A comprehensive assessment of predictors of fertility outcomes in men with non-obstructive azoospermia undergoing microdissection testicular sperm extraction.

BACKGROUND: Microdissection testicular sperm extraction (microTESE) in men with non-obstructive azoospermia (NOA) is the procedure that results in the highest number of sperm cells retrieved for in vitro fertilization (IVF). This study presents a novel assessment of predictors of sperm retrieval as well as downstream embryology and pregnancy outcomes in cases of men with NOA undergoing microTESE. METHODS: A retrospective chart review of 72 men who underwent microTESE for predictors of fertility outcomes including sperm retrieved at microTESE, embryology progression to embryo transfer (ET), cl

The management of obstructive azoospermia: a committee opinion.

Infertility due to obstructive azoospermia may be treated effectively by surgical reconstruction or by retrieval of sperm from the epididymis or testis, followed by in vitro fertilization with intracytoplasmic sperm injection. This replaces the ASRM documents titled "Sperm retrieval for obstructive azoospermia" and "The management of infertility due to obstructive azoospermia," last published in 2008.

Ethical and Legal Considerations in Posthumous Oocyte Retrieval for Minor Girls.

Posthumous gamete retrieval has emerged as a complex issue at the intersection of reproductive technology, bioethics, consent, and law. While posthumous sperm retrieval has received small amounts of legal, ethical, and clinical attention, posthumous oocyte retrieval, particularly in minors, remains largely unaddressed. This review examines the clinical, legal, and ethical considerations surrounding posthumous oocyte retrieval in minor girls. First, the paper explores the medical processes involved in posthumous gamete retrieval, drawing a significant distinction between sperm and oocyte retrie

Methods of surgical sperm extraction and implications for assisted reproductive technology success.

Intracytoplasmic sperm injection offers a chance to surpass severe forms of male factor infertility, including azoospermia. Retrieval of the male gamete from the epididymis or testis provides the chance for biological parenthood for the affected men. In this review, we scrutinize the recent evidence about the surgical sperm retrieval methods for use in association with ICSI. We provide a historical overview of the surgical sperm retrieval methods development, its indication in both azoospermic and non-azoospermic men, and the technical aspects of each method. We also present and critically dis

Age as only predictive factor for successful sperm recovery in patients with Klinefelter's syndrome.

The study was performed to determine factors affecting successful sperm retrieval by testicular sperm extraction in patients with nonmosaic Klinefelter's syndrome (KS). From May 2001 to February 2007, 27 azoospermic patients were diagnosed as having nonmosaic KS. All patients underwent sperm testicular extraction. Patient's age, testicular volume, serum follicle-stimulating hormone (FSH) and inhibin B were assessed as predictive factors for successful sperm recovery. Of the 27 Klinefelter's patients examined, eight (29.6%) had successful sperm recovery. The comparisons of serum FSH, inhibin B

Proteomic biomarkers in seminal plasma as predictors of reproductive potential in azoospermic men.

INTRODUCTION: Azoospermia, characterized by an absence of sperm in the ejaculate, represents the most severe form of male infertility. While surgical sperm retrieval in obstructive azoospermia (OA) is successful in the majority of cases, patients with non-obstructive azoospermia (NOA) show retrieval rates of only about 50% and thus frequently have unnecessary surgery. Surgical intervention could be avoided if patients without preserved spermatogenesis are identified preoperatively. This prospective study aimed to discover biomarkers in seminal plasma that could be employed for a non-invasive d

[Surgical testicular extraction of spermatozoa in men with non-obstructive azoospermia].

Non-obstructive azoospermia (NOA) is a severe form of male infertility. The only option to help men with NOA to become biological fathers is to surgically extract spermatozoa from the testicles, and in this review different modalities are discussed. Microdissection testicular sperm extraction seems to achieve better sperm retrieval rates compared with both testicular sperm aspiration and testicular sperm extraction. However, there are significant limitations in the current literature, and without prospective randomised trials it is not possible to define the optimal sperm retrieval technique f

Microdissection testicular sperm extraction: an update.

Patients with non-obstructive azoospermia (NOA) were once considered to be infertile with few treatment options due to the absence of sperm in the ejaculate. In the last two decades, the advent of intracytoplasmic sperm injection (ICSI), and the application of various testicular sperm retrieval techniques, including fine needle aspiration (FNA), conventional testicular sperm extraction (TESE) and microdissection testicular sperm extraction (micro-TESE) have revolutionized treatment in this group of men. Because most men with NOA will have isolated regions of spermatogenesis within the testis,

Surgical recovery of sperm in non-obstructive azoospermia.

The development of intracytoplasmic sperm injection (ICSI) opened a new era in the field of assisted reproduction and revolutionized the assisted reproductive technology protocols for couples with male factor infertility. Fertilisation and pregnancies can be achieved with spermatozoa recovered not only from the ejaculate but also from the seminiferous tubules. The most common methods for retrieving testicular sperm in non-obstructive azoospermia (NOA) are testicular sperm aspiration (TESA: needle/fine needle aspiration) and open testicular biopsy (testicular sperm extraction: TESE). The optima

Reassessing reconstruction in the management of obstructive azoospermia: reconstruction or sperm acquisition?

Treatments for male factor infertility secondary to reconstructable obstructive azoospermia include either surgical reconstruction or direct sperm retrieval. We examine the risks and benefits of both types of therapies and discuss their respective medical and economic implications. Most male factor infertility studies comparing vasectomy reversal with sperm retrieval favor the former as the more cost-effective therapy for obstructive azoospermia. Analysis should include assessment of direct procedural costs and indirect costs, including the cost of complications, lost productivity, and multipl

Non-obstructive azoospermia: Micro-TESE combined with advanced sperm processing for enhanced assisted reproductive outcomes.

INTRODUCTION: Non-obstructive azoospermia (NOA), which accounts for 10% of male infertility cases, is managed by microdissection testicular sperm extraction (mTESE), currently consider the gold standard technique. While mTESE improves sperm retrieval rates (SRR) and minimizes testicular damage, this is only the first step in the assisted reproductive technology (ART) pathway, whose goal is live birth. Advanced sperm preparation techniques, including SpermVD® vitrification and synchronous oocyte retrieval, may further optimize outcomes. However, live birth and ongoing pregnancy rates (LBR) afte

Fertility in men with Klinefelter's syndrome.

Patients with a Klinefelter syndrome (KS), defined by a 47 XXY karyotype, were long considered infertile. Testicular sperm extraction (TESE) now allows them to access fatherhood. We will present the data of studies since first experiment of TESE. Several factors influencing TESE outcome were proposed in these different studies. Among them, clinical and hormonal parameters have reported by few studies, age has been one of the most discussed prognostic factor of positive sperm retrieval rate. Data seems to show that TESE carried out before an age greater than 30 has a poorer prognosis for positi

Cryopreservation of testicular and epididymal sperm: techniques and clinical outcomes of assisted conception.

The introduction of the technique of intracytoplasmic sperm injection to achieve fertilization, especially using surgically retrieved testicular or epididymal sperm from men with obstructive or non-obstructive azoospermia, has revolutionized the field of assisted reproduction. The techniques for the retrieval of spermatozoa vary from relatively simple percutaneous sperm aspiration to open excision (testicular biopsy) and the more invasive Micro-TESE. The probability of retrieving spermatozoa can be as high as 100% in men with obstructive azoospermia (congenital bilateral absence of the vas def
