, the hypothalamus-pituitary-adrenal axis, and various metabolic pathways. The gut microbiota can also influence circadian rhythms. In this study, we observed the gut microbiota composition of patients suffering from narcolepsy type 1, narcolepsy type 2, and idiopathic hypersomnia. We did not observe any changes in the alpha diversity of the

Genotype of congenital adrenal hyperplasia patients with testicular adrenal rest tumor.

UNLABELLED: Testicular adrenal rest tumor (TART) is one of the important complications that can cause infertility in male patients with congenital adrenal hyperplasia (CAH) and should therefore be diagnosed and treated at an early age. The factors that result in TART in CAH have not been completely understood. The aim of this study is to evaluate the genotype-phenotype correlation in CAH patients with TART. METHOD: Among 230 malepatients with CAH who were followed upwith regular scrotal ultrasonography in 11 different centers in Turkey, 40 patients who developed TARTand whose CAH diagnosis wa

Handling missing data in rest-activity time series measured by actimetry.

A handling procedure of off-wrist episodes in actimetry time series of motor activity is presented using two records (regular vs. irregular sleep-wake cycle and daytime activity) of 14 consecutive days sampled in 1-minute epochs. We generated single missing value (NA) intervals of 1 h, 2 h, 4 h, 6 h, 12 h, and 24 h as well as random NA episodes following probabilistic rules to simulate real-life off-wrist episodes. Then, we replaced these episodes with "zeroes" (i.e., the default of immobility records), mean or median of the remaining 13 days corresponding to the missing bins. Single missing e

Youth with Down syndrome display widespread increased functional connectivity during rest.

Studies of resting-state functional connectivity in young people with Down syndrome (DS) have yielded conflicting results. Some studies have found increased connectivity while others have found a mix of increased and decreased connectivity. No studies have examined whole-brain connectivity at the voxel level in youth with DS during an eyes-open resting-state design. Additionally, no studies have examined the relationship between connectivity and network selectivity in youth with DS. Thus, the current study sought to fill this gap in the literature. Nineteen youth with DS (Mage = 16.5; range 7-

[Testicular adrenal rest tumor in the background of congenital adrenal hyperplasia].

The prevalence of testicular adrenal rest tumours varies in different forms of congenital adrenal hyperplasia. Patients with 21-hydroxilase deficiency usually have bilateral and palpable testicular nodules. Although adrenal rest tumours are well documented in the literature, the diagnosis and management require a multidisciplinary approach: the cooperative work of endocrinologists, urologists, pathologists and radiologists is essential. In the case of an early diagnosis, appropriately increased corticosteroid treatment may reduce the tumour mass. In advanced stages, tumours can lead to irrever

Age-dependent skeletal muscle transcriptome response to bed rest-induced atrophy.

Short-term muscle disuse induces significant muscle loss in older adults and in some reports may be more accelerated with aging. Identifying muscle transcriptional events in response to bed rest may help identify therapeutic targets to offset muscle loss. Therefore, we compared the muscle transcriptome between young and older adults after bed rest and identified candidate targets related to changes in muscle loss. RNA was sequenced (HiSeq, Illumina; DESeq, R) from muscle biopsies obtained from young [ n = 9; 23 yr (SD 3)] and older [ n = 18; 68 yr (SD 6)] adults before and after 5-day bed rest

Breathe: Your Mitochondria Will Do the Rest… If They Are Healthy!

Dysfunctions of the mitochondrial electron transport chain cause severe, currently untreatable, diseases in humans. A new study by Jain et al. (2019) reports increased oxygen levels in the brain of complex-I-deficient mice. Reducing the O2 levels by hypoxia, carbon monoxide, or anemia, improved the clinical phenotype and prolonged the lifespan of the mouse model.

Defects in the Alternative Splicing-Dependent Regulation of REST Cause Deafness.

The DNA-binding protein REST forms complexes with histone deacetylases (HDACs) to repress neuronal genes in non-neuronal cells. In differentiating neurons, REST is downregulated predominantly by transcriptional silencing. Here we report that post-transcriptional inactivation of REST by alternative splicing is required for hearing in humans and mice. We show that, in the mechanosensory hair cells of the mouse ear, regulated alternative splicing of a frameshift-causing exon into the Rest mRNA is essential for the derepression of many neuronal genes. Heterozygous deletion of this alternative exon
