Infant sucking ability, non-organic failure to thrive, maternal characteristics, and feeding practices: a prospective cohort study.

This prospective study examined the relation of neonatal sucking to later feeding, postnatal growth, maternal postpartum depression, and feeding practices. Healthy infants of at least 37 weeks gestational age were recruited. At 1 week of age, a strain-gage device was attached to the infant's cheeks during sucking to identify sucking efficiency. Two-hundred and two infants (100 males, 102 females; mean age 39.6 weeks, SD 1.1 weeks) with efficient sucking and 207 (101 males, 106 females; mean gestational age 39.4 weeks, SD 1.2 weeks) with inefficient sucking were identified. Growth was measured

Management of severe feeding dysfunction in children with fetal alcohol syndrome.

In the past three years, we have seen three children who required prolonged inpatient admissions for severe feeding dysfunction resulting in nutritional and growth problems. The pregnancies were significant for severe maternal alcohol abuse during the entire pregnancy. All of the subjects had multiple physical findings and dysmorphic features seen in children with mild to severe fetal alcohol syndrome. Feeding evaluations showed significant delays in oral motor development. Specific oral motor dysfunction was not a commonality and did not appear to be a major determinant in delays in feeding d

Upper Lip Frenulum Findings and Breastfeeding Problems in Healthy Newborns.

IMPORTANCE: Breastfeeding improves health outcomes in neonates. However, infants' oral anatomy may alter latch and sucking mechanics, hampering breastfeeding. The role of maxillary frenulum (upper lip-tie) has remained controversial. OBJECTIVE: To investigate the association between upper lip-tie clinical findings, such as thickness, attachment, and upper lip mobility, and early breastfeeding problems. DESIGN, SETTING, AND PARTICIPANTS: In this prospective cohort study (Lingual Frenulum in Newborn Infants substudy), consecutive mother-infant dyads were studied from August 2, 2023, to January

Characterization and diagnosis of feeding difficulties in full-term neonates in NICUs: a multicenter study.

BACKGROUND: Feeding difficulties significantly impact neonatal well-being. This study explores the clinical characteristics, etiologies, and diagnostic practices for full-term neonates with feeding difficulties in neonatal intensive care units (NICUs). METHODS: This retrospective cohort study recruited full-term infants admitted to NICUs participating in the China Neonatal Genome Project from March 2017 to December 2021, diagnosed with feeding difficulties persisting >72 h. RESULTS: Among 220 patients, the most common symptoms were poor sucking (39.5%), vomiting (22.3%), and dysphagia (14.1%

Upper airway size while sucking on a pacifier in an infant with micrognathia.

In an infant with micrognathia, who was being evaluated after an apparent life-threatening event, respiration timed lateral radiographs showed an increase in the width of the middle airway space during inspiration from 2 mm without the pacifier to 9 mm while sucking on a pacifier. This observation is consistent with the hypothesis that the well-documented association between the pacifier use and reduced risk of sudden infant death syndrome may be at least in part related to changes in airway size.

Mandibular asymmetry and breastfeeding problems: experience from 11 cases.

The authors review the experiences of 11 mother-infant pairs who had breastfeeding problems related to the infants' mandibular asymmetry. Lower jaw asymmetry is an early identifiable sign of torticollis, and a possible contributor to latch difficulties, nipple pain, and poor milk transfer. Pediatricians and lactation consultants should look for signs of lower jaw asymmetry combined with a preference for turning the head to one side in newborns who present with breastfeeding difficulties. By recognizing these anatomic irregularities early in the course of breastfeeding, pediatric health care pr

Chronic alcohol exposure and lactation: extended observations.

Previous results from our laboratory have indicated that chronic (8 days) alcohol administration inhibits suckling-induced prolactin (PRL) release in response to 30 min of suckling. In addition, chronic alcohol administration to dams resulted in growth retardation of their litters. The present study was done to examine how an extended period of suckling (120 min) affected suckling-induced PRL release after chronic alcohol exposure. In addition, it was also examined whether the growth retardation observed during alcohol exposure persisted after alcohol infusions were discontinued. Dams were imp

The early social environment of premature and fullterm infants.

The behaviors of 10 mothers of prematures and 29 mothers of fullterms were compared from 7-h observations made in the home when the infants were 2, 3, 4 and 5 weeks post-term. The observation day was divided into four mutually exclusive interactional contexts that together made up the total day: feeding time, changing or bathing time, time when the mother and infant were in physical contact but the mother was not caretaking, and time the infant was alone. Measures of ten maternal behaviors were also compared: moving, rocking, patting, caressing, talking, looking, engaging in vis-a-vis with the

Effect of Single-Use, Laser-Cut, Slow-Flow Nipples on Respiration and Milk Ingestion in Preterm Infants.

PURPOSE: Single-use, laser-cut, slow-flow nipples were evaluated for their effect on respiration and milk ingestion in 13 healthy preterm infants (32.7-37.1 weeks postmenstrual age) under nonlaboratory, clinical conditions. METHOD: The primary outcomes of minute ventilation and overall milk transfer were measured by using integrated nasal airflow and volume-calibrated bottles during suck bursts and suck burst breaks during slow-flow and standard-flow nipple bottle feedings. Wilcoxon signed-ranks tests were used to test the effect of nipple type on both outcomes. RESULTS: Prefeeding minute ve

MOLECULAR CHARACTERIZATION OF ORNITHOSTRONGYLUS QUADRIRADIATUS ISOLATED FROM A RACING PIGEON (COLUMBA LIVIA DOMESTICA).

An infestation of Ornithostrongylus quadriradiatus, has been found in a racing pigeon Columba livia domestica). In addition, this individual had a mild infection with Trichomonas spp. and Eimeria spp. The bird showed symptoms of increased appetite; unformed, dark- green feces; and lack of typical male behavior, but the general condition of this pigeon was good. The pigeon was euthanized because of a permanent inability to fly due to an injury to the wing. A postmortem examination revealed the presence of very small dark-pink twisted nematodes with the morphology of O. quadriradiatus in the sma

[Applying Developmental Care Theory to the Improvement of Self-Regulation and Feeding Difficulties in a Late Preterm Infant Receiving Cardiac Surgery: A Nursing Experience].

When preterm infants present with organ anomalies that require immediate surgical intervention after birth, their immature nervous systems often limit their ability to cope with the stress of surgery and subsequent treatments. This often results in difficulties with self-regulation and related health issues. The case described in this study involves a late preterm female infant born at 36+4 weeks of gestation who underwent cardiac surgery on the fifth day after birth. Postoperatively, she exhibited neonatal neurodevelopmental disorganization, ineffective suck-swallow reflexes, and inadequate n

The reliability of the Neonatal Oral-Motor Assessment Scale.

OBJECTIVES: Sucking problems in preterm infants can be specified by means of visual observation. The Neonatal Oral-Motor Assessment Scale (NOMAS) is the visual observation method most commonly used to assess the non-nutritive sucking (NNS) and nutritive sucking (NS) skills of infants up to approximately 8 weeks postterm. During the first 2 min of a regular feeding the infant's sucking skill is assessed, either immediately or on video. Although NOMAS has been used since 1993, little is known about the method's reliability. The aim of our study was to determine the test-retest and inter-rater re

The prevalence of ankyloglossia in 302 newborns with breastfeeding problems and sucking difficulties in Barcelona: a descriptive study.

AIM: Oral and maxillofacial development is influenced by the lingual frenulum and also affects breathing, occlusion, sucking, swallowing, speech, among others. Ankyloglossia in the newborn may result in breastfeeding difficulties: maternal nipple pain and/or erosion or mastitis, poor weight gain and excessively long breastfeeds. The main objective of this work is to study the prevalence of ankyloglossia in newborns with breastfeeding difficulties. MATERIALS AND METHODS: This is a transversal descriptive study of 302 patients, between 0 and 6 months, who attended the hospital as a result of br

Quantitative analysis of survival of hypoglossal neurons in neonatally nerve-injured rats: Correlation with milk intake.

INTRODUCTION: Tongue movement innervated by the hypoglossal (XII) nerve is essential for the survival of neonatal rats. The pups with bilateral XII nerve resection failed to suckle milk and did not survive, and the pups with unilateral XII nerve resection showed disturbed suckling capability and lower survival rates. The present study was performed to investigate the relation between neuronal population and milk intake of developing rats that had received various degrees of crush injuries to the unilateral XII nerve during the neonatal period. METHODS: The right XII nerve of postnatal day 1 (

Breastfeeding the neurologically impaired infant.

Care of neurologically impaired infants usually is complex and challenging. Mothers of these infants who want to breastfeed should be supported, given sound information, and assisted to set realistic expectations. The health care team can provide guidance while continuously assessing the infant's status. When and if the infant will feed from the breast cannot be predicted, but the lactation consultant and other members of the health care team should support the mother to do what she can and wants to do for her infant.

Milk-induced, opioid-mediated antinociception in rats at the time of cesarean delivery.

Four experiments were conducted in rats within 2 hr of cesarean delivery to assess antinociception at birth and its possible opioid bases. Morphine antinociception was established in a dose-dependent fashion (0.0625-5.0 mg/kg bw). Analgesia was naloxone (1.0 mg/kg) reversible. In succeeding experiments, antinociception equivalent to that produced by 0.0625-0.125 mg/kg morphine injections was induced by a single 20-microliters bolus of milk (commercial half-and-half) that was delivered over 1-2 s to the middle of the tongue. This, too, was naloxone reversible. Milk-induced antinociception was m

Respiratory control during nipple feeding in preterm infants.

The present study was designed to investigate the development of respiratory control during feeding in premature infants. Cardiorespiratory disturbances during feeding were evaluated with polygraphic monitoring in 24 premature infants within 1 week of beginning nipple feeds. During the initial study, 15 infants exhibited one or more episodes of short apnea (greater than or equal to 10 sec) and three infants exhibited prolonged apnea (greater than or equal to 20 sec). Bradycardia developed in seven infants; apnea and decreases in oxygen saturation invariably preceded the development of bradycar
