"En bloc" excision of diabetic membranes.

We treated 16 eyes with diabetic tractional retinal detachment with a new method of surgical dissection whereby diabetic preretinal membranes and adherent posterior hyaloid were excised and removed as a single unit. This method differs from previously reported methods of diabetic membrane dissection by utilizing the posterior hyaloid to lift and immobilize the membrane as the membrane is excised with horizontally cutting scissors. The preservation of the posterior hyaloid facilitates the separation of the membrane from the retina. Fibrovascular proliferation was more completely removed and ble

The induction of retinal detachment.

The induction of retinal detachment is dependent upon several factors whose relative importance remains controversial. These factors are retinal breaks, often associated with vitreous traction, inertial forces and changes in chorioretinal adhesion. Retinal breaks may be classified as holes, tears and dialyses. The relation to the vitreous base is an important prognostic indicator. Once a retinal break has occurred, inertial forces disturb the normal vitreo-retinal association. At that point, the development of a clinical detachment also depends upon loosening of the normal chorioretinal adhesi

Localised gamma irradiation and experimental intraocular proliferation.

A controlled study was undertaken to assess the effect of gamma irradiation on post-traumatic intraocular cellular proliferation. A standard perforating injury in the posterior segment of the rabbit eye was used to induce intraocular cellular proliferation and vitreo-retinal membrane formation. The site of injury was irradiated with an ophthalmic Cobalt60 applicator which provided a continuous source of gamma rays. Non-irradiated eyes developed traction retinal detachments associated with post-traumatic vitreo-retinal membranes. Irradiated eyes developed attenuated membranes or atrophic retina

[Direct optic nerve and retinal injury caused by retrobulbar injections].

The authors report on 2 patients with direct damage to the optic nerve and retina: In a 25-year-old deaf-mute patient a peripheral iridectomy was performed after lid and retrobulbar anesthesia. The amaurosis noticed immediately after surgery was caused by direct injury to the optic nerve. A 69-year-old patient with high myopia was treated with retrobulbar steroid injections. The globe was perforated and a series of 4 retinal defects from the periphery to close to the optic nerve head were created. An unhealed retinal detachment developed. The importance of informing the patient preoperatively

Inadvertent intraocular injection of botulinum toxin A.

This report describes an inadvertent perforation of the globe while injecting botulinum toxin A into the medial rectus muscle without electromyography guidance. Botulinum toxin A does not appear to have toxic effects on the human retina. Close observation and necessary measures to treat any retinal breaks and control increased intraocular pressure should be undertaken.

[Optical coherent tomography in diagnosis of peripheral retinal breaks].

The purpose of our study was to develop an optimal method of peripheral retinal breaks visualization and determine the presence of vitreoretinal traction using optical coherence tomography (OCT). A total of 152 patients (176 eyes) with peripheral retinal breaks were assessed, among them 57 men and 95 women, aged from 18 to 76. To visualize retinal periphery we used RTVue-100 optical coherent tomograph (USA). Photoregistration was done with Nikon NF-505 fundus camera (Japan). All patients were examined with Goldmann lens. OCT application enabled us to determine structural features and configura

Retinal detachment as a complication of retrobulbar anesthesia.

The authors report an unusual case of retinal detachment after cataract surgery in a 48-year-old man in which it appeared that the retrobulbar needle passed through the sclera and tracked beneath the retina before perforating the retina superior to the disc. The cause of the detachment was not clearly defined until a depigmented line was observed after reattachment of the retina.

THE RISK OF RETINAL BREAKS OR DETACHMENT IN FELLOW EYE WITH LATTICE DEGENERATION AFTER PRIMARY RHEGMATOGENOUS RETINAL DETACHMENT.

PURPOSE: Lattice poses risks to patients with prior detachments. This study evaluates such patients' likelihood of rhegmatogenous retinal detachments (RRD) and retinal breaks (RB) and assesses the efficacy of prophylactic laser retinopexy and cryopexy. METHODS: Retrospective cohort study using the TriNetX registry. Patients either with or without prior contralateral RRD/RB and also with fellow eye lattice were included. Logistic regression calculated the likelihood of RRD/RB in the fellow eye. The number needed to treat (NNT) was calculated for prophylactic laser retinopexy and cryopexy. RES

Amniotic membrane graft for persistent macular hole following retinal detachment repair in Knobloch syndrome.

We report the case of a 7-year-old girl with Knobloch syndrome who presented with retinal detachment due to a macular hole, which was treated with vitrectomy and silicone oil tamponade. The macular hole persisted, and recurrent silicone oil emulsification required exchanges, with two subsequent vitrectomies. Finally, an amniotic membrane graft was used to seal the macular hole in an effort to avoid silicone oil dependence for retinal attachment. The macular hole was successfully closed, and the retina remained attached 6 months after surgery.

Scleral buckling for retinal separation: with and without encirclement.

Two similar patient populations with previously unoperated, atraumatic, rhegmatogenous retinal separations were surgically treated using the same technique with encircling elements in one series (290 eyes) and without encircling elements in the other (463 eyes). Nine percent of the encircled group remained detached compared with 7.6% in the other group. The reason for ultimate surgical failure in both groups was proliferative vitreoretinopathy. Encirclement offered no advantage with respect to surgical success.

One hundred consecutive pars plana vitrectomies using the vitrophage.

One hundred consecutive eyes underwent vitrectomy with the disposable vitrophage via a pars plana approach. Eyes were divided into two main categories based on opacity location-in the anterior or posterior segment. Patients with posterior segment opacities had diabetic retinopathy, nondiabetic vitreous hemorrhages, retinal detachments associated with vitreous hemorrhage, bands, or membranes, and intraocular foreign bodies or silicone oil. Patients with anterior segment opacities were treated for pupillary membranes resulting from surgical trauma or perforating injuries, and for complications f

An improvement in visual acuity accompanied by the development of RPE tear: a case report.

BACKGROUND: Retinal pigment epithelium (RPE) tear is a well-known complication of RPE detachment and is typically associated with significant visual acuity decline. However, in this case, despite the occurrence of an RPE tear there was an unexpected improvement in visual acuity. CASE PRESENTATION: A 68-year-old male presented with blurred vision in his right eye of a month's duration. Pachychoroid neovasculopathy was diagnosed. After monthly anti-vascular endothelial growth factor injections over a three months period. the choroidal neovascular membrane gradually diminished. While the pigment

Pre- and postoperative OCT features and surgical outcomes of advanced retinitis pigmentosa with macular hole: case series and literature review.

BACKGROUND: Macular hole (MH) is a rare complication of retinitis pigmentosa (RP) and has an adverse impact on residual visual function. At present, the underlying mechanisms are not fully understood and surgical experience is limited. METHODS: We reviewed the medical records and optical coherence tomography (OCT) scans in a cohort of eight eyes of seven RP patients with MH in order to report their OCT features and vitreoretinal surgical prognosis. RESULTS: This study includes four lamellar macular holes (LMHs) and four full-thickness macular holes (FTMHs). Pre-operative OCT revealed other m

Anti-vascular endothelial growth factors in combination with vitrectomy for complications of proliferative diabetic retinopathy.

BACKGROUND: Vitrectomy is an established treatment for the complications of proliferative diabetic retinopathy (PDR). However, a number of complications can occur during and after vitrectomy for PDR. These include bleeding and the creation of retinal holes during surgery, and bleeding, retinal detachment and scar tissue on the retina after surgery. These complications can limit vision, require further surgery and delay recovery. The use of anti-vascular endothelial growth factor (anti-VEGF) agents injected into the eye before surgery has been proposed to reduce the occurrence of these complica
