Noninvasive surrogate endpoints in MASH trials: current perspectives and challenges.

BACKGROUND: Metabolic dysfunction-associated steatohepatitis (MASH) is a leading cause of cirrhosis and liver-related morbidity. Current regulatory pathways rely on histology and long-term clinical outcomes, but repeated liver biopsy is noninvasive and poorly reproducible; regulators have signaled interest in noninvasive tests (NITs) as surrogate endpoints. METHODS: This review synthesizes contemporary regulatory perspectives, the evidentiary framework for biomarker qualification, and the published performance of leading noninvasive tests (vibration-controlled transient elastography [VCTE], m

Regulatory Alignment on Surrogate Endpoints for Slowly Progressive Ultra-Rare Neurological Diseases: Bridging the Gap Between Biomarker Science and Global Regulatory Acceptance.

BACKGROUND: For slowly progressive ultra-rare diseases, hard clinical endpoints such as mortality or sustained functional decline are often impractical within feasible trial timeframes. Surrogate biomarkers, including fluid and imaging analytes, offer a pathway to accelerate drug development. Achieving cross-jurisdictional regulatory acceptance for these surrogates remains a profound scientific and policy challenge. METHODS: We conducted a narrative synthesis incorporating regulatory guidance documents from the FDA, EMA, and NMPA, alongside PubMed-indexed literature on surrogate endpoint vali

The Role of a Standardised Two-session Peri-transfer Acupuncture Protocol in Frozen Embryo Transfer for Women with Recurrent Implantation Failure: A Retrospective Cohort Study.

BACKGROUND: Recurrent implantation failure (RIF) remains a major challenge in assisted reproductive technology. Acupuncture has been used as a complementary therapy to improve pregnancy outcomes, but its efficacy in women with RIF remains uncertain. AIM: The aim of this study was to determine whether a standardised two-session peri-transfer acupuncture protocol, compared with no acupuncture, is associated with improved live birth rate in women with RIF undergoing frozen embryo transfer (FET). Clinical pregnancy and ongoing pregnancy were secondary outcomes. SETTINGS AND DESIGN: This retrospe

Parental KIR/HLA-C Profiles and Reproductive Failure: A Phenotype-Stratified Retrospective Analysis of Couples With Fertility Disorders.

Interactions between maternal killer-cell immunoglobulin-like receptors (KIR) and fetal human leukocyte antigen-C (HLA-C) play a key role in implantation and placentation, yet their clinical relevance remains controversial, partly due to heterogeneous phenotyping of reproductive failure. We conducted a retrospective observational study of 73 biologically comparable clinically referred couples undergoing reproductive immunology assessment. Couples involving donor gametes, surrogacy, or multiple male partners were excluded. Clinical phenotypes were classified as isolated recurrent implantation f

Evaluation of Event-Free Survival as a Surrogate Endpoint for Overall Survival in Muscle-Invasive Bladder Cancer Following Neoadjuvant Treatment: A Systematic Review and Meta-Analysis.

INTRODUCTION: Event-free survival (EFS), a common endpoint in neoadjuvant/perioperative oncology trials, allows for accelerated treatment evaluation while awaiting results of overall survival (OS). However, the surrogacy of EFS for OS in muscle-invasive bladder cancer (MIBC) has yet to be established. This meta-analysis evaluated EFS as a surrogate endpoint for OS in neoadjuvant-treated MIBC. METHODS: A systematic literature review identified neoadjuvant or perioperative clinical trials for MIBC reporting both EFS and OS. Treatment effect association was evaluated by assessing the association

Large choledochal cyst of the common bile duct in a 2-month-old girl: a case report.

INTRODUCTION: Choledochal cysts are rare congenital anomalies primarily affecting females, with a higher incidence in Asian populations and often diagnosed in childhood. Characterized by an abnormal pancreaticobiliary union, these cysts can lead to serious complications, including malignancy in about 7.5 % of cases. Surgical excision is the preferred treatment to prevent severe outcomes, highlighting the importance of early diagnosis and long-term follow-up. CASE PRESENTATION: A 2-month-old girl, conceived via egg donation and gestational surrogacy, presented with a 2-week history of jaundice

A descriptive cohort study of pregnancy and parenthood in women with cystic fibrosis.

Women with cystic fibrosis (wwCF) are increasingly undertaking pregnancy. This study assessed the current state of relationships, fertility, pregnancy and parenthood in a total cohort of 217 wwCF. Overall, 64% of wwCF were in long-term heterosexual relationships, 32% were single and 4% were in same-sex relationships; 64 wwCF had 111 children; 97 (87.4%) were conceived naturally and 10 (9%) by assisted reproduction. One woman had two children by surrogacy, one couple adopted a child and six wwCF had a role as a step-parent. Of the 217 wwCF, 31 (14%) died at a mean age of 41.4 years; they had 18

Disease-free survival as a surrogate outcome for localized kidney cancer-A systematic review and meta-analysis.

BACKGROUND: Disease-free survival (DFS) is commonly used as a primary end point in trials for localized renal cell carcinoma (RCC), although its validity as a surrogate for overall survival (OS) remains uncertain. The objective of this study was to evaluate whether DFS is a reliable surrogate end point for OS in this setting. METHODS: The authors conducted a systematic review and trial-level meta-analysis of randomized controlled trials assessing systemic or perioperative therapies in patients with localized RCC. Databases were searched from inception to June 10, 2025. Trial-level surrogacy b

Progression-free survival and objective response rate as a surrogate end point for overall survival in metastatic renal cell carcinoma: A systematic review and meta-analysis.

The objective of this systematic review and meta-analysis was to evaluate the validity of progression-free survival (PFS) and the objective response rate (ORR) as surrogate end points for overall survival (OS) in randomized controlled trials (RCTs) of metastatic renal cell carcinoma. The MEDLINE, Embase, and Cochrane CENTRAL databases were searched from inception to June 10, 2025. Associations between treatment effects on OS, PFS, and ORR were assessed using Pearson correlation coefficients (r). Surrogate threshold effect (STE) analyses determined the minimum PFS hazard ratio (HR) or ORR odds

Evaluating amyloid-beta as a surrogate endpoint in trials of anti-amyloid-beta drugs in Alzheimer's disease: a Bayesian meta-analysis.

Aim: The use of amyloid-beta (Aβ) clearance to support regulatory approvals of drugs in Alzheimer's disease (AD) remains controversial. We evaluate Aβ as a potential trial-level surrogate endpoint for clinical function in AD. Materials & methods: Data on the effectiveness of anti-Aβ monoclonal antibodies (MABs) on Aβ and multiple clinical outcomes were identified from randomized controlled trials through a literature review. A Bayesian bivariate meta-analysis was used to evaluate Aβ as a surrogate endpoint for clinical function across all MABs and for each individual anti-Aβ MAB. The analysis

10 tips on performing economic evaluation in kidney disease.

Nephrology has benefited from a growing body of high-quality clinical evidence, including clinical trials of pharmacological therapies and health service research on alternative care approaches. Consequently, there is an increasing need to perform economic evaluations in kidney disease to inform reimbursement decisions and optimise healthcare spending, thereby improving patient care within budget constraints. Cost-effectiveness assesses if the additional health gains are worth any additional costs by estimating differences in the quality and quantity of life, and the costs, from the point of i

Financial conflicts among physician speakers at the April 12, 2024 Oncology Drug Advisory Meeting: Who decided that MRD can be a novel regulatory endpoint in myeloma?

BACKGROUND: In April 2024, the Oncology Drug Advisory Committee (ODAC) voted to approve minimal residual disease (MRD) as a new regulatory endpoint for multiple myeloma (MM) despite its poor trial-level surrogacy. This is expected to result in faster MM drug approvals, a potential boon for the pharmaceutical companies that make them. This study investigates the prevalence of financial conflicts of interest (FCOIs) with these companies among United States (US)-based physician speakers at the meeting. METHODS: Public data regarding the past 3 years of pharmaceutical company payments to US-based

Analysis of Genitourinary Rhabdomyosarcoma in Phase 3 clinical trials.

PURPOSE: Pediatric Rhabdomyosarcoma (RMS) is a morbid and often lethal condition characterized by a paucity of clinical data. Beyond a detailed risk categorization system, it is unclear if genitourinary (GU) sites (bladder/prostate, paratesticular, female organs) have outcomes distinct from non-GU sites. This study pools primary data from phase-3 clinical trials involving pediatric RMS to evaluate this question. METHODS AND MATERIALS: We obtained primary data from three Children's Oncology Group pediatric RMS trials (NCT00075582, NCT00354835, NCT00354744) evaluating low- (LR), intermediate- (

Outcomes of older patients with follicular lymphoma using individual data from 5922 patients in 18 randomized controlled trials.

Limited data exist to describe the clinical features and outcomes for elderly patients with follicular lymphoma (FL). The Follicular Lymphoma Analysis of Surrogacy Hypothesis (FLASH) group performed a prospectively planned pooled analysis of individual patient data from first-line randomized controlled trials (RCTs) and examined associations between age (≤70 vs >70 years), clinical characteristics, and FL outcomes. We identified 18 multicenter clinical RCTs in the FLASH database that enrolled elderly patients (>70 years). Primary end points were early disease outcomes, CR24 and CR30, and progr
