[Möbius syndrome].

Möbius syndrome is a rare congenital disease characterized by facial and abducens nerve palsy. Children are unable to smile, frown, suck, grimace, blink their eyes, and to move their eyes laterally. The aim of this study was to analyze the diagnosis and treatment of this disease. Maxillofacial examination reveals: facial diplegia, retrognathism, palatine and dental malformations. Möbius syndrome is usually associated with multiple cranial nerve involvement, limb or orofacial malformation, and Poland syndrome. Although the pathogenesis of the syndrome is unclear, a number of mechanisms have bee

Musculature of the soft palate: clinico-anatomic correlations and therapeutic implications in the treatment of cleft palates.

OBJECTIVE: Hypoplasia of the maxilla, often described as a classic sequela to surgical repair of the cleft palate, has been rare in our experience. We believe that our surgical technique, which includes dividing the nasal mucosa and the abnormal muscular insertions at the posterior border of the hard palate, is an important factor in preventing this sequela. METHOD: We compared the anatomy of 12 normal palates in cadavers to the anatomy of cleft palates seen at operation and to the anatomy of one cleft palate in a fetus aged 34 weeks. RESULTS: In cleft palates, the muscular fibres have an ab

Total mandibular alveolar osteotomy: an alternate choice to other surgical procedures.

Patients whose facial esthetics are not severely compromised but who need surgical intervention for the correction of a dentoskeletal problem can be treated by repositioning of the whole mandibular alveolar segment in the direction needed to allow for such correction. The challenge to achieve efficient and relatively stable results with the use of combined orthodontic and surgical methods has been met by the use of various surgical techniques. The lower total alveolar osteotomy is another viable surgical technique that could be considered when treatment is being planned in an orthognathic surg

Genial swing orthomorphic osteotomy: a novel technique for retrogenia-associated obstructive sleep apnoea.

Retrogenia, despite its anatomical significance, remains under-recognized as a contributing factor in obstructive sleep apnoea (OSA). While retrognathia and micrognathia are well-documented, retrogenia as an isolated morphological variant has received limited attention in the context of OSA. This technical note introduces the genial swing orthomorphic osteotomy (GSOO), a novel orthomorphic surgical technique specifically designed to address dysmorphology associated with retrogenia. The procedure repositions the genial segment to enhance posterior airway space, while also achieving favourable f

Total alloplastic temporomandibular joint replacement in an adolescent with bilateral ankylosis and revision to all-titanium components.

Temporomandibular joint (TMJ) ankylosis in growing patients may result in severe mandibular hypomobility, progressive dentofacial deformity and upper airway compromise. The selection of reconstructive treatment remains challenging because autologous reconstruction offers growth potential, whereas alloplastic replacement enables immediate mobilization and precise correction of complex deformities. We report the staged management of a 15-year-old female patient with bilateral destructive TMJ ankylosis, marked mandibular retrognathia and tracheostomy-dependent airway obstruction. Treatment consis

Facial Phenotypic Pattern in Severe Congenital Neutropenia Type 4 (G6PC3 Deficiency): Description and Clinical Value.

Severe congenital neutropenia type 4 (SCN4) is an autosomal recessive disorder caused by mutations in the third subunit of the enzyme glucose-6-phosphatase (G6PC3). Patients with mutations in G6PC3 suffer from congenital neutropenia and additional complex developmental abnormalities. Previous reports have found that SCN4 has recognizable dysmorphic facial features. Several features have been described in patients with SCN4, including triangular or round face, full cheeks, midface hypoplasia, malar flattening, frontal bossing, broad forehead, deep-seated eyes, depressed nasal bridge, high-arche

Correcting Class III Malocclusion Using Multiloop Edgewise Archwire (MEAW) Combines Temporary Anchorage Devices (TADs) With Modified Class III Elastics for Camouflage Treatment of in Adolescent: A Case Report.

Introduction: One of the most difficult types of malocclusion to cure is Class III malocclusion. Maxillary protraction is a common component of orthodontic/surgical treatments for Class III individuals since maxillary retrognathia is the primary factor contributing to their condition. Objective: A Vietnamese girl of 16 years old presented with severe symptoms of anterior crossbite and Class III malocclusion of the skeletal and dental structures. Among the dental characteristics were a prognathic mandible, retrognathic maxilla, and proclined lower incisors. The medical history revealed no abnor

Successful management and stability of maxillary class II dentoalveolar protrusion with extractions of upper second and lower third molars, using en-masse distalization and vertical control with bone miniscrews: Adult case report with 7-year follow-up.

This case report describes the successful long-term management of a 19-year-old female patient presenting with a skeletal Class II pattern, mild anterior open bite, and mandibular retrognathia. The orthodontic treatment approach involved the distal movement of the maxillary and mandibular dentitions through the extraction of the maxillary second molars and mandibular third molars, combined with the use of extra-alveolar infrazygomatic crest (IZC) and buccal shelf (BS) miniscrews for anchorage. The treatment outcome achieved a stable, well-aligned dentition with ideal intercuspation and an impr

An interstitial de-novo microdeletion of 3q26.33q27.3 causing severe intrauterine growth retardation.

Chromosomal abnormalities involving an interstitial or a terminal deletion of 3q26.33 and/or 3q27 have rarely been described. Here we report on a fetus of 22+1 weeks' gestational age with severe intrauterine growth restriction and multiple abnormalities detected by ultrasound examination. Post-mortem molecular cytogenetic investigation (array-comparative genomic hybridization) identified a de-novo interstitial ∼6.17 Mbp microdeletion of 3q26.33q27.3. The clinical and molecular findings in this patient are compared with the previous literature on cases with overlapping interstitial 3q-deletions

Molecular characterization of microduplication 22q11.2 in a girl with hypernasal speech.

We present a 12-year-old girl with karyotype 46,XX. A comparative genomic hybridization array revealed a 3.172-Mb microduplication on 22q11.2. This chromosome 22q11.2 region microduplication has been described in patients with variable phenotypes; a large majority of them have identical 3-Mb duplications. The girl presented mild mental motor retardation, facial dysmorphism consisting of a long narrow face, widely spaced eyes, downslanting palpebral fissures, broad nasal base, short philtrum, thin upper lip, micro/retrognathia, low set and retroverted ears, microcephaly, high-arched palate, hyp

Correlations between cephalic posture and facial type in patients suffering from breathing obstructive syndrome.

AIM: In this teleradiographic study, the authors investigate the correlations between facial kind, breathing pattern and cephalic posture. METHODS The research was conducted on a 94 schoolchildren sample. For each patient teleradiographic analysis in-L-L projection and rhinomanometric examination were performed. On the basis of clinical evidences, rhinomanometric results and nasopharyngeal space evaluation, the sample was divided into two groups: the first one included 36 oral breather patients (OB), the second one 58 nasal breathers (NB). RESULTS: The two groups do not differ significantly,

A case with mosaic partial duplication of 1q: prenatal and postmortem clinical and cytogenetic evaluations.

Partial trisomy 1q including different segments of the long arm is a rare cytogenetic anomaly. Especially the cases with mosaic proximal tandem duplication of 1q included a longer fragment are very rare. Cases who have partial 1q trisomy showed large phenotypic variation due to the differences in size of the duplicated segments of 1q. The clinical phenotype of most cases is characterized by multiple congenital anomalies especially including central nervous system and developmental delay. We describe a prenatally diagnosed case with mild cerebral ventriculomegaly and karyotype with mosaic pure

Two craniosynostotic patients with 11q deletions, and review of 48 cases.

Many chromosomal abnormalities have craniofacial manifestations. One such abnormality, partial monosomy of chromosome 11q, is associated with metopic synostosis and resultant trigonocephaly. We reviewed 48 published cases of 11q deletions and translocations. Eighty percent were associated with abnormal head shape. Also commonly found were hypertelorism, ptosis of the eyelids, wide or low nasal bridge, apparently low-set malformed ears, down-turned mouth, micro/retrognathia, digital and cardiac anomalies, and psychomotor retardation. We report on two patients referred for abnormal head shape. T

[Dentofacial morphology of obstructive sleep apnea syndrome patients].

Sleep apnea syndrome is a condition characterized by recurrent interruption of breathing during sleep. Triad of symptoms for the disease are insomnia, daytime sleepiness and snoring. Recently, the patients complained of these symptoms have progressively increased. And so serious attention has been given to investigate the entity of this new clinical syndrome in medical and dental aspects. Three types of sleep apnea are classified; central, obstructive and mixed type. Most of patients identified this syndrome include obstructive or mixed types of sleep apnea. Obstructive sleep apnea has been pr

Diagnosis and management of obstructive sleep apnoea in adults.

Obstructive sleep apnoea is the most common form of sleep-disordered breathing. It is characterised by recurrent occlusion of the airway during sleep. Ensuing apnoeas terminate in arousal from sleep and lead to non-restorative sleep, excessive daytime sleepiness and adverse cardiovascular and neurocognitive effects. A sleep study should be offered to patients reporting witnessed apnoeas or symptoms related to non-restorative sleep. It should also be considered in the presence of predisposing factors for obstructive sleep apnoea (e.g. obesity, tonsillar hypertrophy, retrognathia, refractory hyp

Ehlers-Danlos/myopathy overlap syndrome caused by a large de novo deletion in COL12A1.

Autosomal dominant and recessive mutations in COL12A1 cause the Ehlers-Danlos/myopathy overlap syndrome. Here, we describe a boy with fetal hypokinesia, severe neonatal weakness, striking hyperlaxity, high arched palate, retrognathia, club feet, and pectus excavatum. His motor development was initially delayed but muscle strength improved with time while hyperlaxity remained very severe causing recurrent joint dislocations. Using trio exome sequencing and a copy number variation (CNV) analysis tool, we identified an in-frame de novo heterozygous deletion of the exons 45 to 54 in the COL12A1 ge
