Impact of voice and swallowing problems in the elderly.

OBJECTIVES: 1) Evaluate the prevalence and quality-of-life impact of voice and swallowing problems in the elderly; 2) determine treatment trends and barriers to treatment. STUDY DESIGN: Cross-sectional study of independent-living residents in two retirement communities. Prevalence of dysphonia and dysphagia, voice-related quality of life (VRQOL), 7-point Likert scale of dysphagia severity, Center for Epidemiologic Studies Depression (CES-D) scale, and barriers to treatment were collected. Spearman correlation and ANOVA statistics were performed. RESULTS: A total of 248 residents responded wi

[Swallowing disorders in the elderly: an underestimated problem].

Swallowing disorders are frequently observed in the elderly and often accompany an organic pathology. Such disorders may quickly constitute a threat for life: inhalation syndrome, pulmonary infection, undernutrition, dehydration. Diagnosing swallowing disorders is difficult and relies on anamnesis, clinical examination combined with paraclinical investigations. However, there is little place for medicinal or surgical treatment, their management being integral and multidisciplinary.

The Swallowing Centre: rationale for a multidisciplinary management.

The need for professional management of dysphagic patients is growing. The scenario of patient care settings spans from the acute ward to chronic care facilities or home, requiring a health care network able to integrate hospital and community resources and optimise human and instrumental resources. This is also valid for Swallowing Centres, where admission, management, treatment and follow-up of discharged patients are a priority. The complexity of symptoms and the specificity of the underlying disease require a multidisciplinary approach to the patient. The coordinator of the Swallowing Cent

[Swallowing phobia: symptoms, diagnosis and treatment].

INTRODUCTION: Choking phobia (or swallowing phobia) is characterized by a fear of swallowing foods, liquids or pills, sometimes after an episode of choking on food. METHODS: Forty-one case reports on swallowing phobia from 1978 to 2005 were studied. Clinical and therapeutic variables of the disorder were studied. RESULTS: It appears to occur more often in females (two-thirds of the cases) and has a high comorbidity with anxiety disorders (panic disorder, 41 %; obsessive conditions, 22 %, and separation anxiety, 15 %). Life-events and eating traumatic antecedents are frequently present (44% a

Anal pain secondary to swallowed bone.

SummarySwallowed foreign bodies can be a source of morbidity or rarely mortality since they can impact in the pharynx and the oesophagus and cause discomfort or even perforate to cause mediastinitis. Most (80% to 90%)1 foreign bodies including swallowed bone pass into the stomach, proceed through the intestines and are passed in stool without problems. Swallowed bone has been documented to cause intestinal perforation2, enterovesical fistula3 and perianal abscesses4. Two cases of swallowed bones, which passed through the alimentary tract to finally cause anal pain and anal fistula, are present

[Strategy for safe nutrition in swallowing disorders].

Swallowing deficits may affect the ability of patients to feed properly and sufficiently, therefore inducing malnutrition. In order to provide effective and durable treatment, a team of experts should evaluate the swallowing function of each individual patient. Treatment protocols must be carefully analyzed, and adapted to progression of the disease. We refer paradigmatically to patients with Huntington disease, although indications may be conveniently extended to other patients with impaired transit of food.

Interrater reliability of videofluoroscopic swallow evaluation.

The past two decades have brought an enormous widening of interest in and knowledge about swallowing disorders. The most frequently used technique for swallow evaluation is X-ray videofluoroscopy. Most interventions are based on this examination. Only a few studies assessing interobserver reliability of videofluoroscopy have been published. The aim of our study was to assess the interobserver reliability of videofluoroscopy for swallow evaluation. Fifty-one consecutive dysphagic patients referred for videofluoroscopy were entered into the study regardless of their underlying disorder. The firs

[Dietetic management of patients with impaired swallowing].

Swallowing deficits may be present in patients with neurological, otolaryngoiatric or internal medicine diseases. These deficits impair their feeding capability, thus inducing malnutrition. Diet therapy in these conditions involves specific instructions for properly choosing, handling and eating food, in order to allow patients to preserve their autonomy and safety while eating, thus preventing malnutrition. We refer paradigmatically to patients with Huntington disease, which is the main neurologic cause of swallowing deficits, although these guidelines may be conveniently extended to other pa

Primary care prescribing. Hard to swallow.

The Department of Health has invited applications from primary care trusts wishing to become pathfinder sites for repeat dispensing schemes. The NHS plan anticipated that a national scheme would be in operation by 2004 Research suggests repeat dispensing is welcomed by patients but does not save large amounts of money. Joint training for all those involved in a scheme is a key issue.

Swallowing syncope: complex mechanisms of the reflex.

A 69-year-old woman was admitted to our hospital for the examination of syncope. When she ate solid food, she had dizziness or loss of consciousness. The ambulatory ECG suggested sino-atrial block during swallowing with a maximum sinus pause of 6 seconds. An electrophysiologic study revealed pre-existing sinus node dysfunction, which was exaggerated by the balloon inflation in the esophagus. Atropine counteracted the slowing of the basal sinus rate induced by esophageal pressure, but it did not block the effect on the maximum sinus node recovery time. This observation suggested that the syncop

Clinical utility of the modified barium swallow.

The purpose of this investigation was to evaluate the immediate and clinically relevant information gained from the modified barium swallow study and to determine the impact of the procedure on patient management. A database containing a nonrandom sample of 608 swallowing studies was reviewed. Results showed that only 10.4% of the studies were classified as normal examinations and aspiration occurred in 32.4%. However, swallowing abnormality without aspiration was recorded in 57.2% of the studies. Five additional outcome variables were assessed: referrals made to other specialties, effectivene

[Electroglottographic evaluation of swallowing in Parkinson's disease].

The feasibility of using electroglottography (EGG) to record and measure aspects of swallowing was tested in 31 patients with Parkinson's disease (72.8 +/- 4.8 years) and 10 control subjects (74.6 +/- 4.4 years). The patients were divided into three groups: those feeling dysphagia frequently (D(+), n = 9), those feeling dysphagia occasionally (D(+/-), n = 8), and those not feeling dysphagia (D(-), n = 14). The instrument delivered a 250 microA, 52 kHz carrier signal to a pair of surface electrodes on each side of the larynx at the level of the thyroid lamina. Tissue impedance across the neck w

Pharyngoesophageal pressure in patients with swallowing disorders.

We developed and tested a pressure transducer to correctly determine swallowing pressure at different sites in the pharynx. In normal individuals three pressure peaks were obtained in the mesopharynx, hypopharynx and cervical esophagus, respectively. A patient with central dysphagia demonstrated markedly low mesopharyngeal and hypopharyngeal swallowing pressure. One tongue cancer patient who had undergone right hemiglossectomy, including partial resection of the root of the tongue and bilateral superior neck dissection, had markedly low swallowing pressure in the mesopharynx and vallecula area

[Swallowing of an amniotic string by a fetus at term].

Amniotic band syndrome is a rare pathology which involves a group of fetal malformations due to the formation of bands between the fetus and the extraembryonic derivatives. These fetal malformations are extremely variable and their type depends upon the period of gestation during which the bands develop. In rare cases, these bands could strangulate the umbilical cord vessel and lead to fetal death. The authors report an exceptional case fo swallowing of the amniotic band by a fetus at term and fetal death due to strangulation of the umbilical cord vessels by amniotic band.

Case report--swallowed piece of archwire.

A fifteen year old healthy, male patient contacted our office stating that whilst eating a bread roll he had swallowed a piece of orthodontic archwire. He complained of stomach pain and was advised to contact his local medical practitioner who had the area radiographed. The radiographic examination revealed the piece of archwire to be lodged in the pylorus. The patient was then referred to a consultant physician who, using a gastroscope and with the orthodontist present, to assist with supervision of the orthodontic appliance during gastroscopy and to determine that the piece removed had not u
