A Simulation Suture Compared With a Clinical Suture for Training on Laparoscopic Simulators: Objective Measurements Indicate Acceptable Physical Characteristics.

OBJECTIVE: Simulation sutures are a low-cost option for training purposes, but they may not perform as well as the more expensive clinical suture. Trainees at our institution have raised concerns about their quality and integrity compared with clinical suture. The objective was to determine whether significant differences in strength of the sutures and knot holding capabilities between low and high-cost sutures existed. METHODS: Two sutures were compared: 3-0 braided silk simulation suture (Sim*Vivo LLC, Willsboro, NY) and 3-0 Perma-Hand silk braided clinical suture (Ethicon, Somerville, NJ).

The Laparoscopy in Biliary Exploration Research and Training Initiative (LIBERTI) trial: simulator-based training for laparoscopic management of choledocholithiasis.

BACKGROUND: Laparoscopic cholecystectomy with common bile duct exploration (LCBDE) is equivalent in safety and efficacy to endoscopic retrograde cholangiopancreatography (ERCP) plus laparoscopic cholecystectomy (LC) while decreasing number of procedures and length of stay (LOS). Despite these advantages LCBDE is infrequently utilized. We hypothesized that formal, simulation-based training in LCBDE would result in increased utilization and improve patient outcomes across participating institutions. METHODS: Data was obtained from an on-going multi-center study in which simulator-based transcys

Comparative Effectiveness of Physical and Virtual Reality Simulators in Robotic Surgical Training.

Introduction: Robotic surgery training requires effective simulation methods to ensure proficiency. Virtual reality (VR) simulators and physical models offer different approaches. Methods: A study was conducted with 30 surgical residents, divided into two groups: one trained on a high-fidelity physical simulator and the other on a VR simulator. Both groups completed standardized exercises, followed by an assessment of their surgical performance using the da Vinci Surgical System simulator. Performance scores were analyzed using statistical methods, including t-tests and multiple linear regress

Exploring the feasibility and effectiveness of simulated root cause analysis for radiology training.

The Accreditation Council for Graduate Medical Education (ACGME) requires radiology residents to engage in an annual safety event that leads to actionable changes in residency programs. Root cause analysis (RCA) is frequently used to fulfill this requirement. Because actual RCA is rare, simulations are a suitable alternative. We evaluated the feasibility and effectiveness of using simulated RCA curriculum to meet the ACGME safety requirement. During the 2023-2024 academic year, quality improvement staff in the radiology department developed a simulated RCA and invited radiology residents to pa

Validation of a three-dimensionally printed simulator for training in endoscopic sinonasal surgery.

Listeria monocytogenes (Listeria) is a Gram-positive facultative intracellular pathogen. Mouse studies typically employ intravenous injection of Listeria, which results in systemic infection. After injection, Listeria quickly disseminates to the spleen and liver due to uptake by CD8α+ dendritic cells and Kupffer cells. Once phagocytosed, various bacterial proteins enable Listeria to escape the phagosome, survive within the cytosol, and infect neighboring cells. During the first three days of infection, different innate immune cells (e.g. monocytes, neutrophils, NK cells, dendritic cells) media

Advanced Manufacturing in the Fabrication of a Lifelike Brain Glioblastoma Simulator for the Training of Neurosurgeons.

Neurosurgeons require considerable expertise and practical experience to deal with the critical situations commonly encountered in complex surgical operations such as cerebral cancer; however, trainees in neurosurgery seldom have the opportunity to develop these skills in the operating room. Physical simulators can give trainees the experience they require. In this study, we adopted advanced molding and replication techniques in the fabrication of a physical simulator for use in practicing the removal of cerebral tumors. Our combination of additive manufacturing and molding technology with ela

Improvement in reading performance through training with simulated thalamic visual prostheses.

Simulations of artificial vision are used to provide the researcher an opportunity to explore different aspects of visual prosthesis device design by observing subject performance on various tasks viewed through the simulation. Such studies typically use normal, sighted subjects to measure performance at a given point in time. Relatively few studies examine performance changes longitudinally to quantitatively assess the benefits from a training plan that would be akin to post-implantation rehabilitation. Here, we had six normal, sighted subjects use a standard reading task with daily practice

Observations from 450 shoulder dystocia simulations: lessons for skills training.

Poor neonatal outcomes after shoulder dystocia have been associated with inappropriate management. Until there are significant developments in the prediction and subsequent prevention of shoulder dystocia, improving shoulder dystocia management through practical training may be the most effective method of reducing the associated morbidity and mortality. Four hundred fifty simulated shoulder dystocia scenarios, managed by 95 midwives and 45 doctors from six U.K. hospitals during the course of 1 year, were video recorded during a study of obstetric emergency training. Analysis of recorded data

Effect of simulator training on driving after stroke: a randomized controlled trial.

BACKGROUND: Neurologically impaired persons seem to benefit from driving-training programs, but there is no convincing evidence to support this notion. The authors therefore investigated the effect of simulator-based training on driving after stroke. METHODS: Eighty-three first-ever subacute stroke patients entered a 5-week 15-hour training program in which they were randomly allocated to either an experimental (simulator-based training) or control (driving-related cognitive tasks) group. Performance in off-road evaluations and an on-road test were used to assess the driving ability of subjec

A Novel Pelvic Hand-Sewn Bowel Anastomosis Simulator for Surgical Training.

Hand-sewn bowel anastomosis (HSBA) is an important skill in general surgery. Due to technological advances such as surgical staplers for bowel, HSBA has become less common thus reducing general surgery residents' exposure to the technique. Difficult anatomical locations such as the pelvis face an even lower clinical exposure, though proficiency is crucial as consequences can include significant morbidity and mortality. Opportunities to develop this skill has been delegated primarily to surgical simulators. Given that cadaveric and porcine training is cost prohibitive and not readily available,

Numerical simulation and parallel implementation of freight train air brake system.

Numerical simulations of railway air brake systems are becoming increasingly computationally demanding due to the growing complexity and length of trains. This study introduces a parallel computing-enhanced model for simulating pressure dynamics in heavy-haul trains. The proposed approach assigns dedicated threads to each vehicle, with additional threads managing the interface fluxes of brake pipe connections, and is implemented in C# using the ThreadPool and Parallel libraries. A train configuration consisting of one locomotive and 116 vehicles was simulated under various brake pipe pressure
