OBJECTIVE: To compare plain soap, alcohol hand rub and iodophors as hand hygiene measures in a neonatal intensive care unit (NICU).
DESIGN: Randomized, crossover, three-armed, controlled trial with blinded outcome measurement.
SETTING: Level III NICU.
PARTICIPANTS: 35 NICU nurses.
INTERVENTION: Participants were assigned to plain soap hand washing, alcohol hand rub and povidone-iodine hand scrub by a random cross-over design. Interventions were preceded by 14-day neutral periods. Cultures from hands were taken before and after each hand-hygiene use, prior to 5 patient-care activities.

A patient with acute pancreatitis developed subcutaneous fat necrosis of the anterior abdominal wall secondary to leakage of pancreatic enzymes through a rent in the peritoneum following paracentesis. The same patient also had another subcutaneous complication of pancreatitis, namely, nodular liquifying panniculitis of the lower extremities. The diagnosis was made by the typical histological findings of subcutaneous fat necrosis, foci of necrotic cells with a "ghost-like" appearance, and basophilic-staining calcium soaps deposited around the necrotic cells.

Hand washing is effective for the prevention and transmission of pathogens but washing with water only is not effective at reducing contamination. The study focussed on the evaluation of hand washing behaviour in Basic schools (Primary and Junior High Schools) in Ghana. Patch testing is performed to find out the causative allergens for patients, and once they are found, to indicate the exclusive usage of daily necessities that do not contain the causative allergens is necessary, such as when the contact dermatitis is recurrent, persistent, or complicated by secondary hyperpigmentation. Such a procedure has been called "allergen control." Sufficient success has been obtained in the categories of soaps, shampoos, cosmetics, underwear, and some dental metals in Japan with cooperation of industries, and summaries of these investigations are reported.

Prior to the late 1970s, cosmetic dermatitis was quite common in Japan. Once sensitizers were established, in cosmetics, soaps, shampoos, and other commonly used substances, effective treatment was only possible if the cycle of recrudescence could be broken. The Allergen Controlled System accomplished this by preventing contact with the main contact sensitizers. Although the number of cases of pigmented cosmetic dermatitis has declined because of restrictions on the use of several major sensitizers, the Allergen Controlled System remains an effective treatment for the cases that still occur.

Plica neuropathica also known as "birds nest" is irreversible matting of hair seen in otherwise healthy individuals and is usually associated with use of ionic/herbal soaps, shampoos, parasitic infestations, psychiatric alterations, or neglect. Anagen effluvium is a known side effect of many immunosuppressive drugs. Rarely matting of hair has been reported following azathioprine. We report plica neuropathica in two chronically ill patients on immunosuppressive drugs, that is, azathioprine and methotrexate along with a review of literature of its etiological factors and pathogenesis.
